Abstract

Assemblies of one or more implant structures make possible the achievement of diverse interventions involving the fusion and/or stabilization of lumbar and sacral vertebra in a non-invasive manner, with minimal incision, and without the necessitating the removing the intervertebral disc. The representative lumbar spine interventions, which can be performed on adults or children, include, but are not limited to, lumbar interbody fusion; translaminar lumbar fusion; lumbar facet fusion; trans-iliac lumbar fusion; and the stabilization of a spondylolisthesis.

Claims (17)

1. A method for anterior lumbar interbody fusion comprising creating an insertion path from a first anterolateral region of a selected lumbar vertebra in a caudal direction across an intervertebral disc into an opposite anterolateral region of an adjacent lumbar vertebra, providing a bone fixation implant comprises an elongated implant structure having a rectilinear cross section along at least a proximal portion of the implant structure and including an exterior surface region treated to provide bony in-growth or through-growth along the implant structure, and inserting the bone fixation implant through the insertion path laterally from the anterolateral region of the selected lumbar vertebra in a caudal direction across the intervertebral disc into an opposite anterolateral region of the adjacent lumbar vertebra.

2. A method according to claim 1 wherein the elongated implant structure has a triangular cross section.

3. A method according to claim 1 wherein the insertion path is created in a non-invasive manner without prior removal of the intervertebral disc.

4. A method according to claim 1 wherein the insertion path comprises a bore sized approximately at or approximately about an outer maximum dimension of the bone fixation implant.

A method according to claim 1 wherein at least a first implant structure enters on an anterolateral side of the selected lumbar vertebra, passes caudally through the intervertebral disc, and into an opposite anterolateral region of the adjacent lumbar vertebra, and wherein at least a second implant structure enters on an opposite anterolateral side of the selected lumbar vertebra, passes caudally through the intervertebral disc and into the opposite anterolateral region of the adjacent lumbar vertebra, the first and second implant structures crossing each other in transit through the intervertebral disc.

6. A method according to claim 1 wherein at least one implant structure enters on the an anterolateral side of the lumbar vertebra L4, passes caudally through the intervertebral disc, and into an opposite anterolateral region of the lumbar vertebra L5.

A method according to claim 6 wherein at least one implant structure enters on an opposite anterolateral side of the lumbar vertebra L4, passes caudally through the intervertebral disc and into the opposite anterolateral region of the lumbar vertebra L5, the implant structures crossing each other in transit through the intervertebral disc.

8. A method according to claim 1 wherein the bony in-growth region or through-growth region comprises a porous plasma spray coating on the implant structure.

9. The method according to claim 1 , wherein the proximal portion of the elongated implant structure is configured to engage the cortical bone of the selected lumbar vertebra after implantation and resist rotation.

10. The method according to claim 1 , wherein the first and second implant structures are separate and freely moveable from each other.

11. The method according to claim 1 wherein the first anterolateral region is located on a first lateral side of the spine and the opposite anterolateral region is on a second lateral side of the spine, wherein the first lateral side is opposite the second lateral side.

A system comprising at least one bone fixation implant comprises an elongated implant structure having a rectilinear cross section along at least a proximal portion of the implant structure and including an exterior surface region treated to provide bony in-growth or through-growth along the implant structure inserted through an insertion path laterally from an anterolateral region of the selected lumbar vertebra in a caudal direction across the intervertebral disc into an opposite anterolateral region of an adjacent lumbar vertebra to affect anterior lumbar interbody fusion.

13. A system according to claim 12 wherein the elongated implant structure has a triangular cross section.

14. A system according to claim 13 wherein the bony in-growth region or through-growth region comprises a porous plasma spray coating on the implant structure.

15. A system comprising at least a first implant structure comprising an elongated implant structure having a rectilinear cross section along at least a proximal portion of the implant structure and including an exterior surface region treated to provide bony in-growth or through-growth along the implant structure inserted through an insertion path that enters on an anterolateral side of the selected lumbar vertebra, passes caudally through the intervertebral disc, and into an opposite anterolateral region of the adjacent lumbar vertebra, and at least a second implant structure comprising an elongated implant structure having a rectilinear cross section including an exterior surface region treated to provide bony in-growth or through-growth along the implant structure that enters through an opposite anterolateral side of the selected lumbar vertebra, passes caudally through the intervertebral disc and into the opposite anterolateral region of the adjacent lumbar vertebra, the first and second implant structures crossing each other in transit through the intervertebral disc, affecting anterior lumbar interbody fusion.

16. A system according to claim 15 wherein at least one of the first and second implant structures has a triangular cross section.

17. A system according to claim 15 wherein the bony in-growth region or through-growth region comprises a porous plasma spray coating on the implant structure.
