Abstract

A patient-worn monitoring system and method for geographically dispersed health care locations. A patient-worn monitoring system comprises a network, a body-worn monitoring station, a monitoring station server, a remote command center, and a rules engine. The network comprises a first sub-network and a second sub-network. The body-worn monitoring station comprises monitoring equipment. The body worn monitoring station monitors data elements from a patient assigned to a health care location and sends monitored data elements to a monitoring station server via the first sub-network. The monitoring station server receives the monitored data elements from the body worn monitoring station and relays the monitored data elements to the remote command center via the second sub-network. The remote command center receives the monitored data elements from the monitoring station server, associates the monitored data elements with the patient assigned to the health care location, accesses patient data elements indicative of a medical condition associated with the patient, and establishes a patient-specific rule associated with the patient. The rules engine selects data elements from the monitored data elements and the patient data elements associated with the patient and applies the patient-specific rule to the selected data elements continuously and simultaneously. A determination is made whether the patient-specific rule for the patient has been contravened. An alert is issued from the remote command center in the event the patient-specific rule for the patient has been contravened.

Claims (79)

1. A patient monitoring system for geographically dispersed health care locations comprising a network, a body-worn monitoring station, a monitoring station server, a remote command center, and a rules engine, wherein,

the network comprises a first sub-network and a second sub-network;

the body-worn monitoring station comprises monitoring equipment, and wherein the body worn monitoring station comprises instructions for:

monitoring data elements from a patient assigned to a health care location; and sending monitored data elements to a monitoring station server via the first sub-network;

the monitoring station server comprises instructions for:

receiving the monitored data elements from the body worn monitoring station; and

relaying the monitored data elements to the remote command center via the second sub-network;

the remote command center comprises instructions for:

receiving the monitored data elements from the monitoring station server;

associating the monitored data elements with the patient assigned to the health care location;

accessing patient data elements indicative of a medical condition associated with the patient;

establishing a patient-specific rule associated with the patient; and

the rules engine comprises instructions for:

selecting data elements from the monitored data elements and the patient data elements associated with the patient;

applying the patient-specific rule to the selected data elements continuously and simultaneously;

determining in an automated fashion 24 hours per day 7 days per week from the remote command center whether the patient-specific rule for the patient has been contravened; and

issuing an alert from the remote command center in the event the patient-specific rule for the patient has been contravened.

2. The patient monitoring system of claim 1 , wherein the patient specific rule for the patient comprises an algorithm.

3. The patient monitoring system of claim 1 , wherein the selected data elements comprise a physiological data element of the patient and a clinical data element of the patient.

4. The patient monitoring system of claim 1 , wherein the selected data elements comprise a physiological data element of the patient and a medication data element of the patient.

5. The patient monitoring system of claim 1 , wherein the selected data elements comprise a physiological data element of the patient and a laboratory data element of the patient.

6. The patient monitoring system of claim 1 , wherein the selected data elements comprise a clinical data element of the patient and a laboratory data element of the patient.

7. The patient monitoring system of claim 1 , wherein the selected data elements comprise a physiological data element of the patient and another physiological data element of the patient.

8. The patient monitoring system of claim 1 , wherein the selected data elements comprise at least two data elements of the patient selected from the group consisting of a physiological data element, a clinical data element of the patient, a medication data element of the patient, and a laboratory data element of the patient.

9. The patient monitoring system of claim 1 , wherein the first and second sub-networks are selected from the group consisting of a wired network and a wireless network.

10. The patient monitoring system of claim 9 , wherein the wireless network is selected from the group consisting of a Bluetooth-compatible network, an 802.11-compatible network, and a short-range, slotted protocol network.

11. The patient monitoring system of claim 1 , wherein the monitoring station server is wearable by the patient.

12. The patient monitoring system of claim 11 , wherein the first sub-network is a wired network and the second sub-network is a wireless network.

13. The patient monitoring system of claim 11 , wherein the first sub-network is a wireless network and the second sub-network is a wireless network.

14. The patient monitoring system of claim 1 , wherein the patient is a fetus carried by an expectant mother and wherein the monitoring equipment comprises a tachodynamometer.

15. The patient monitoring system of claim 1 , wherein the monitoring equipment is at least one sensor selected from the group consisting of an ECG sensor, a pulse oximeter, a thermometer, a respiration sensor, a heart rate sensor, and an audio sensor.

16. The patient monitoring system of claim 1 , wherein the monitored data elements are associated with a patient identifier.

17. The patient monitoring system of claim 1 , wherein the health care location is selected from the group consisting of a hospital, a nursing home, a mobile health care facility, a space-based health care facility, a residence, an emergency room, an intensive care unit, a field health care facility, an operating room, and a step-down unit.

18. The patient monitoring system of claim 17 , wherein the mobile health care facility is selected from the group consisting of a ship, a helicopter, an ambulance, a MASH unit, and a a tent hospital.

