Abstract

A system includes an implantable polymer for increasing the dimensions of layers of tissue. In one embodiment, the implantable polymer is delivered transorally using an endoscope and an injection needle. The system allows for visualization and injection of polymer into an esophageal sphincter. Methods of treating gastroesophageal reflux disease include insertion of the distal portion of the injection needle into the esophageal tissue and injection of a polymer liquid to enlarge tissue dimensions. The polymer can be injected at several sites to distribute localized pressures and attain better distribution of the tissue bulking effect.

Claims (20)

1. A method of treating a sphincter in an esophagus of a patient to alleviate gastroesophageal reflux disease, said method comprising: providing a catheter and an injector coupled to said catheter, said catheter having a tissue piercing needle distal end; providing a liquid polymer which will polymerize in site at the site of injection to form a bulking implant, wherein the polymer comprises a polyisocyanate capped polyol; introducing said catheter into said esophagus of the patient; piercing an exterior surface of said sphincter of said esophagus of the patient with said tissue piercing needle distal end on said catheter; advancing said tissue piercing needle distal end a distance in an interior of said sphincter to a tissue site; and injecting a controlled volume of said liquid polymer into said tissue site to create a controlled volume of in-situ implant in said sphincter to bulk said sphincter; and thereby reducing at least one of the frequency and the quantity of sphincter reflux.

2. The method of claim 1 , wherein said volume of liquid polymer is sufficient to create a bolus having a diameter in the range of about 1-10 mm in said esophageal sphincter.

3. The method of claim 2 , wherein said bolus of polymer having a diameter of 1 to 10 mm is injected into an esophageal sphincter to reduce a duration of esophageal sphincter relaxation.

4. The method of claim 2 , wherein said bolus of polymer is delivered to said sphincter to cause a reduction in the opening of said sphincter.

5. The method of claim 2 , wherein said bolus of polymer is delivered to said sphincter to cause a tissue contraction around said implant.

The method of claim 1 , wherein said volume of liquid polymer reduces at least one of: the frequency of sphincter relaxation; the frequency of reflux of stomach contents into an esophagus; the frequency of a symptom of reflux of stomach contents into an esophagus; and the incidence of a sequela of reflux of stomach contents into an esophagus.

7. The method of claim 1 , wherein the polymer delivery device delivers said volume of polymer to a submucosa of said sphincter.

8. The method of claim 1 , wherein said polymer implant comprises a biocompatible polymer which initiates a fibrotic response.

9. The method of claim 1 , wherein said polymer implant further comprises free polyisocyanate.

10. The method of claim 1 , wherein said catheter is directed to said sphincter transorally.

11. The method of treating a mammalian sphincter as recited in claim 1 , wherein said catheter is directed to said sphincter transorally through an endoscope.

12. The method of claim 1 , wherein said biocompatible polymer is polymerized in situ upon delivery of said polymer in said delivery device to said sphincter.

A method of treating a sphincter in an esophagus of a patient to alleviate gastroesophageal reflux disease, said method comprising: providing a catheter and an injector coupled to said catheter, said catheter having a tissue piercing needle distal end; providing a liquid polymer which will polymerize in site at the site of injection to form a bulking implant, wherein the polymer comprises a polyisocyanate capped polyol; introducing said catheter into said esophagus of the patient; piercing an exterior surface of said sphincter of said esophagus of the patient with said tissue piercing needle distal end on said catheter; advancing said tissue piercing needle distal end a distance in an interior of said sphincter to a tissue site; and injecting a controlled volume of said liquid polymer into said tissue site to create a controlled volume of in-situ implant in said sphincter to bulk said sphincter; and thereby reducing at least one of the frequency and the quantity of sphincter reflux; and

wherein said polymer implant comprises a biocompatible polymer which polymerizes in situ by reaction with water, derive from one or both of water contained in the tissue of said sphincter, and aqueous solution mixed with the polymer at or near the time of administration of the polymer to the sphincter.

A method of treating a sphincter in an esophagus of a patient to alleviate gastroesophageal reflux disease, said method comprising: providing a catheter and an injector coupled to said catheter, said catheter having a tissue piercing needle distal end; providing a liquid polymer which will polymerize in site at the site of injection to form a bulking implant, wherein the polymer comprises a polyisocyanate capped polyol; introducing said catheter into said esophagus of the patient; piercing an exterior surface of said sphincter of said esophagus of the patient with said tissue piercing needle distal end on said catheter; advancing said tissue piercing needle distal end a distance in an interior of said sphincter to a tissue site; and injecting a controlled volume of said liquid polymer into said tissue site to create a controlled volume of in-situ implant in said sphincter to bulk said sphincter; and thereby reducing at least one of the frequency and the quantity of sphincter reflux; and

wherein the polymer delivery device delivers said polymer to create a tightening of said sphincter without permanently damaging anatomical structures near said sphincter.

15. The method of claim 14 , wherein said polymer delivery device delivers said polymer to an esophageal sphincter without permanently disrupting an aorta positioned near said esophageal sphincter.

16. The method of claim 14 , wherein said polymer delivery device delivers said polymer to an esophageal sphincter without permanently damaging a vagus nerve positioned near said esophageal sphincter.

17. The method of claim 14 , wherein said polymer delivery device delivers said polymer to an esophageal sphincter without permanently damaging an esophageal plexus of nerves and veins positioned near said esophageal sphincter.

18. The method of claim 14 , wherein said polymer delivery device delivers said polymer to an esophageal sphincter while preserving a blood supply to said esophageal sphincter.
