Abstract

A method and a device for improving cardiac function are provided. The device is packaged in a collapsed state in an end of a catheter. Portions of a frame construction of the device spring outwardly when the catheter is withdrawn from the device. Anchoring formations on the frame construction secure the frame construction to a myocardium of the heart. A membrane secured to the frame construction then forms a division between volumes of an endocardial cavity of the heart on opposing sides of the membrane.

Claims (49)

1. A method of improving a patient's cardiac function, comprising:

(a) inserting a reinforced membrane having a pressure receiving surface into a left ventricular chamber of the patient's heart;

(b) positioning the membrane within the left ventricular chamber in a partitioning configuration to partition the left ventricular chamber into a main productive portion of the left ventricular chamber and a secondary non-productive portion of the left ventricular chamber with the pressure receiving surface of the membrane defining in part the main productive portion of the left ventricular chamber; and

(c) securing the reinforced membrane within the left ventricular chamber in the partitioning configuration.

2. The method of claim 1 , wherein the ventricular partitioning created by the reinforced membrane decreases tension in tissue in a portion of a heart wall defining in part the non-productive portion of the left ventricle during a pump cycle of the ventricle.

3. The method of claim 1 , wherein the ventricular partitioning by the membrane placement increases cardiac output of the patient's heart.

4. The method of claim 1 , wherein a pressure of at least 60 mm Hg acts on the pressure receiving surface of the reinforced membrane after being positioned in the partitioning configuration within the patient's left ventricle.

5. The method of claim 1 , wherein the reinforced membrane is non-circular.

6. The method of claim 1 , further comprising:

introducing the reinforced membrane in a folded condition through a tubular passage in a catheter into the left ventricle of the patient's heart; and

unfolding the folded reinforced membrane after leaving the catheter, the unfolded reinforced membrane having an area substantially larger than a cross-sectional area of the tubular passage.

The method of claim 6 , wherein the reinforced membrane is reinforced at least in part by a frame construction which has a collapsed state to facilitate advancement through a tubular passage in a catheter and an expanded state after leaving the catheter having a cross dimension substantially larger than a cross dimension of the tubular passage.

8. The method of claim 7 , wherein the membrane is secured to the frame construction while both the frame construction and the membrane are jointly being introduced through the tubular passage.

9. The method of claim 8 , wherein expansion of the frame construction causes the membrane to unfold.

10. The method of claim 7 , wherein the frame construction is anchored at two spaced locations to the wall of the ventricle, thereby retaining the reinforced membrane at a select angle relative to the ventricle.

11. The method of claim 10 , wherein the reinforced membrane is anchored at three spaced locations to the wall of the ventricle, the locations being at corners of a triangle, thereby retaining the reinforced membrane in a select plane.

12. The method of claim 11 , wherein the reinforced membrane is anchored with anchoring formations.

13. The method of claim 12 , wherein at least two of the anchoring formations are anchoring hooks having sharp ends that penetrate the wall of the ventricle.

14. The method of claim 13 , wherein at least two of the anchoring hooks axe distal anchoring hooks on a distal portion of the frame construction and rotate into the wall of the ventricle while a catheter forming the tubular passage is withdrawn off the frame construction.

The method of claim 12 , wherein at least two of the anchoring formations are proximal anchoring hooks on a proximal portion of the frame construction, the proximal portion of the frame construction expanding after leaving the tubular passage, expansion of the proximal portion of the frame construction causing movement of the proximal anchoring hooks away from a center line of the tubular passage so that the proximal anchoring hooks move into contact with the wall of the ventricle.

16. The method of claim 15 , wherein beating of the heart embeds the proximal anchoring hooks into the wall of the left ventricle.

The method of claim 7 , wherein the frame construction includes a main frame that expands away from a center line of the tubular passage, and a support frame which, when the main frame is in its expanded state, is mounted to the main frame within an opening defined by the main frame, the support frame supporting the membrane.

18. The method of claim 17 , wherein the support frame withstands a ventricular pressure of at least 60 mm Hg acting on the membrane.

19. The method of claim 17 , wherein the support frame, when the main frame is in the collapsed state, is collapsed into an elongated arrangement extending along a length of the tubular passage.