19. The patient monitoring system of claim 1 , wherein monitoring equipment comprises physiological sensors and wherein monitored data elements comprise physiological data elements.

20. The patient monitoring system of claim 1 , wherein the remote command center further comprises:

an external network interface, wherein the external network interface comprises instructions for connecting to an external network; and

instructions for providing a health care provider access to the remote command center via the external network.

21. The patient monitoring system of claim 20 , wherein the external network is selected from the group consisting of a wired network, a wireless network, a cable network, a fiber optic network, and the Internet.

22. The patient monitoring system of claim 20 , wherein the health care provider is selected from the group consisting of a physician, a nurse, a clinician, a diagnostician, and a intensivist.

23. The patient monitoring system of claim 20 , wherein the remote command center further comprises instructions for sending the health care provider the alert if the patient-specific rule for the hospitalized patient has been contravened.

24. A method for monitoring patients assigned to geographically dispersed health care locations comprising:

associating a body-worn monitoring station with a patient assigned to a health care location, wherein the body-worn monitoring station comprises monitoring equipment;

monitoring data elements from a patient assigned to a health care location;

sending monitored data elements to a monitoring station server via a first sub-network;

relaying the monitored data elements from the monitoring station server to a remote command center via a second sub-network;

receiving the monitored data elements at the remote command center from the monitoring station server;

associating the monitored data elements with the patient assigned to the health care location;

accessing patient data elements indicative of a medical condition associated with the patient;

establishing a patient-specific rule associated with the patient;

selecting data elements from the monitored data elements and the patient data elements associated with the patient;

applying the patient-specific rule to the selected data elements continuously and simultaneously;

determining in an automated fashion 24 hours per day 7 days per week at the remote command center whether the patient-specific rule for the patient has been contravened;

and

issuing an alert from the remote command center in the event the patient-specific rule for the patient has been contravened.

25. The method of claim 24 , wherein the patient specific rule for the patient comprises an algorithm.

26. The method of claim 24 , wherein the selected data elements comprise a physiological data element of the patient and a clinical data element of the patient.

27. The method of claim 24 , wherein the selected data elements comprise a physiological data element of the patient and a medication data element of the patient.

28. The method of claim 24 , wherein the selected data elements comprise a physiological data element of the patient and a laboratory data element of the patient.

29. The method of claim 24 , wherein the selected data elements comprise a clinical data element of the patient and a laboratory data element of the patient.

30. The method of claim 24 , wherein the selected data elements comprise a physiological data element of the patient and another physiological data element of the patient.

31. The method of claim 24 , wherein the selected data elements comprise at least two data elements of the patient selected from the group consisting of a physiological data element, a clinical data element of the patient, a medication data element of the patient, and a laboratory data element of the patient.

32. The method of claim 24 , wherein the first and second sub-networks are selected from the group consisting of a wired network and a wireless network.

33. The method of claim 24 , wherein the monitoring station server is wearable by the patient.

34. The method of claim 32 , wherein wireless network is selected from the group consisting of a Bluetooth-compatible network, an 802.11-compatible network, and a short-range, slotted protocol network.

35. The method of claim 33 , wherein the first sub-network is a wired network and the second sub-network is a wireless network.

36. The method of claim 33 , wherein the first sub-network is a wireless network and the second sub-network is a wireless network.

37. The method of claim 24 , wherein the patient is a fetus carried by an expectant mother and wherein the monitoring equipment comprises a tachodynamometer.

38. The method of claim 24 , wherein the monitoring equipment is at least one sensor selected from the group consisting of an ECG sensor, a pulse oximeter, a thermometer, a respiration sensor, a heart rate sensor, and an audio sensor.

39. The method of claim 24 , wherein the monitored data elements are associated with a patient identifier.

40. The method of claim 24 , wherein the health care location is selected from the group consisting of a hospital, a nursing home, a mobile health care facility, a space-based health care facility, a residence, an emergency room, an intensive care unit, a field health care facility, an operating room, and a step-down unit.

41. The method of claim 40 , wherein the mobile health care facility is selected from the group consisting of a ship, a helicopter, an ambulance, a MASH unit, and a tent hospital.

42. The method of claim 24 , wherein monitoring equipment comprises physiological sensors and wherein monitored data elements comprise physiological data elements.

43. The method of claim 24 further comprising:

interfacing with an external network; and

providing a health care provider access to the remote command center via the external network.

44. The method of claim 43 , wherein the external network is selected from the group consisting of a wired network, a wireless network, a cable network, a fiber optic network, and the Internet.

45. The method of claim 43 , wherein the health care provider is selected from the group consisting of a physician, a nurse, a clinician, a diagnostician, and a intensivist.

46. The method of claim 43 further comprising sending the health care provider the alert if the patient-specific rule for the hospitalized patient has been contravened.
