Abstract

A collateral ventilation bypass trap system directly linked with a patient's lung or lungs may be utilized to increase the expiratory flow from the diseased lung or lungs, thereby treating one aspect of chronic obstructive pulmonary disease. The system includes a trap, a filter/one-way valve and an air carrying conduit. The system also includes a retention device for securing system elements in position.

Claims (61)

1. A ventilation bypass system comprising:

a conduit having a distal end configured to pass into a lung of a patient through an opening in a chest wall, wherein the opening in the chest wall passes from outside the patient through skin of the chest, muscle, parietal pleura and visceral pleura into the lung;

a retention plate projecting from the conduit between a proximal end and the distal end, the retention plate configured to remain external to the patient and limit insertion of the conduit into the lung through the opening in the chest wall; and

a flow-control device connected to the proximal end of the conduit;

a fixation device operatively associated with the distal end of the conduit and adapted to engage the lung to substantially prevent unnecessary movement of the conduit;

wherein the fixation device comprises a hinged structure which has a first configuration adapted to permit the distal end of the conduit to pass into the lung of a patient through the opening in the chest wall and a second configuration adapted to prevent removal of the distal end of the conduit from the lung; and

wherein the ventilation bypass system is configured to allow air to escape from the lung through the opening in the chest wall.

2. The ventilation bypass system according to claim 1 , further comprising a spring structure to control the hinged structure.

3. The ventilation bypass system according to claim 1 , wherein the fixation device comprises said hinged structure having at least one flap attached to the distal end of the conduit by a hinge.

4. The ventilation bypass system according to claim 3 , wherein the fixation device comprises said hinged structure having at least two flaps attached to the distal end of the conduit by a hinge.

5. The ventilation bypass system according to claim 1 , wherein the retention plate is adapted to be secured to skin of the patient with adhesive.

6. The ventilation bypass system according to claim 1 , wherein the fixation device is attached to the distal end of the conduit.

7. The ventilation bypass system of claim 1 , wherein the flow-control device comprises a filter.

8. The ventilation bypass system of claim 1 , wherein the flow-control device comprises a valve.

9. The ventilation bypass system of claim 1 , wherein the flow-control device comprises a one-way valve.

10. The ventilation bypass system of claim 1 , wherein the flow-control device comprises a containment vessel adapted to store discharge from the lung.

11. The ventilation bypass system of claim 1 , wherein the fixation device includes means for controlling a configuration of the hinged structure.

12. A system for treating chronic obstructive pulmonary disease comprising:

a conduit having a distal end configured to pass into a lung of a patient through an opening in a chest wall, wherein the opening in the chest wall passes from outside the patient through skin of the chest, muscle, parietal pleura and visceral pleura into the lung;

a retention plate projecting from the conduit between a proximal end and the distal end the retention plate configured to remain external to the patient and engage the skin and control insertion of the conduit into the lung through the opening in the chest wall; and

a hinged fixation device connected to the distal end of the conduit wherein the hinged fixation device has a first configuration adapted to permit the distal end of the conduit to pass into the lung of a patient through the opening in the chest wall and a second configuration adapted to prevent removal of the distal end of the conduit from the lung;

wherein the ventilation bypass system is configured to allow air to escape from the lung through the opening in the chest wall via the conduit.

13. The system of claim 12 , wherein the retention plate is adapted to be secured to skin of the patient with adhesive.

14. The system for treating chronic obstructive pulmonary disease of claim 12 , further comprising a flow-control device connected to the proximal end of the conduit wherein the flow-control device comprises a filter.

15. The system of claim 12 , further comprising a flow-control device connected to the proximal end of the conduit wherein the flow-control device comprises a one-way valve.

16. The system of claim 12 , further comprising a flow-control device connected to the proximal end of the conduit wherein the flow-control device comprises a discharge trap.

17. The system of claim 12 , wherein, the hinged fixation device comprises at least one flap and wherein in the first configuration of the hinged fixation device the flap is substantially parallel to the conduit and in the second configuration of the hinged fixation device the flap is other than substantially parallel to the conduit.

The system of claim 12 , wherein, the hinged fixation device comprises at least two flaps and wherein in the first configuration of the hinged fixation device the flaps are substantially parallel to the conduit and in the second configuration of the hinged fixation device the flaps are other than substantially parallel to the conduit.

