Abstract

The present invention relates to injecting a cycline compound into a diseased intervertebral disc.

Claims (28)

1. A method of treating degenerative disc disease in an intervertebral disc having a nucleus pulposus, comprising transdiscally administering a formulation comprising a cycline compound into the intervertebral disc, wherein said cycline compound is selected from the group consisting of lymecycline, doxycycline and minocycline, and wherein said cycline compound inhibits an MMP, TNF-α, or both an MMP and TNF-α, in a non-specific manner.

2. The method of claim 1 , wherein the formulation further comprises a buffer.

3. The method of claim 1 , wherein the cycline compound is administered in an amount effective to inhibit an MMP present in the nucleus pulposus and thereby arrest degradation of an extracellular matrix.

4. The method of claim 1 , wherein the formulation further comprises a second therapeutic agent.

5. The method of claim 1 , wherein the cycline compound is administered in an amount effective to inhibit TNF-α present in the nucleus pulposus and thereby reduce pain.

6. The method of claim 1 , wherein the cycline compound is administered in an amount effective to inhibit TNF-α present in the nucleus pulposus and thereby arrest degradation of an extracellular matrix.

7. The method of claim 1 , wherein the administration comprises providing the formulation in a patch attached to an outer wall of the annulus fibrosus.

8. The method of claim 1 , wherein the administration comprises providing the formulation in a depot at a location closely adjacent to an outer wall of the annulus fibrosus.

9. The method of claim 1 , wherein the administration comprises providing the formulation in a depot at a location closely adjacent to an endplate of an adjacent vertebral body.

10. The method of claim 1 , wherein the intervertebral disc is an intact disc.

11. The method of claim 1 , wherein the formulation is administered in an amount of less than 1 cc.

12. The method of claim 1 , wherein the concentration of the cycline compound is at least about 20 mg/ml.

13. The method of claim 1 , wherein the formulation further comprises a sustained release device.

14. The method of claim 13 , wherein the sustained release device provides controlled release.

15. The method of claim 13 , wherein the sustained release device provides continuous release.

16. The method of claim 13 , wherein the sustained release device provides intermittent release.

17. The method of claim 13 , wherein the sustained release device comprises microspheres.

18. The method of claim 13 , wherein the formulation is provided closely adjacent to the outer wall of the annulus fibrosus.

19. The method of claim 1 , wherein the concentration of cycline compound in the formulation is less than about 100 mg/ml.

20. The method of claim 1 , wherein the formulation further comprises a growth factor present in an amount effective to repair disc tissue.

21. The method of claim 20 , wherein the growth factor is a TGF-β.

22. The method of claim 20 , wherein the growth factor is provided by platelet concentrate.

23. The method of claim 1 , wherein the formulation is injected into the nucleus pulposus.

24. The method of claim 1 , wherein the formulation is injected into the annulus fibrosus.

25. The method of claim 1 , wherein a portion of the nucleus pulposus is removed prior to administering the formulation into the intervertebral disc.

26. The method of claim 1 , wherein the administration is performed through a needle.

27. The method of claim 26 , wherein the needle has a maximum gauge of 24 gauge.

28. The method of claim 1 , wherein the formulation further comprises at least one additional therapeutic agent.
