Abstract

Embodiments of the invention include devices and methods for implanting a vertebral body spacer. A lateral surgical approach is contemplated with some disclosed procedures. Instruments of some embodiments are insertable into a disc space to achieve distraction, to determine a desired disc space height, and to select a corresponding implant.

Claims (75)

1. An implant with an elongated body extending along a longitudinal axis and positionable in a spinal disc space comprising:

a convexly curved upper surface orientable toward an endplate of an upper vertebra;

a convexly curved lower surface orientable toward an endplate of a lower vertebra;

a leading end portion and an opposite trailing end portion;

an at least partially convexly curved anterior sidewall including an anteriorly-facing surface extending between the leading end portion and the trailing end portion, the anteriorly-facing surface including a maximally anterior portion defining a maximum width of the elongated body, a first anteriorly-facing convexly curved surface portion defining a first convex curve extending from the leading end portion to the maximally anterior portion, and a second anteriorly-facing convexly curved surface portion defining a second convex curve extending from the trailing end portion to the maximally anterior portion; and

a posterior sidewall extending between the leading end portion and the trailing end portion;

wherein the body includes a height between the upper and lower surfaces corresponding to a desired disc space height between the upper vertebral endplate and the lower vertebral endplate; and

wherein the leading end portion is structured for insertion into the disc space in an at least partially collapsed condition and the height is sized to restore the collapsed disc space to the desired disc space height as the body is inserted in the collapsed disc space; and

wherein the height of the implant increases from the leading end portion to a location near its center; and

wherein the height of the implant at the location at its center defines a maximum height of the implant, and wherein the implant defines a varying height between the upper and lower surfaces along a length of the implant between the leading and trailing end portions, the varying height of the implant being symmetrical relative to a transverse axis passing through the location of the implant defining the maximum height.

2. The implant of claim 1 wherein the upper surface and the lower surface are each convexly curved along an entire length of the body.

The implant of claim 1 wherein the leading end portion includes a nose rounded between the upper surface and the lower surface, the nose defining a first convex curvature that curves along the height of the implant from the convexly curved upper surface to a leading end surface of the elongated body, the nose also defining a second convex curvature that curves along the height of the implant from the convexly curved lower surface to the leading end surface.

4. The implant of claim 3 wherein the nose is completely rounded between the sidewalls.

5. The implant of claim 1 wherein an average height of the anterior sidewall is greater than an average height of the posterior sidewall.

6. The implant of claim 1 wherein the trailing end portion is planar and extends between the sidewalls and the upper and lower surfaces.

7. The implant of claim 1 wherein the body includes:

a first notch in a first one of the sidewalls and extending to the trailing end portion; and

a second notch in a second one of the pair of sidewalls and extending to the trailing end portion.

8. An implant with an elongated body extending along a longitudinal axis and positionable in a spinal disc space comprising:

a convexly curved upper surface orientable toward an endplate of an upper vertebra;

a convexly curved lower surface orientable toward an endplate of a lower vertebra;

a leading end portion and an opposite trailing end portion;

an at least partially convexly curved anterior sidewall including an anteriorly-facing surface extending between the leading end portion and the trailing end portion, the anteriorly-facing surface defining a maximally anterior portion, a first anteriorly-facing convexly curved surface portion extending between the leading end portion and the maximally anterior portion, and a second anteriorly-facing convexly curved surface portion extending between the trailing end portion and the maximally anterior portion; and

a posterior sidewall extending between the leading end portion and the trailing end portion;

wherein the body includes a height between the upper and lower surfaces corresponding to a desired disc space height between the upper vertebral endplate and the lower vertebral endplate; and

wherein the leading end portion is structured for insertion into the disc space in an at least partially collapsed condition and the height is sized to restore the collapsed disc space to the desired disc space height as the body is inserted in the collapsed disc space; and

wherein the height of the implant increases from the leading end portion to a location near its center; and

wherein the height of the implant at the location at its center defines a maximum height of the implant, and wherein the implant defines a varying height between the upper and lower surfaces along a length of the implant between the leading and trailing end portions, the varying height of the implant being symmetrical relative to a transverse axis passing through the location of the implant defining the maximum height; and

wherein the body includes:

a first notch in a first one of the sidewalls; and

a second notch in a second one of the pair of sidewalls; and

further comprising a coupling member having first and second fingers positionable in respective ones of the first and second notches to secure the body to the coupling member.

9. The implant of claim 1 wherein the upper and lower surfaces each include a number of engagement members therealong and projecting outwardly therefrom to engage bony tissue of the adjacent vertebral endplate when the body is positioned in the spinal disc space.

