Abstract

Disclosed are surgical tools, tool sets and methods for percutaneously accessing and preparing treatment sites within the spine for subsequent treatment procedures. The treatment site may be an inter-vertebral motion segments in the lumbar and sacral regions of the spine. The tool set may comprise introducer tools and bone dilators for accessing and tapping into a targeted site, such as, for example, the anterior surface of the S1 vertebral body. The tool set may also comprise cutters and extractors for preparing the treatment site for subsequent treatment procedures. The tool set may additionally comprise a bone graft inserter, an exchange system, and/or a temporary distraction tool for further preparing the treatment site for subsequent treatment procedures.

Claims (43)

1. A tissue removal tool, comprising:

an elongate tubular sheath, having a proximal end and a distal end;

an elongate shaft axially moveably positioned within the sheath, the shaft comprising a proximal shaft end and a distal shaft end;

a plurality of filaments carried by and axially extending distally beyond the distal end of the shaft;

wherein the filaments each comprise a proximal end and a free distal end, and at least one filament comprises a helical shape on the distal end;

wherein the filaments are moveable between a first, reduced cross sectional profile when the shaft is proximally retracted to position the filaments within the sheath, and a second, expanded cross sectional profile when the shaft is axially distally advanced with respect to the sheath, to expose the filaments beyond the distal sheath end, the filaments biased to overlap each other in the second profile and configured to entangle tissue fragments and to extract tissue fragments from a body cavity.

2. A tissue removal tool as in claim 1 , wherein the filaments are substantially parallel to a longitudinal axis of the sheath when the filaments are in the first profile.

3. A tissue removal tool as in claim 1 , wherein the filaments are randomly organized when the shaft is in the second profile.

4. A tissue removal tool as in claim 1 , comprising at least ten filaments.

5. A tissue removal tool as in claim 1 , comprising within the range of from about 5 to about 40 filaments.

6. A tissue removal tool as in claim 1 , wherein the filaments comprise a nickel titanium alloy.

7. A tissue removal tool as in claim 1 , wherein the filaments comprise stainless steel.

8. A tissue removal tool as in claim 1 , wherein each filament has a diameter of no greater than about 0.05 inches.

9. A tissue removal tool as in claim 1 , wherein each filament has a diameter of no greater than about 0.02 inches.

10. A tissue removal tool as in claim 1 , further comprising a handle on the proximal shaft end.

11. A tissue removal tool as in claim 1 , wherein the tool is dimensioned to be introduced into and used in an intervertebral space.

12. A tissue removal tool as in claim 1 , wherein the plurality of filaments forms a core with a central hollow lumen formed by the plurality of filaments, the central hollow lumen having no intervening core structure.

13. A tissue removal tool as in claim 1 , comprising within the range of from about 7 to 14 filaments.

14. A tissue removal tool as in claim 1 , wherein each filament has a diameter about 0.01 inches.

15. A tissue removal tool as in claim 1 , wherein the plurality of filaments define a cross sectional dimension within the range of from about 0.50 inches to about 1.50 inches when in the second, expanded cross sectional profile.

16. A tissue removal tool as in claim 1 , wherein a plurality of filaments comprise a helical shape on the distal end.

17. A tissue removal tool as in claim 1 , wherein at least one filament comprises a spiral shape on the distal end.

18. A spinal tissue extraction tool, adapted for insertion into a target disc space, comprising:

an elongate tubular sheath;

an elongate shaft, having a proximal end and a distal end, movably positionable within the sheath; and

a tissue extraction head axially extending distally beyond the distal end of the shaft;

wherein the tissue extraction head comprises a plurality of overlapping nonparallel filaments comprising free distal ends biased radially-outwardly for entangling tissue fragments and for extracting tissue fragments from the target disc space, the plurality of filaments forming a core with a central hollow lumen formed by the plurality of filaments, wherein the filaments have a proximal end and a distal end, and at least one filament comprises a spiral shape on the distal end.

19. A spinal tissue extraction tool as in claim 18 , wherein the extraction head comprises a plurality of wires.

20. A spinal tissue extraction tool as in claim 18 , wherein the extraction head comprises an unraveled wire coil.

21. A spinal tissue extraction tool as in claim 18 , wherein the extraction head comprises at least about 10 filaments.

22. A spinal tissue extraction tool as in claim 18 , wherein the extraction head comprises at least about 20 filaments.

23. A spinal tissue extraction tool as in claim 18 , wherein the filaments are randomly arranged.

24. A spinal tissue extraction tool as in claim 18 , comprising within the range of from about 5 to about 40 filaments.

25. A spinal tissue extraction tool as in claim 18 , wherein the filaments comprise a nickel titanium alloy.

26. A spinal tissue extraction tool as in claim 18 , wherein the filaments comprise stainless steel.

27. A spinal tissue extraction tool as in claim 18 , wherein each filament has a diameter of no greater than about 0.05 inches.

28. A spinal tissue extraction tool as in claim 18 , wherein each filament has a diameter of no greater than about 0.02 inches.

29. A spinal tissue extraction tool as in claim 18 , further comprising a handle on the proximal end of the shaft.

30. A tissue removal tool as in claim 18 , comprising within the range of from about 7 to 14 filaments.

31. A tissue removal tool as in claim 18 , wherein each filament has a diameter about 0.01 inches.

32. A tissue removal tool as in claim 18 , wherein the filaments are moveable between a first, reduced cross sectional profile when the shaft is proximally retracted to position the filaments within the tubular sheath, and a second, expanded cross sectional profile when the shaft is axially distally advanced with respect to the sheath, to expose the filaments beyond the distal end of the sheath, wherein the filaments are biased to self-expand to the second, expanded cross sectional profile defining a cross sectional dimension within the range of from about 0.50 inches to about 1.50 inches when in the second, expanded cross sectional profile.

33. A tissue removal tool as in claim 18 , wherein at least one filament comprises a helical shape on the distal end.

34. A tissue removal tool as in claim 18 , wherein a plurality of the filaments comprise a spiral shape on the distal end.
