Abstract

A spinal screw assembly providing an adjustable securement of a fixation rod across at least two vertebrae. The assembly includes a pedicle screw having a spherical head portion, a threaded shaft portion and a tool engagement surface in the head portion for use in driving the screw into a vertebrae. The head portion of the screw is positioned in a body member adjacent a curvilinear surface disposed about an aperture in the end of the body member such that the shaft portion of the screw extends therethrough and the curvilinear inner surface abuts and mates with the head portion of the screw so as to define a ball joint therewith. The body member additionally defines a pair of opposed parallel slots therein adapted to receive a portion of the fixation rod and a locking cap bears against the fixation rod to releasably secure the rod within the assembly.

Claims (32)

1. A variable angle spinal screw assembly adapted to be secured to a vertebrae for providing an adjustable securement of a fixation rod across at least two vertebrae, said assembly comprising:

a pedicle screw having a substantially spherical head portion, a threaded shaft portion and a tool engagement surface defined by the head portion for use in driving the screw into a vertebrae;

a body member for receiving said head portion of said screw therein and defining an opening in an inner end thereof for the extension of said shaft portion of the screw therethrough, a pair of opposed parallel slots therein for receiving a portion of a fixation rod therebetween, a curvilinear interior surface disposed about said opening for abutting and mating with said substantially spherical head portion of said screw so as to allow variable angular movement of said body member with respect to said pedicle screw while maintaining said interior surface of said body member in mating contact with said head portion of said screw;

a locking cap releasably securable within said body member such that said cap bears against the portion of a fixation rod disposed between said slots to secure the rod within said assembly; and

a keyed interface between said pedicle screw and said body member whereby said pedicle screw can be inserted into a vertebrae and said body member subsequently disposed about said substantially spherical head portion of said screw such that said head portion abuts and mates with said curvilinear interior surface of said body member to provide said variable angular movement of said body member with respect to said screw;

wherein said keyed interface comprises a first threaded surface in said body member about said opening in said inner end thereof and a second threaded surface on said head portion of said screw, said first threaded surface being adapted to threadably engage said second threaded surface such that said screw can be inserted into a vertebrae and said body member threaded onto and over said head portion of said screw to position said curvilinear interior surface of said body member such that said interior surface can abut and mate with said body portion.

2. The assembly of claim 1 including means for locking said cap within said body member against the fixation rod upon said cap being pressed into said body member and against the rod and releasing said cap from the rod upon said cap being rotated with respect to said body member.

The assembly of claim 2 wherein a first plurality of axially-spaced locking teeth are formed on said body member and a second plurality of axially-spaced locking are teeth formed on said locking cap, said teeth being configured such that upon aligning said second plurality of teeth with said first plurality and pressing said locking cap into said body member, said pluralities of teeth cooperate to allow said cap to move into said body member to abut the transverse portion of the fixation rod disposed between said slots and engage to prevent withdrawal of said cap until said second plurality of locking teeth are rotated out of axial alignment with said first plurality of teeth, whereupon said cap can be withdrawn from the fixation rod releasing the securement of the fixation rod in said assembly.

The assembly of claim 1 including a bushing locatable within said body member outwardly adjacent the head portion of said screw, said bushing defining a seat for said portion of the fixation rod and a bifurcated skirt for abutting said head portion of said screw upon said locking cap pressing against said portion of the fixation rod whereby the force exerted on the head portion of the screw is distributed about said head portion.

The assembly of claim 1 including a bushing locatable within said body member outwardly adjacent the head portion of said screw, said bushing defining a seat for said portion of the fixation rod and a bifurcated skirt for abutting said head portion of said screw upon said locking cap pressing against said portion of the fixation rod whereby the force exerted on the head portion of the screw is distributed about said head portion.

6. A variable angle spinal screw assembly adapted to be secured to a vertebrae for providing an adjustable securement of a fixation rod across at least two vertebrae, said assembly comprising:

a pedicle screw having a substantially spherical head portion, a threaded shaft portion and a tool engagement surface defined by the head portion for use in driving the screw into a vertebrae;

a body member for receiving said head portion of said screw therein and defining an opening in an inner end thereof for the extension of said shaft portion of the screw therethrough, a pair of opposed parallel slots therein for receiving a portion of a fixation rod therebetween, a curvilinear interior surface disposed about said opening for abutting and mating with said substantially spherical head portion of said screw so as to allow variable angular movement of said body member with respect to said pedicle screw while maintaining said interior surface of said body member in mating contact with said head portion of said screw;

a locking cap releasably securable within said body member such that said cap bears against the portion of a fixation rod disposed between said slots to secure the rod within said assembly; and

a keyed interface between said pedicle screw and said body member whereby said pedicle screw can be inserted into a vertebrae and said body member subsequently disposed about said substantially spherical head portion of said screw such that said head portion abuts and mates with said curvilinear interior surface of said body member to provide said variable angular movement of said body member with respect to said screw;

wherein said keyed interface comprises a first plurality of axially disposed contact surfaces disposed about said opening in said body member, at least one of said surfaces differing in configuration from the other of said first contact surfaces and a second plurality of axially disposed contact surfaces disposed about said substantially spherical head portion of said screw, at least one of said second contact surfaces differing in configuration from the other second contact surfaces and each of said first contact surfaces being identically configured with one of said contact surfaces whereby said body member can be inserted over said head portion of said screw by aligning said identically configured surfaces and whereupon rotation of said body member with respect to said screw misaligns said identically configured contact surfaces whereby said interior curvilinear surface on said body member can abut and mate with said substantially spherical portion of said screw head.