10. The implant of claim 9 wherein the engagement members comprise a number of teeth along portions of the sidewalls.

11. The implant of claim 10 wherein the teeth are raked toward the direction of implant insertion.

12. An implant with an elongated body extending along a longitudinal axis and positionable in a spinal disc space comprising:

an upper surface orientable toward an endplate of an upper vertebra;

a lower surface orientable toward an endplate of a lower vertebra;

a leading end portion structured for insertion into the spinal disc space and an opposite trailing end portion;

an at least partially convexly curved anterior sidewall including an anteriorly-facing surface extending between the leading end portion and the trailing end portion, the anteriorly-facing surface including a maximally anterior portion defining a maximum width of the elongated body, a first anteriorly-facing convexly curved surface portion defining a first convex curve extending from the leading end portion to the maximally anterior portion, and a second anteriorly-facing convexly curved surface portion defining a second convex curve extending from the trailing end portion to the maximally anterior portion; and

a posterior sidewall extending between the leading end portion and the trailing end portion, the posterior sidewall having an average height less than the average height of the anterior sidewall; and

wherein at least one of the upper surface and the lower surface is convexly curved such that a height of the implant near its center is greater than any height of the implant toward its leading end portion; and

wherein the height of the implant at a location near its center defines a maximum height of the implant, and wherein the implant defines a varying height between the upper and lower surfaces and extending along a length of the implant between the leading and trailing end portions, the varying height of the implant being generally symmetrical relative to a transverse axis passing through the location of the implant defining the maximum height; and

wherein the convexly curved surface includes convex portions arranged on opposite sides of the transverse axis which passes through the location of the implant defining the maximum implant height, the convex portions extending commonly along a constant radius of curvature.

13. The implant of claim 12 wherein the upper surface and the lower surface are each convexly curved along an entire length of the body.

14. The implant of claim 12 wherein the leading end portion includes a nose rounded between the upper surface and the lower surface, the nose defining a first convex curvature that curves along the height of the implant from the convexly curved upper surface to a leading end surface of the elongated body, the nose also defining a second convex curvature that curves along the height of the implant from the convexly curved lower surface to the leading end surface.

15. The implant of claim 14 wherein the nose is completely rounded between the sidewalls.

16. The implant of claim 12 wherein the trailing end portion is planar and extends between the sidewalls and the upper and lower surfaces.

17. The implant of claim 12 wherein the body includes:

a first notch in a first one of the sidewalls and extending to the trailing end portion; and

a second notch in a second one of the pair of sidewalls and extending to the trailing end portion.

18. The implant of claim 17 further comprising a coupling member having first and second fingers positionable in respective ones of the first and second notches to secure the body to the coupling member.

19. The implant of claim 12 wherein the upper and lower surfaces each include a number of engagement members therealong and projecting outwardly therefrom to engage bony tissue of the adjacent vertebral endplate when the body is positioned in the spinal disc space.

20. The implant of claim 19 wherein the engagement members comprise a number of teeth along portions of the sidewalls.

21. The implant of claim 20 wherein the teeth are raked toward the direction of implant insertion.

22. An implant with an elongated body extending along a longitudinal axis and positionable in a spinal disc space comprising:

a convexly curved upper surface;

a convexly curved lower surface;

a leading end portion defined by a convexly curved leading end surface that extends between the upper surface and the lower surface, the convexly curved leading end surface defining a first convex curvature that curves along a height of the implant from the convexly curved upper surface and also defining a second convex curvature that curves along the height of the implant from the convexly curved lower surface;

a trailing end portion defined by a substantially planar surface that extends between the upper surface and the lower surface;

an at least partially convexly curved anterior sidewall including an anteriorly-facing surface including a maximally anterior portion defining a maximum width of the elongated body, a first anteriorly-facing convexly curved surface portion defining a first convex curve extending from the leading end portion to the maximally anterior portion, and a second anteriorly-facing convexly curved surface portion defining a second convex curve extending from the trailing end portion to the maximally anterior portion; and

a posterior sidewall extending between the leading end portion and the trailing end portion; and

wherein the height of the implant near its center is greater than any height of the implant toward its leading end; and

wherein the height of the implant at a location near its center defines a maximum height of the implant, and wherein the implant defines a varying height between the upper and lower surfaces and extending along a length of the implant between the leading and trailing end portions, the varying height of the implant being generally symmetrical relative to a transverse axis passing through the location of the implant defining the maximum height; and

wherein each of the convexly curved upper and lower surfaces includes convex portions arranged on opposite sides of the transverse axis which passes through the location of the implant defining the maximum implant height, and wherein the convex portions are symmetrical relative to the transverse axis.

23. The implant of claim 1 wherein each of the convexly curved upper and lower surfaces are symmetrical relative to the transverse axis which passes through the location of the implant defining the maximum implant height.

24. The implant of claim 12 wherein the height of the implant near its center is between about 7 mm and 19 mm; and

wherein the height of the implant at its leading end is at least about 3 mm less than the height of the implant near its center.

25. The implant of claim 12 wherein the convex portions are symmetrical relative to the transverse axis which passes through the location of the implant defining the maximum implant height.

26. The implant of claim 22 wherein a height of the implant near its center is between about 8 mm and 14 mm; and

wherein a height of the implant at its leading end is at least about 3 mm less than the height of the implant near its center.

27. The implant of claim 22 wherein the convex portions extend commonly along a constant radius of curvature.

28. The implant of claim 1 wherein the first and second convex curves defined by the first and second convexly curved surface portions are part of the same convex curvature extending from the leading end portion to the trailing end portion, and wherein said maximally anterior portion comprises a point along the convex curvature.

29. The implant of claim 12 wherein the first and second convex curves defined by the first and second convexly curved surface portions are part of the same convex curvature extending from the leading end portion to the trailing end portion, and wherein said maximally anterior portion comprises a point along the convex curvature.

30. The implant of claim 22 wherein the first and second convex curves defined by the first and second convexly curved surface portions are part of the same convex curvature extending from the leading end portion to the trailing end portion, and wherein said maximally anterior portion comprises a point along the convex curvature.
