Partial view interfraction treatment target motion management using volumetric imaging
A method of the present disclosure includes identifying, based on a reference image, a full motion range of a target, wherein the full motion range of the target defines a full internal target volume (ITV). The method further includes determining, based on a motion image, a partial motion range of the target, wherein the partial motion range is an untracked portion of the full motion range of the target comprising one of an untracked plane or an untracked axis. The method further includes generating, by a processing device, a partial ITV based on the partial range of motion of the target, wherein the partial ITV is a volume swept by the target as the target moves through the partial motion range, and wherein the partial ITV is smaller than the full ITV. The method further includes providing the partial ITV to assist in tracking the target.
1 . A method, comprising:
identifying, based on a plurality of reference images acquired, by a first imaging source having a pre-treatment frame of reference, before or at a beginning of a treatment session, a full motion range of a target, wherein the full motion range of the target defines a full internal target volume (ITV);
acquiring, by a second imaging source different than the first imaging source and having an in-treatment frame of reference, a plurality of three-dimensional (3D) intrafraction motion images during a treatment session, wherein a treatment beam is directed to the target during the treatment session;
determining, based on the plurality of 3D intrafraction motion images acquired during the treatment session, a partial motion range of the target during the treatment session, wherein the partial motion range is an untracked portion of the full motion range of the target comprising one of an untracked plane or an untracked axis;
generating, by a processing device, a partial ITV based on the partial motion range of the target, wherein the partial ITV is a volume swept by the target as the target moves through the partial motion range, and wherein the partial ITV is smaller than the full ITV;
providing the partial ITV to assist in tracking the target; and
volumetrically aligning the partial ITV to the full ITV to reduce baseline shift and setup error associated with the target and to compensate for the untracked plane or the untracked axis, and wherein volumetrically aligning the partial ITV to the full ITV comprises determining a registration between the partial ITV and the full ITV across the pre-treatment frame of reference of the first imaging source and the in-treatment frame of reference of the second imaging source.

The method of claim 1 , wherein the plurality of reference images include a four-dimensional (4D) inhale and exhale planning image, and wherein the 4D inhale and exhale planning image is one of: a kilovoltage computed tomography (kV-CT) image, a magnetic resonance imaging (MRI) image, a kilovoltage cone beam computed tomography (kV- CBCT) image, or a megavoltage computed tomography (MVCT) image.

The method of claim 1 , wherein the plurality of reference images include a three-dimensional (3D) inhale and exhale planning image, and wherein the 3D inhale and exhale planning image is one of: a kilovoltage computed tomography (kV-CT) image, a magnetic resonance imaging (MRI) image, a kilovoltage cone beam computed tomography (kV- CBCT) image, or a megavoltage computed tomography (MVCT) image.
4 . The method of claim 1 , wherein the plurality of 3D intrafraction motion images are one of: kilovoltage cone beam computed tomography (kV-CBCT) images, megavoltage cone beam computed tomography (MV-CBCT) images, or megavoltage computed tomography (MVCT) images.
5 . The method of claim 1 , further comprising: tracking a motion of the target using a two-dimensional motion x-ray image.
6 . The method of claim 1 , wherein the plurality of reference images are generated by the first imaging source and the plurality of 3D intrafraction motion images are generated by the second imaging source, and wherein the first imaging source and the second imaging source are at different positions with respect to a patient.
7 . A system comprising:
a memory; and
a processing device, operatively coupled with the memory, to:
identify, based on a plurality of reference images acquired, by a first imaging source having a pre-treatment frame of reference, before or at a beginning of a treatment session, a full motion range of a target, wherein the full motion range of the target defines a full internal target volume (ITV);
acquire, by a second imaging source different than the first imaging source and having an in-treatment frame of reference, a plurality of three-dimensional (3D) intrafraction motion images during a treatment session, wherein a treatment beam is directed to the target during the treatment session;
determine, based on the plurality of 3D intrafraction motion images acquired during the treatment session, a partial motion range of the target during the treatment session, wherein the partial motion range is an untracked portion of the full motion range of the target comprising one of an untracked plane or an untracked axis;
generate a partial ITV based on the partial motion range of the target, wherein the partial ITV is a volume swept by the target as the target moves through the partial motion range, and wherein the partial ITV is smaller than the full ITV;
provide the partial ITV to assist in tracking the target; and
volumetrically align the partial ITV to the full ITV to reduce baseline shift and setup error associated with the target and to compensate for the untracked plane or the untracked axis, and wherein volumetrically aligning the partial ITV to the full ITV comprises determining a registration between the partial ITV and the full ITV across the pre-treatment frame of reference of the first imaging source and the in-treatment frame of reference of the second imaging source.

