Abstract

Apparatuses and methods for removing solid tissue from beneath a tissue surface are described. The methods rely on positioning an energy conductive element at a target site beneath the tissue surface and energizing the element so that it can vaporize tissue. The element is then moved in a pattern which provides the desired tissue removal geometry, such as spherical, ovoid, or cylindrical. Usually, the shaft is moved, typically rotated or reciprocated, and the energy conductive element is moved relative to the shaft, typically by pivoting a rigid element or bowing a flexible element.

Claims (41)

1. A tissue ablation device comprising:

a shaft having a proximal end, a distal end, and a lumen therethrough;

at least one flexible energy conductive element disposed near the distal end of the shaft, the conductive element being repositionable to vaporize tissue;

an aspiration connector coupled to the lumen near the proximal end of the shaft and configured to connect to an aspiration source to withdraw vaporized tissue from the distal end of the shaft to the aspiration connector; and

a drive mechanism electrically coupled to the conductive element, the drive mechanism configured to bow the conductive element between a substantially linear profile where the element lies directly over the shaft and a series of arcuate profiles continuously spaced progressively away from the shaft, thereby providing a continuous series of arcuate tissue removal paths and further configured to rotate the conductive element about the shaft, wherein the drive mechanism rotates the conductive element while bowing the conductive element between the continuously spaced arcuate profiles while energy is applied to the conductive element thereby vaporizing all tissue in the path of the conductive element such that intact tissue is not withdrawn from the distal end of the shaft.

2. A device as in claim 1 , wherein the shaft is substantially rigid.

3. A device as in claim 2 , wherein the shaft has a diameter in the range from 0.1 mm to 20 mm and a length in the range from 0.5 cm to 50 cm.

4. A device as in claim 1 , wherein the shaft has a diameter in the range from 0.1 mm to 20 mm and a length in the range from 0.5 cm to 50 cm.

5. A device as in claim 4 , wherein the shaft is flexible and has a length in the range from 25 cm to 250 cm.

6. A device as in claim 1 , further comprising a power supply connector disposed near the proximal end of the shaft and configured to electrically couple to the energy conductive element.

7. A device as in claim 6 , further comprising a handle secured to the proximal end of the shaft.

8. A device as in claim 7 , where the aspiration connector and/or the power supply connector are disposed on the handle.

9. A device as in claim 6 , wherein the flexible conductive elements comprise at least one wire electrode and at least one ribbon electrode, wherein the power supply connector provides for connecting the wire and ribbon electrodes to opposite poles of a high frequency electrical power supply.

10. A device as in claim 9 , wherein the drive mechanism advances the wire electrodes radially ahead of the ribbon electrodes.

11. A device as in claim 1 , wherein the drive mechanism is a motor attachable to the proximal end of the shaft.

12. A device as in claim 1 , wherein the energy conductive element comprises an electrode which conducts high frequency electrical energy.

13. A device as in claim 1 , wherein the energy conductive element comprises a heating element.

14. A device as in claim 13 , wherein the heating element comprises an optical fiber for delivering light energy to heat the energy conductive element.

15. A device as in claim 13 , wherein the heating element comprises an electrical resistance heater.

16. A device as in claim 1 , wherein the energy conductive element comprises a wire.

17. A device as in claim 1 , wherein the element comprises a ribbon.

18. A device as in claim 1 , comprising at least two or more energy conductive elements, wherein the drive mechanism is adapted to bow the elements at substantially the same rate.

19. A tissue ablation system comprising:

a tissue ablation device including:

a shaft having a proximal end, a distal end, and a lumen therethrough;

at least one flexible energy conductive element disposed near the distal end of the shaft, the conductive element being repositionable to vaporize tissue;

an aspiration connector coupled to the lumen near the proximal end of the shaft and configured to connect to an aspiration source to withdraw vaporized tissue from the distal end through the lumen; and

a drive mechanism electrically coupled to the conductive element, the drive mechanism configured to bow the conductive element between a substantially linear profile where the element lies directly over the shaft and a series of arcuate profiles continuously spaced progressively away from the shaft, thereby providing a continuous series of arcuate tissue removal paths and further configured to rotate the conductive element about the shaft, wherein the drive mechanism rotates the conductive element while bowing the conductive element between the continuously spaced arcuate profiles while energy is applied to the conductive element thereby vaporizing all tissue in the path of the conductive element such that intact tissue is not withdrawn from the distal end of the shaft; and

an aspiration source connected to the aspiration connector of the tissue ablation device configured to withdraw vaporized tissue from the tissue ablation device.

20. The system of claim 19 , wherein the shaft of the tissue ablation device has a diameter in the range from 0.1 mm to 20 mm and a length in the range from 0.5 cm to 50 cm.

21. The system of claim 19 , where the drive mechanism comprises a motor attachable to the proximal end of the shaft.

22. The system of claim 19 , wherein the energy conductive element of the tissue ablation device comprises an electrode which conducts high frequency electrical energy.

23. The system of claim 19 , wherein the energy conductive element of the tissue ablation device comprises a heating element.

24. The system of claim 23 , wherein the heating element comprises an optical fiber for delivering light energy to heat the energy conductive element.

25. The system of claim 23 , wherein the heating element comprises an electrical resistance heater.

26. The system of claim 19 , wherein the energy conductive element of the tissue ablation device comprises a wire.

27. The system of claim 19 , wherein the element of the tissue ablation device comprises a ribbon.

28. The system of claim 19 , where the tissue ablation device further comprises at least two or more energy conductive elements, wherein the drive mechanism is adapted to bow the elements at substantially the same rate.

29. The system of claim 19 , where the tissue ablation device further comprises a power supply connector disposed near the proximal end of the shaft and configured to electrically couple to the energy conductive element.

30. The system of claim 29 , wherein the flexible conductive elements of the tissue ablation device comprise at least one wire electrode and at least one ribbon electrode, wherein the power supply connector provides for connecting the wire and ribbon electrodes to opposite poles of a high frequency electrical power supply.

31. The system of claim 30 , wherein the drive mechanism of the tissue ablation device advances the wire electrodes radially ahead of the ribbon electrodes.
