Abstract

A surgical device integrating a suction mechanism with a coagulation mechanism is provided for improving lesion creation capabilities. The device comprises an elongate member having an insulative covering attached about means for coagulating soft tissue. Openings through the covering expose regions of the coagulation-causing elements and are coupled to lumens in the elongate member which are routed to a vacuum source. A fluid source to passively transport fluid along the contacted soft tissue surface may be provided in order to push the maximum temperature deeper into tissue.

Claims (37)

1. A surgical device adapted to be coupled to a vacuum source and for coagulating soft tissue comprising:

an elongate member comprising a side wall and a lumen, where the lumen is fluidly coupleable to the vacuum source;

a tissue heating element adjacent the side wall within the elongate member; and

an opening through the side wall in communication with the lumen and exposing the tissue heating element where the tissue heating element is recessed within the opening such that the tissue heating element is offset from the opening, the tissue heating element being exposed only at the opening; and

where the side wall is flexible to conform to soft tissue to form a seal against the tissue upon application of a vacuum from the vacuum source so that the vacuum pulls tissue within the opening causing the tissue to contact the tissue heating element.

2. The device of claim 1 , wherein the tissue heating element comprises at least one helical winding having at least one pitch and at least one winding width.

3. The device of claim 2 , wherein the pitch is greater than or equal to two times the width.

4. The device of claim 2 , wherein the pitch is greater than or equal to four times the width.

5. The device of claim 1 , wherein an external surface of the side wall along the opening is offset from the tissue heating element by a distance, wherein the width of the opening is greater than or equal to the offset distance.

6. The device of claim 5 , wherein the width of the opening is greater than two times the offset distance.

7. The device of claim 1 , wherein the tissue heating element comprises an electrode.

8. The device of claim 7 , wherein the electrode is adapted to transmit RF energy to heat tissue.

9. The device of claim 7 , wherein the electrode is adapted to heat tissue by resistive heating of the electrode.

10. The device of claim 7 , wherein the tissue heating element comprises a transmission body structurally sufficient to transmit vibration.

11. The device of claim 10 , wherein the transmission body defines a plurality of openings.

12. The device of claim 11 , wherein the transmission body comprises a plurality of rings.

13. The device of claim 11 , wherein the transmission body comprises a helix.

14. The device of claim 10 , wherein the transmission body is connected to a drive shaft.

15. The device of claim 1 , wherein the tissue heating element comprises a means for tissue coagulation.

16. A system for coagulating soft tissue comprising:

a device as described by claim 1 , and a generator adapted to be coupled to the heating element.

17. The system of claim 16 , wherein the heating element comprises an RF electrode and where the generator comprises an RF generator.

18. The system of claim 16 , wherein the heating element comprises a resistive beating element arid where the generator comprises a DC power supply.

19. The system of claim 16 , wherein the tissue heating element comprises a vibration transmission body and where the generator is an ultrasonic generator.

20. A surgical device for use with a vacuum source and a power supply for applying energy to soft tissue comprising:

an elongate member comprising a side wall and a lumen, where the lumen is fluidly coupleable to the vacuum source;

a recessed tissue heating element adjacent the side wall within the elongate member and adapted to be coupled to the power supply for applying energy to the soft tissue; and

an opening through the side wall in communication with the lumen and exposing the recessed tissue heating element, where the sidewall comprises a flexible material and where the tissue heating element is recessed within the opening and is offset by a vertical distance from an edge of the opening; and

where the sidewall is sufficiently flexible to conform to the soft tissue and form a seal upon application of the vacuum source so that the vacuum is able to draw tissue into the opening causing the tissue to contact the recessed tissue heating element.

21. The surgical device of claim 20 , where the recessed tissue heating element comprises an element selected from the group consisting of a vibration element, an RF electrode, and a resistive heating element.

The surgical device of claim 20 , where the lumen comprises a vacuum lumen and where the device further comprises a fluid lumen extending through the elongate member and adapted to be coupled to a fluid source, where the fluid lumen is coupled to the vacuum lumen such that application of the vacuum source causes fluid to flow through the fluid lumen and vacuum lumen and over the recessed tissue heating element.

23. A surgical device for creating curvilinear lesions in soft tissue using a power supply and adapted apply vacuum from a vacuum source, the device comprising:

an elongate member comprising a side wall and a lumen, where the lumen is fluidly coupleable to the vacuum source;

a recessed tissue heating element adjacent the side wall within the elongate member, the recessed tissue heating element adapted to be coupled to the power supply; and

an opening in the side wall having a length and a width and where the length is greater than the width, where the opening is in communication with the lumen and exposes the tissue heating element, where the sidewall adjacent to the opening comprises a flexible material sufficient to conform to a surface of the soft tissue and form a seal against the soft tissue over the entire exposed portion of the tissue heating element upon application of the vacuum source, where the seal causes the vacuum to draw tissue into the opening to contact the recessed tissue heating element.

24. The surgical device of claim 23 , where the recessed tissue heating element comprises an element selected from the group consisting of a vibration element, an RF electrode, and a resistive heating element.

The surgical device of claim 23 , where the lumen comprises a vacuum lumen and the device further comprises a fluid lumen extending through the elongate member and adapted to be coupled to a fluid source, where the fluid lumen is coupled to the vacuum lumen such that application of the vacuum source causes fluid to flow through the fluid lumen and vacuum lumen and over the recessed tissue heating element.
