Abstract

The invention provides methods for treating an obstructed biological conduit that include administering to the conduit an agent that can degrade extracellular matrix of obstructing tissue. Particular methods include delivery of an enzyme or a mixture of several enzymes to the area or region of obstruction wherein the enzyme(s) have the capability to degrade extracellular matrix components within the obstruction thereby restoring the normal flow of transported fluid through the conduit. The invention also includes preventively dilating a section of conduit to minimize the risk of obstruction formation.

Claims (24)

1. A method for treating an obstructed artery or vein in a human subject in need of such treatment, the method comprising:

locally administering to the wall of the artery or vein in the human subject a composition comprising an elastase,

wherein administration of the composition results in proteolysis of elastin in the wall of the artery or vein leading to enlargement of the diameter of the artery or vein.

2. The method of claim 1 , wherein the composition comprises a pancreatic elastase.

3. The method of claim 1 or 2 , wherein the method further comprises inserting a portion of a delivery apparatus into the wall of the artery or vein.

4. The method of claim 1 or 2 , wherein the composition is administered by a catheter.

5. The method of claim 1 or 2 , wherein the artery or vein is obstructed by stenosis.

6. The method of claim 5 , wherein the stenosis permits passage of an insufficient volume of blood prior to the treatment.

7. The method of claim 1 or 2 , wherein the subject is suffering from coronary obstruction.

8. The method of claim 1 or 2 , wherein the artery or vein is obstructed by intimal hyperplasia.

9. The method of claim 1 or 2 , wherein the composition is administered directly into the wall of the artery or vein.

10. A method for treating an artery or vein connected to an arteriovenous hemodialysis graft in a human subject in need of such treatment, the method comprising:

locally administering to the wall of the artery or vein in the human subject a composition comprising an elastase,

wherein administration of the composition results in proteolysis of elastin in the wall of the artery or vein leading to enlargement of the diameter of the artery or vein.

11. The method of claim 10 , wherein the artery or vein is obstructed.

12. The method of claim 11 , wherein the artery or vein is obstructed by intimal hyperplasia.

13. The method of claim 10 , wherein a vein is treated.

14. The method of claim 13 , wherein the vein is obstructed by intimal hyperplasia.

15. A method for treating a vein in a human subject for use in hemodialysis, the method comprising:

locally administering to the wall of the vein in the human subject a composition comprising an elastase,

wherein administration of the composition results in proteolysis of elastin in the wall of the vein leading to enlargement of the diameter of the vein.

16. The method of claim 15 , further comprising connecting the vein to an artery.

17. The method of claim 16 , wherein the vein is connected to the artery via a graft.

18. The method of claim 10 , 11 , 12 , 13 , 14 , 15 , 16 or 17 , wherein the composition comprises a pancreatic elastase.
