Abstract

A urethral prosthesis with prostatic and bulbar segments connected by two types of ties allows the prosthesis to assume at least two configurations different with inter-segmental distances adapted to situations where the patient either has or does not have normal control of the external sphincter. This is particularly useful for a patient undergoing an anesthetic procedure that affects the external sphincter muscles. When the muscles are anesthetized, the prosthesis may provide constant urine voiding, and when the anesthetic effects wear off, the prosthesis may assume a different configuration to allow the sphincter to reassert control over urinary voiding.

Claims (28)

1. A medical device for insertion into a patient's urethra, comprising:

a first tubular segment comprising a smooth outer surface and an inner lumen surface, the first tubular segment including a distal portion defining a distal opening, the first segment also including a proximal portion defining a proximal opening, the first segment defining a lumen extending between the distal opening and the proximal opening;

a second tubular segment comprising a smooth outer surface and an inner lumen surface, the second tubular segment including a distal portion defining a distal opening, the second segment also including a proximal portion defining a proximal opening, the second segment defining a lumen extending between the distal opening and the proximal opening; and

an externally-adjustable tie connecting the proximal portion of the first segment and the distal portion of the second segment, the tie extending outside of the patient's body when the medical device is inserted into the patient's urethra.

2. The device of claim 1 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is externally adjustable by the tie.

3. The device of claim 2 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is shortened such that the device adopts a compact configuration.

4. The device of claim 3 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is shortened such that the device allows for constant fluid communication between the lumen of the first segment and the lumen of the second segment.

5. The device of claim 4 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is shortened such that the proximal portion of the first segment is in direct contact with the distal portion of the second segment.

6. The device of claim 3 , wherein the compact configuration of the device holds the urethra and external sphincter muscles open, thereby providing constant urine drainage through the lumen of the first segment and the lumen of the second segment.

The device of claim 2 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is extended such that the distance permits insertion of an object between the two segments such that the object intercepts fluid communication between the lumens of the first and second segments.

8. The device of claim 7 , wherein the distance between the first and second segments permits operation of an external sphincter muscle.

9. The device of claim 7 further comprising a retrieval piece attached to the proximal portion of the second segment, whereby pulling the retrieval piece increases the distance between the two segments.

10. The device of claim 1 , wherein the tie comprises a thread with two ends, and wherein the thread forms at least one loop between the proximal portion of the first segment and the distal portion of the second segment.

11. The device of claim 1 , wherein the first segment further comprises an inflatable balloon.

12. The device of claim 1 further comprising a third segment having a proximal portion and a distal portion, the third segment defining a lumen, the distal portion of the third segment being removably connected to the proximal portion of the second segment.

13. A medical device for insertion into a patient's urethra, comprising:

a first segment for placement in the prostatic section of the urethra, the first segment including a distal portion defining a distal opening, the distal portion comprising a closed tip, the first segment also including a proximal portion defining a proximal opening, the first segment defining a lumen extending between the distal opening and the proximal opening;

a second segment for placement in the bulbar section of the urethra, the second segment including a distal portion defining a distal opening, the second segment also including a proximal portion defining a proximal opening, the second segment defining a lumen extending between the distal opening and proximal opening; and

an adjustable tie connecting the proximal portion of the first segment and the distal portion of the second segment, the tie extending outside the patient's body when the medical device is inserted into the patient's urethra.

14. The device of claim 13 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is externally adjustable by the tie.

15. The device of claim 14 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is shortened such that the device adopts a compact configuration.

16. The device of claim 15 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is shortened such that the device allows for constant fluid communication between the lumen of the first segment and the lumen of the second segment.

17. The device of claim 15 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is shortened such that the proximal portion of the first segment is in direct contact with the distal portion of the second segment.

18. The device of claim 15 , wherein the compact configuration of the device holds the urethra and external sphincter muscles open, thereby providing constant urine drainage through the lumen of the first segment and the lumen of the second segment.

19. The device of claim 14 , wherein the distance between the proximal portion of the first segment and the distal portion of the second segment is extended such that the distance permits insertion of an object between the two segments such that the object intercepts fluid communication between the lumens of the first and second segments.

20. The device of claim 19 , wherein the distance between the first and second segments permits operation of an external sphincter muscle.

21. The device of claim 19 further comprising a retrieval piece attached to the proximal portion of the second segment, whereby pulling the retrieval piece increases the distance between the two segments.

22. The device of claim 14 , wherein the tie comprises a thread with two ends, and wherein the thread forms at least one loop between the proximal portion of the first segment and the distal portion of the second segment.
