Abstract

An apparatus and method for placing adjacent vertebrae at a fixed distance and angular relationship relative to each other, fixing said vertebrae in said position by use of a milling block engaging each of said adjacent vertebrae and then using a milling means, the depth, length and excursion of which from side to side are controlled by said apparatus to machine out a defined thickness of bone and a space of defined length, height, width and shape in preparation for receiving an interbody spinal implant or graft of known size and configuration are disclosed.

Claims (32)

1. A method of preparing a spinal disc space between adjacent vertebrae, comprising:

providing a guide sleeve housing and a central distractor, the guide sleeve housing having an opening therethrough;

accessing the disc space;

inserting the central distractor into the disc space between the adjacent vertebrae, the central distractor contacting the adjacent vertebrae from within the disc space;

distracting the disc space with the central distractor;

positioning, after distracting the disc space, the guide sleeve housing in an operative position with respect to the disc space with the central distractor in the disc space, at least a portion of, the guide sleeve housing being positioned in contact with at least a portion of each of the adjacent vertebrae;

providing a guide sleeve having an opening therethrough;

connecting the guide sleeve to the guide sleeve housing with the opening of the guide sleeve housing being in communication with the opening of the guide sleeve; and

performing surgical procedures through the openings of the guide sleeve and the guide sleeve housing.

2. The method of claim 1 , further comprising removably connecting the guide sleeve housing to the central distractor.

3. The method of claim 1 , wherein an axis passes through the openings of the guide sleeve housing and the guide sleeve when the guide sleeve housing and the guide sleeve are connected.

4. An apparatus for use in human spinal surgery across at least a portion of a disc space between two adjacent vertebral bodies of a human spine, said apparatus comprising:

a body;

at least two extensions extending from said body and configured to be inserted into the disc space between the adjacent vertebral bodies, each of said extensions having a leading end for insertion first into the disc space, a longitudinal axis therethrough, and a maximum length, said extensions being joined to each other by a connecting member at said leading ends, said connecting member having a length as measured in a direction perpendicular to the longitudinal axes of said extensions, said connecting member having a maximum width in a direction parallel to the longitudinal axes of said extensions that is less than the maximum length of each of said extensions, each of said extensions having a portion adapted to bear against each of the adjacent endplates of the adjacent vertebral bodies from within the disc space, said portions of said extensions each having an upper surface configured to contact one of the adjacent end plates of the adjacent vertebral bodies and a lower surface configured to contact the other of the adjacent end plates of the adjacent vertebral bodies, said portions of said extensions each having a maximum height between said upper and lower surfaces and a length sufficient to space the adjacent vertebral bodies in a selected relationship to one another; and

an opening between said extensions configured to receive at least a portion of a cutting instrument.

5. The apparatus of claim 4 , wherein said extensions are tapered proximate their leading ends to facilitate insertion into the disc space between the adjacent vertebral bodies.

6. The apparatus of claim 4 , wherein said extensions have an interior surface and an exterior surface, said interior surface of each of said extensions being concave in a plane transverse to the longitudinal axis of each of said extensions.

7. The apparatus of claim 6 , wherein the cutting instrument has an outside perimeter, said interior surface of each of said extensions being configured to conform to at least a portion of the shape of said outside perimeter.

8. The apparatus of claim 4 , wherein said opening is configured to permit the cutting instrument to move between said extensions in a direction along the longitudinal axis of each of said extensions.

9. The apparatus of claim 4 , wherein said opening is configured to permit the cutting instrument to move between said extensions in a direction transverse to the longitudinal axis of each of said extensions.

10. The apparatus of claim 4 , wherein said opening extends along a majority of the length of said portion of each of said extensions.

11. A distractor for use in human spinal surgery across at least a portion of a disc space between two adjacent vertebral bodies of a human spine, said distractor comprising:

a first extension and a second extension opposite said first extension, said first and second extensions adapted to be inserted into the disc space between the adjacent vertebral bodies, said first and second extensions each having a leading end for insertion first into the disc space and a trailing section opposite said leading end, a maximum length therebetween, and a longitudinal axis along the maximum length, said first and second extensions being joined to each other by a connecting member at said leading ends, said connecting member having a length as measured in a direction perpendicular to the longitudinal axes of said extensions, said connecting member having a maximum width in a direction parallel to the longitudinal axes of said extensions that is less than the maximum length of each of said extensions, said first and second extension each having a portion adapted to bear against each of the adjacent endplates of the adjacent vertebral bodies from within the disc space, said portion of said first and second extensions having an upper surface adapted to contact one of the adjacent end plates of the adjacent vertebral bodies and a lower surface adapted to contact the other of the adjacent end plates of the adjacent vertebral bodies, said portion of said first and second extensions having a maximum height between said upper and lower surfaces and a length sufficient to space the adjacent vertebral bodies in a selected relationship to one another; and

an opening between said first and second extensions configured to receive at least a portion of a cutting instrument.

12. The distractor of claim 11 , wherein said first and second extensions are tapered proximate their leading ends to facilitate insertion into the disc space between the adjacent vertebral bodies.

13. The distractor of claim 11 , wherein said first and second extensions have an interior surface and an exterior surface, said interior surface of each of said first and second extensions being concave in a plane transverse to the longitudinal axis of each of said first and second extensions.

14. The distractor of claim 13 , wherein the cutting instrument has an outside perimeter, said interior surface of each of said first and second extensions being configured to conform to at least a portion of the shape of the outside perimeter of the cutting instrument.

15. The distractor of claim 11 , wherein said opening is configured to permit the cutting instrument to move between said first and second extensions in a direction along the longitudinal axis of each of said first and second extensions.

16. The distractor of claim 11 , wherein said opening is configured to permit the cutting instrument to move between said first and second extensions in a direction transverse to the longitudinal axis of each of said first and second extensions.

17. The distractor of claim 11 , wherein said opening extends along a majority of the length of said portion of each of said first and second extensions.

18. The distractor of claim 11 , wherein said first and second extensions each include a rail member.

19. The distractor of claim 18 , wherein said rail member is configured to engage and move along a track.