7. The assembly of claim 6 wherein said first and second pluralities of axially disposed contact surfaces each comprise an octagonal configuration wherein one of said surfaces in each of said pluralities is of a different length than the remaining surfaces in said plurality of surfaces.

8. A variable angle spinal screw assembly adapted to be secured to a vertebrae for providing an adjustable securement of a fixation rod across at least two vertebrae, said assembly comprising:

a pedicle screw having a substantially spherical head portion, a threaded shaft portion and a tool engagement surface defined by the head portion for use in driving the screw into a vertebrae;

a body member for receiving said head portion of said screw therein and defining a generally cylindrical side wall, an opening in an inner end of said member for the extension of said shaft portion of the screw therethrough, a pair of opposed parallel slots in said side wall for receiving a portion of a fixation rod therebetween, a curvilinear interior surface disposed about said opening for abutting and mating with said substantially spherical head portion of said screw so as to allow variable angular movement of said body member with respect to said pedicle screw while maintaining said interior surface of said body member in mating contact with said head portion of said screw, and a first plurality of locking teeth formed in said side wall;

a locking cap releasably securable within said body member such that said cap bears against the portion of a fixation rod disposed between said slots to secure the rod within said assembly, said cap defining a second plurality of locking teeth thereon, said pluralities of teeth being configured so as to allow said locking cap to be inserted into said body member to abut the portion of the fixation rod disposed between said slots and engage to prevent withdrawal of said cap until said second plurality of teeth are rotated out of alignment with said first plurality of teeth, whereupon said cap can be withdrawn from the fixation rod releasing the securement of the fixation rod in said assembly; and

a keyed interface between said pedicle screw and said body member whereby said pedicle screw can be inserted into a vertebrae and said body member subsequently disposed about said head portion of said screw such that said head portion abuts and mates with said curvilinear interior surface of said body member to provide said variable angular movement of said body member with respect to said screw;

wherein said keyed interface comprises a first threaded surface in said body member about said opening in said inner end thereof and a second threaded surface on said head portion of said screw, said first threaded surface being adapted to threadably engage said second threaded surface such that said screw can be inserted into a vertebrae and said body member threaded onto and over said head portion of said screw to position said curvilinear interior surface of said body member such that said interior surface can abut and mate with said body portion.

The assembly of claim 8 including a bushing locatable within said body member outwardly adjacent the head portion of said screw, said bushing defining a seat for said portion of the fixation rod and a bifurcated skirt for abutting said head portion of said screw upon said locking cap pressing against said portion of the fixation rod whereby the force exerted on the head portion of the screw is distributed about said head portion.

The assembly of claim 9 including a keyed interface between said pedicle screw and said body member whereby said pedicle screw can be inserted into a vertebrae and said body member subsequently disposed about said head portion of said screw such that said head portion abuts and mates with said curvilinear interior surface of said body member to provide said variable angular movement of said body member with respect to said screw.

11. A variable angle spinal screw assembly adapted to be secured to a vertebrae for providing an adjustable securement of a fixation rod across at least two vertebrae, said assembly comprising:

a pedicle screw having a substantially spherical head portion, a threaded shaft portion and a tool engagement surface defined by the head portion for use in driving the screw into a vertebrae;

a body member for receiving said head portion of said screw therein and defining a generally cylindrical side wall, an opening in an inner end of said member for the extension of said shaft portion of the screw therethrough, a pair of opposed parallel slots in said side wall for receiving a portion of a fixation rod therebetween, a curvilinear interior surface disposed about said opening for abutting and mating with said substantially spherical head portion of said screw so as to allow variable angular movement of said body member with respect to said pedicle screw while maintaining said interior surface of said body member in mating contact with said head portion of said screw, and a first plurality of locking teeth formed in said side wall;

a locking cap releasably securable within said body member such that said cap bears against the portion of a fixation rod disposed between said slots to secure the rod within said assembly, said cap defining a second plurality of locking teeth thereon, said pluralities of teeth being configured so as to allow said locking cap to be inserted into said body member to abut the portion of the fixation rod disposed between said slots and engage to prevent withdrawal of said cap until said second plurality of teeth are rotated out of alignment with said first plurality of teeth, whereupon said cap can be withdrawn from the fixation rod releasing the securement of the fixation rod in said assembly; and

a keyed interface between said pedicle screw and said body member whereby said pedicle screw can be inserted into a vertebrae and said body member subsequently disposed about said head portion of said screw such that said head portion abuts and mates with said curvilinear interior surface of said body member to provide said variable angular movement of said body member with respect to said screw;

wherein said keyed interface comprises a first plurality of axially disposed contact surfaces disposed about said opening in said body member, at least one of said surfaces differing in configuration from the other of said first contact surfaces and a second plurality of axially disposed contact surfaces disposed about said substantially spherical head portion of said screw, at least one of said second contact surfaces differing in configuration from the other second contact surfaces and each of said first contact surfaces being identically configured with one of said contact surfaces whereby said body member can be inserted over said head portion of said screw by aligning said identically configured surfaces and whereupon rotation of said body member with respect to said screw misaligns said identically configured contact surfaces whereby said interior curvilinear surface on said body member can abut and mate with said substantially spherical portion of said screw head.

The assembly of claim 11 wherein said first and second pluralities of axially disposed contact surfaces each comprise an octagonal configuration wherein one of said surfaces in each of said pluralities is of a different length than the remaining surfaces in each of said pluralities of surfaces.
