Excisional devices and methods
A platform device for material excision or removal from vascular structures for either handheld or stereotactic table or robotics platform use may comprise a work element or elements configured to selectively open and close at least one articulable beak or scoopula configured to penetrate and remove intra-vascular materials or obstructions or follow a central lumen of another device or over a wire in a longitudinal direction. Flush and vacuum tissue transport mechanisms may be incorporated as well as single or multiple arrays of image guidance elements, directional elements, ablation elements and other interventional assistance elements. A single tube or an inner sheath and an outer sheath which may be co-axially disposed relative to a work element may be configured to actuate a beak or beaks or scoopulas and provisions for simultaneous or differential beak or scoopula closing under their differential rotation may be incorporated.
1 . A device configured to cut an occlusion within a vasculature, comprising:
an outer flexible tube defining a longitudinal axis;
a rotatable scoopula disposed within the outer flexible tube and defining an open side portion;
an inner sheath disposed within the rotatable scoopula and configured to move parallel to the longitudinal axis;
a first pair of first and second articulable beaks, disposed within the inner sheath and rotatable around the longitudinal axis, the first pair of articulable beaks partially extending into the open side portion of the scoopula, and
a second pair of first and second articulable beaks that is smaller than and coaxially disposed relative to the first pair of articulable beaks, the second pair of articulable beaks being configured to advance distally relative to the rotatable scoopula and to cut a pilot hole in the occlusion in the vasculature by selectively assuming an open and an at least partially closed configuration while rotating against the occlusion;
wherein the first pair of articulable beaks are configured to advance over the second pair of articulable beaks and to further cut the occlusion by selectively assuming an open and a at least partially closed configuration while centered on the pilot hole and rotating against the occlusion.

The device of claim 1 , further comprising an expandable and collapsible chamber that is configured to move parallel to the longitudinal axis and disposed at a distal end of a central tube positioned between the first pair of articulable beaks, the chamber being configured to accommodate an imaging device within a volume of fluid contained within the chamber.

The device of claim 2 , further comprising a plurality of curved blades attached to an outer surface of the chamber, the chamber being configured to advance along and rotate about the longitudinal axis and to widen the pilot hole in the occlusion by selectively expanding or collapsing while cutting the occlusion using the curved blades.
4 . The device of claim 2 , wherein the second pair of articulable beaks is disposed at a distal end of the chamber.
5 . The device of claim 2 , wherein the imaging device is one of an optical coherence tomography imaging device and an ultrasound imaging device.
6 . The device of claim 2 , wherein the expandable and collapsible chamber comprises a surface that defines a central lumen that emerges at a distal end of the chamber.
7 . The device of claim 6 , wherein the second pair of articulable beaks are configured to be advanced within the central tube, through the central lumen and past the distal end of the chamber.
8 . The device of claim 1 , further comprising at least one inflatable support element attached to the rotatable scoopula or to the outer flexible tube, the at least one inflatable support element being configured to selectively anchor the device within the vasculature.
9 . The device of claim 8 , wherein the at least one inflatable support element comprises an expandable cuff attached to the flexible tube, the expandable cuff being configured to selectively enable a blood flow in the vasculature or seal off the blood flow in the vasculature.
10 . The device of claim 8 , wherein the at least one inflatable support element is attached to an outer surface of the rotatable scoopula and comprises a first inflatable support element and a second inflatable support element spaced apart from the first inflatable support to define a channel therebetween, the channel being configured to selectively enable blood flow therethrough.
