Abstract

Apparatus and methods for stenting are provided comprising a stent attached to a porous biocompatible material that is permeable to endothelial cell ingrowth, but impermeable to release of emboli of predetermined size. Preferred stent designs are provided, as well as preferred manufacturing techniques. Apparatus and methods are also provided for use at a vessel branching. Moreover, embodiments of the present invention may comprise a coating configured for localized delivery of therapeutic agents. Embodiments of the present invention are expected to provide enhanced embolic protection, improved force distribution, and improved recrossability, while reducing a risk of restenosis and thrombus formation.

Claims (38)

1. A method for stenting comprising the steps of:

providing an apparatus comprising a stent having:

a web structure defining an essentially tubular member expandable from a collapsed delivery configuration toward an expanded deployed configuration;

a plurality of longitudinally adjacent web patterns defining the web structure, the web patterns having alternating concavity, two web patterns defining at least one aperture having at least two circumferentially coterminous intercommunicating substantially S-shaped portions; and

a material disposed about an exterior surface of the stent, the material comprising a multiplicity of pores, the pores having a width greater than about 30 μm to permit endothelization of an interior surface of the stent and less than about 100 μm to inhibit emboli release;

advancing the apparatus in a the collapsed delivery configuration to a treatment site within a patient's vascular system;

deploying the apparatus to an the expanded configuration in which the apparatus engages a wall of the vascular system at the treatment site; and

promoting endothelial cell ingrowth through the multiplicity of pores to cover the interior surface of the stent while the material is configured to retain emboli at the treatment site against the wall of the vascular system.

2. The method according to claim 1 , wherein the material is attached to the outer surface of the stent.

3. The method according to claim 2 , wherein the material is bonded to the outer surface of the stent.

4. The method of claim 1 , wherein providing the apparatus further comprises providing the apparatus having an opening through a lateral surface of the stent material.

5. The method of claim 4 , wherein advancing the apparatus to the treatment site further comprises aligning the opening with a side branch of the patient's vascular system.

6. The method of claim 1 , wherein providing the apparatus further comprises providing the apparatus having a coating comprising a therapeutic agent.

7. The method of claim 1 , wherein deploying the apparatus to the expanded configuration further comprises anchoring the apparatus to a wall of the patient's vessel at the treatment site.

8. A method for improved emboli release, comprising:

providing a tubular expandable member having:

a web structure defining an essentially tubular member expandable from a collapsed delivery configuration toward an expanded deployed configuration;

a plurality of longitudinally adjacent web patterns defining the web structure, the web patterns having alternating concavity, two web patterns defining at least one aperture having at least two circumferentially coterminous intercommunicating substantially S-shaped portions; and

a plurality of struts, the tubular member having an inner surface, an outer surface and proximal and distal ends, a material disposed about the outer surface of the tubular member forming a plurality of pores, the pores configured to have widths in the range of about 30 μm to about 100 μm to permit endothelization of the interior surface of the tubular member while inhibiting emboli release;

advancing the tubular member in a collapsed delivery configuration to a treatment site within a patient's vascular system; and

deploying the apparatus to an expanded configuration in which the apparatus engages a wall of the vascular system at the treatment site.

9. The method according to claim 8 , wherein the material is formed in accordance with at least one of a weaving, knitting, and/or braiding process.

10. The method according to claim 9 , wherein the plurality of pores have a size determined as a function of the relative proximity of the plurality of webs.

11. The method according to claim 8 , wherein the material is attached to at least a portion of the tubular member.

12. The method according to claim 11 , wherein the material is attached with an adhesive.

13. The method according to claim 8 , wherein the material is attached in at least one discrete location along a length of the tubular member.

14. The method according to claim 8 , wherein the average width of the pores is about 80 μm.

15. A method of reducing emboli release from an interventional procedure, comprising:

performing an interventional procedure in a patient's vascular system at a treatment site;

providing a tubular expandable member, the tubular member having:

a web structure defining an essentially tubular member expandable from a collapsed delivery configuration toward an expanded deployed configuration;

a plurality of longitudinally adjacent web patterns defining the web structure, the web patterns having alternating concavity, two web patterns defining at least one aperture having at least two circumferentially coterminous intercommunicating substantially S-shaped portions; and

an inner surface, an outer surface and proximal and distal ends, a material disposed about the outer surface of the tubular member forming a plurality of pores having an average width of about 80 μm;

advancing the tubular member in a collapsed delivery configuration to the treatment site within a patient's vascular system; and

deploying the tubular member to an expanded configuration in which the tubular member engages a wall of the vascular system at the treatment site.

16. The method according to claim 15 , wherein the interventional procedure is balloon angioplasty.

17. The method according to claim 15 , further including the step of performing a second interventional procedure after the step of deploying the tubular member.

18. The method according to claim 15 , wherein the material is attached to at least a portion of the tubular member.