19. The system of claim 12 , further comprising a spring structure to control the hinged fixation structure.

20. A system for treating chronic obstructive pulmonary disease comprising:

a conduit having a distal end adapted and configured to pass into a lung of a patient through an opening in a chest wall, wherein the opening in the chest wall passes from outside the patient through skin of the chest, muscle, parietal pleura and visceral pleura into the lung;

a retention plate projecting from the conduit between a proximal end and the distal end which is adapted and configured to remain external to the patient and engage the skin of the chest wall in order to control insertion of the conduit into the lung through the opening in the chest wall;

a fixation device connected to the distal end of the conduit which fixation device includes at least one hinged structure;

wherein, in a first configuration, the fixation device is adapted to permit the distal end of the conduit to pass into the lung of a patient through the opening in the chest wall;

wherein, in a second configuration, the fixation device is adapted to prevent removal of the distal end of the conduit from the lung and the at least one hinged structure is other than substantially parallel to the conduit; and

wherein the system is configured to allow air to escape from the lung through the opening in the chest wall.

21. The system according to claim 20 , further comprising a spring structure to control the fixation device.

22. The system according to claim 20 , wherein the fixation device comprises means for controlling a configuration of the hinged structure.

23. The system according to claim 20 , wherein the retention plate comprises an adhesive positioned to secure the retention plate to the skin of the patient.

24. The system according to claim 20 , wherein the hinged structure comprises an agent adapted to prevent one or more of damage to the lung; and reaction by the lung.

25. The system according to claim 20 , further comprising a flow-control device operatively associated with the proximal end of the conduit, the flow control device comprising one or more of, a valve, a filter and a vessel.

26. A system for treating a patient through an artificial channel in a chest wall which passes from external to the patient, through the chest wall, parietal membrane, visceral membrane and into parenchymal tissue of a lung, the system comprising:

a conduit having a proximal end and a distal end;

the distal end of the conduit being adapted and configured to pass into the lung of a patient through the artificial channel in the chest wall;

the proximal end of the conduit being adapted and configured to remain external to the chest of the patient;

a retention plate projecting from the conduit and spaced from the distal end of the conduit wherein the retention plate is adapted to engage the skin of the chest wall in order to control insertion of the distal end of the conduit into the lung through the opening in the chest wall and secure the proximal end of the conduit external to the chest of the patient;

a fixation member associated with the distal end of the conduit, the fixation member being configurable at a first angle relative to the conduit and a second angle relative to the conduit;

the first angle of the fixation member relative to the conduit being adapted to permit the distal end of the conduit to pass into the lung of a patient through the artificial channel in the chest wall;

the second angle of the fixation member relative to the conduit being adapted to prevent removal of the distal end of the conduit from the lung of the patient through the artificial channel in the chest wall; and

wherein the system is adapted and configured to permit gasses to exit the lung through the artificial channel in the chest wall via the conduit.

27. The system of claim 26 , further comprising means for controlling an angle of the fixation member.

28. The system of claim 26 , wherein the fixation member is attached to the distal end of the conduit by a hinge.

29. The system of claim 26 , comprising a second such fixation member.

30. The system of claim 26 , wherein the retention plate comprises an adhesive surface adapted to secure the retention plate to the skin of the chest of the patient.

31. The system of claim 26 further comprising a flow-control device associated with the proximal end of the conduit.

32. The system of claim 26 further comprising a flow-control device associated with the proximal end of the conduit and wherein the flow-control device comprises a filter.

33. The system of claim 26 further comprising a flow-control device associated with the proximal end of the conduit and wherein the flow-control device comprises a valve.

34. The system of claim 26 further comprising a flow-control device associated with the proximal end of the conduit and wherein the flow-control device comprises means for controlling movement of material through the system.

35. The system of claim 26 further comprising a flow-control device associated with the proximal end of the conduit and wherein the flow-control device comprises a containment vessel adapted to store discharge from the lung.

36. The system of claim 26 wherein the fixation member comprises an agent adapted to prevent damage to the lung.

37. The system of claim 26 wherein the fixation member comprises an agent adapted to prevent reaction by the lung.