The system of claim 7 , wherein the plurality of reference images include a four-dimensional (4D) inhale and exhale planning image, and wherein the 4D inhale and exhale planning image is one of: a kilovoltage computed tomography (kV-CT) image, a magnetic resonance imaging (MRI) image, a kilovoltage cone beam computed tomography (kV- CBCT) image, or a megavoltage computed tomography (MVCT) image.

The system of claim 7 , wherein the plurality of reference images include a three-dimensional (3D) inhale and exhale planning image, and wherein the 3D inhale and exhale planning image is one of: a kilovoltage computed tomography (kV-CT) image, a magnetic resonance imaging (MRI) image, a kilovoltage cone beam computed tomography (kV- CBCT) image, or a megavoltage computed tomography (MVCT) image.
10 . The system of claim 7 , wherein the plurality of 3D intrafraction motion images are one of: kilovoltage cone beam computed tomography (kV-CBCT) images, megavoltage cone beam computed tomography (MV-CBCT) images, or megavoltage computed tomography (MVCT) images.
11 . The system of claim 7 , wherein the processing device is further to: track a motion of the target using a two-dimensional motion x-ray image.
12 . The system of claim 7 , wherein the plurality of reference images are generated by the first imaging source and the plurality of 3D intrafraction motion images are generated by the second imaging source, and wherein the first imaging source and the second imaging source are at different positions with respect to a patient.
13 . A non-transitory computer readable medium comprising instructions that, when executed by a processing device, cause the processing device to:
identify, based on a plurality of reference images acquired, by a first imaging source having a pre-treatment frame of reference, before or at a beginning of a treatment session, a full motion range of a target, wherein the full motion range of the target defines a full internal target volume (ITV);
acquire, by a second imaging source different than the first imaging source and having an in-treatment frame of reference, a plurality of three-dimensional (3D) intrafraction motion images during a treatment session, wherein a treatment beam is directed to the target during the treatment session;
determine, based on the plurality of 3D intrafraction motion images acquired during the treatment session, a partial motion range of the target during the treatment session, wherein the partial motion range is an untracked portion of the full motion range of the target comprising one of an untracked plane or an untracked axis;
generate, by the processing device, a partial ITV based on the partial motion range of the target, wherein the partial ITV is a volume swept by the target as the target moves through the partial motion range, and wherein the partial ITV is smaller than the full ITV;
provide the partial ITV to assist in tracking the target; and
volumetrically align the partial ITV to the full ITV to reduce baseline shift and setup error associated with the target and to compensate for the untracked plane or the untracked axis, and wherein volumetrically aligning the partial ITV to the full ITV comprises determining a registration between the partial ITV and the full ITV across the pre-treatment frame of reference of the first imaging source and the in-treatment frame of reference of the second imaging source.

The non-transitory computer readable medium of claim 13 , wherein the plurality of reference images include a four-dimensional (4D) or a three-dimensional (3D) inhale and exhale planning image, and wherein the 4D inhale and exhale planning image is one of: a kilovoltage computed tomography (kV-CT) image, a magnetic resonance imaging (MRI) image, a kilovoltage cone beam computed tomography (kV-CBCT) image, or a megavoltage computed tomography (MVCT) image.

The non-transitory computer readable medium of claim 13 , wherein the plurality of reference images include a three-dimensional (3D) planning image, and wherein the 3D planning image is one of: a kilovoltage computed tomography (kV-CT) image, a magnetic resonance imaging (MRI) image, a kilovoltage cone beam computed tomography (kV- CBCT) image, or a megavoltage computed tomography (MVCT) image.

The non-transitory computer readable medium of claim 13 , wherein the plurality of 3D intrafraction motion images are one of: kilovoltage cone beam computed tomography (kV-CBCT) images, megavoltage cone beam computed tomography (MV-CBCT) images, or megavoltage computed tomography (MVCT) images.
17 . The non-transitory computer readable medium of claim 13 , wherein the processing device is further to: track a motion of the target using a two-dimensional motion x-ray image.
18 . The non-transitory computer readable medium of claim 13 , wherein to generate the partial ITV the processing device is to project the ITV onto an axis normal to a treatment plane.
19 . The non-transitory computer readable medium of claim 13 , wherein the plurality of 3D intrafraction motion images include a three-dimensional (3D) inhale and exhale intrafraction image.
20 . The non-transitory computer readable medium of claim 13 , wherein the plurality of 3D intrafraction motion images include a four-dimensional (4D) inhale and exhale intrafraction image.