20. The method of claim 1 , further comprising:

deforming reinforced membrane into a non-circular shape.

21. The method of claim 1 , wherein the reinforced membrane expands to a diameter of at least 3 cm.

22. The method of claim 1 wherein said inserting occurs endovascularly.

23. A method of improving a patient's cardiac function, comprising:

(a) inserting a reinforced membrane having a pressure receiving surface into a ventricular chamber of the patient's heart;

(b) positioning the reinforced membrane within the ventricular chamber in a partitioning configuration to partition the ventricular chamber into a main productive portion and a secondary non-productive portion which contains an aneurysmic bulge with the pressure receiving surface of the membrane defining in part the main productive portion of the ventricular chamber; and,

(c) securing the reinforced membrane within the ventricular chamber in the partitioning contiguralion whereby the ventricular partitioning created by the reinforced membrane decreases tension in tissue in said non-productive portion containing said aneurysmic bulge during a pump cycle of the ventricle.

24. A method of improving a patient's cardiac function, comprising:

(a) inserting a reinforced membrane having a pressure receiving surface into a ventricular chamber of the patient's heart;

(b) positioning the reinforced membrane within the ventricular chamber in a partitioning configuration to partition the ventricular chamber into a main productive portion and a secondary non-productive ischaernic portion with the pressure receiving surface of the membrane defining in part the main productive portion of the ventricular chamber; and,

(c) securing the reinforced membrane within the ventricular chamber in the partitioning configuration, whereby the ventricular partitioning by the membrane placement increases cardiac output of the patient's heart.

25. A method of improving a patient's cardiac function, comprising:

(a) introducing through a tubular passage in a catheter into a ventricular chamber of the patient's heart a reinforced membrane having an expanded cross sectional area substantially larger than said tubular passage wherein said reinforced membrane comprises a frame construction comprising anchoring formations including at least two distal anchoring hooks on a distal portion of said frame construction and a membrane secured to said frame construction, said frame construction and membrane adapted to be collapsible together within said tubular passage to facilitate advancement therethrough;

(b) positioning said reinforced membrane within said chamber; in a partitioning configuration to partition said chamber into a main productive portion and a secondary non-productive portion;

(c) expanding a proximal portion of the frame construction after said reinforced membrane leaves the tubular passage, wherein said expansion is adapted to unfold said reinforced membrane, wherein a pressure receiving surface of said reinforced membrane defines in part said main productive portion of said chamber, and further wherein said expansion thereby rotates said distal anchoring hooks comprising sharp ends into a wall of said chamber; and

(d) securing said reinforced membrane within said chamber in the partitioning configuration at three spaced locations to said wall of said chamber, said locations being at the corners of a triangle, thereby retaining the reinforced membrane in a select plane.

26. A method of improving a patient's cardiac function, comprising:

(a) inserting a reinforced membrane having a pressure receiving surface into a left ventricular chamber of the patient's heart;

(b) positioning the reinforced membrane within the left ventricular chamber in a partitioning configuration to partition the left ventricular chamber into a main productive portion of the left ventricular chamber and a secondary non-productive portion of the left ventricular chamber which contains a damaged area of myocardium with the pressure receiving surface of the membrane defining in part the main productive portion of the left ventricular chamber; and,

(c) securing the reinforced membrane within the left ventricular chamber in the partitioning configuration whereby the ventricular partitioning created by the reinforced membrane decreases tension in tissue in said non-productive portion containing said damaged area of myocardium during a pump cycle of the ventricle.

27. A method of improving a patient's cardiac function, comprising:

(a) inserting a reinforced membrane having a pressure receiving surface into a left ventricular chamber of the patient's heart;

(b) positioning the reinforced membrane within the left ventricular chamber in a partitioning configuration to partition the left ventricular chamber into a main productive portion of the left ventricular chamber and a secondary non-productive portion of the left ventricular chamber which contains a regional wall motion abnormality with the pressure receiving surface of the membrane defining in part the main productive portion of the left ventricular chamber; and,

(c) securing the reinforced membrane within the left ventricular chamber in the partitioning configuration whereby the ventricular partitioning created by the reinforced membrane decreases tension in tissue in said non-productive portion containing said regional wall motion abnormality during a pump cycle of the ventricle.
