Abstract

Devices, systems and methods are disclosed for the formation of an arteriovenous fistula. Embodiments include catheter apparatus including an ablation element for creating and/or modifying the fistula. The devices, systems and methods can be used to treat patients with one or more numerous ailments including chronic obstructive pulmonary disease, congestive heart failure, hypertension, hypotension, respiratory failure, pulmonary arterial hypertension, lung fibrosis and adult respiratory distress syndrome.

Claims (57)

1 . A catheter apparatus for being inserted over a guidewire which has been placed between two anatomical structures, said catheter apparatus comprising:

a first elongate tube with a proximal end, a distal end, and a lumen therebetween;

a second elongate tube with a proximal end, a distal end, and a lumen therebetween, said second elongate tube sized and configured to be slidingly advanced and retracted within the lumen of the first elongate tube; and

an ablation element for creating and/or modifying a fistula between the two anatomical structures.

2 . The apparatus of claim 1 wherein the distal end of the second elongate tube is tapered.

3 . The apparatus of claim 2 wherein the tapered end has a conical geometry.

4 . The apparatus of claim 1 , wherein the ablation element is disposed at or on the distal end of the second elongate tube.

5 . The apparatus of claim 1 wherein the ablation element is disposed at or on the distal end of the first elongate tube.

6 . The apparatus of claim 1 wherein the second elongate tube has an axial guidewire lumen.

7 . The apparatus of any one of claims 1 to 6 wherein the ablation element comprises one or more electrodes.

8 . The apparatus of claim 7 wherein the ablation element comprises at least two electrodes.

9 . The apparatus of claim 8 wherein the at least two electrodes are separately connectable in a bipolar circuit.

10 . The apparatus of claim 8 wherein at least one electrode is on the first elongate tube and at least one electrode is on the second elongate tube.

11 . The apparatus of any one of claims 1 to 10 , wherein the first elongate tube includes a deflectable portion at or near its distal end.

12 . The apparatus of claim 11 wherein the distal portion is deflected by a pull wire attached to said distal portion.

13 . The apparatus of claim 1 wherein the second elongate tube can be completely removed from the lumen of the first elongate tube.

14 . The apparatus of any one of claims 1 - 10 further comprising a third elongate element which is insertable into the lumen of the first elongate tube or of the second elongate tube.

15 . The apparatus of claim 14 wherein the third elongate element is insertable into a lumen of the second elongate tube.

16 . The apparatus of claim 15 wherein the third elongate element is insertable into the lumen of the second elongate tube when said second elongate tube resides within the lumen of the first elongate tube.

17 . The apparatus of claim 14 wherein the third elongate element comprises an electrode structure at its distal tip.

18 . The apparatus of claim 17 wherein the electrode structure comprises a resilient coil which retracts into the lumen of the second elongate tube when retracted proximally.

19 . The apparatus of claim 14 wherein the third elongate element has a for penetrating between the anatomical structures and placing a guidewire.

20 . The apparatus of claim 14 wherein the third elongate element comprises an imaging device.

21 . The apparatus of claim 14 wherein the third elongate element comprises a drug delivery device.

22 . The apparatus of claim 14 wherein the third elongate element comprises a radiation delivery catheter.

23 . The apparatus of claim 14 wherein the third elongate element comprises a cutting catheter.

24 . The apparatus of claim 14 wherein the third elongate element comprises a clip delivery catheter.

25 . The apparatus of claim 14 wherein the third elongate element comprises a guidewire lumen.

26 . The apparatus of any one of claims 1 to 10 further comprising an anastomotic clip mounted near the distal end of the second elongate tube.

27 . The apparatus of claim 26 wherein the anastomotic clip is mounted in a recess in the outer surface of the second elongate tube.

28 . The apparatus of claim 27 wherein the anastomotic clip includes a radially expanding portion.

29 . The apparatus of claim 27 further comprising a balloon mounted near the distal end of the second elongate tube.

30 . The apparatus of claim 29 wherein the balloon is mounted under the anastomotic clip.

31 . The apparatus of claim 27 wherein the anastomotic clip is configured to provide at least one of (1) tension between walls of the two anatomical structures; (2) hemostasis in or around the fistula tissue; (3) tension between the two anatomical structures; (4) a reduction in neointimal proliferation into the fistula; (5) a radial force on the tissue surrounding the fistula to maintain a lumen between the structures; (6) a radioactive source to deliver radiation to the tissue surrounding the fistula; (7) a drug delivery depot delivering one or more agents to the tissue of the fistula to enhance long-term patency or biocompatibility; and (8) a control means to adjust the flow of blood through the fistula such as via independent means or in combination with a separate device.

32 . The apparatus of claim 27 wherein the anastomotic clip comprises an electrode to deliver energy to the tissue surrounding the fistula.

33 . The apparatus of claim 1 wherein the ablation element is configured to deliver energy selected from the group consisting of: acoustic energy; electromagnetic energy; thermal energy; chemical energy; light energy; mechanical energy; and radiation.

34 . The apparatus of claim 33 wherein the ablation element is configured to deliver two or more forms of energy.

35 . The apparatus of claim 34 wherein the ablation element is configured to deliver two or more forms of energy simultaneously.

36 . The apparatus of any one of claims 1 to 10 further comprising an expandable balloon which is inflatable within the fistula after the ablation element enlarges said fistula.

37 . The apparatus of claim 36 further comprising an anastomotic clip, said clip placed in the fistula after the balloon enlarges said fistula.

38 . The apparatus of claim 1 wherein the ablation element ablates tissue protruding into the fistula.

39 . The apparatus of any one of claims 1 to 10 , further comprising a handle mounted on the proximal end of at least one of the first elongate tube and the second elongate tube.

40 . The apparatus of claim 39 , further comprising one or more controls used to initiate energy delivery from the ablation element.

41 . The apparatus of claim 39 , further comprising one or more controls to initiate deflection of the distal end of the first elongate tube.

42 . The apparatus of claim 14 , wherein the distal end of the third elongate element has a spiral shape.

43 . The apparatus of claim 42 , wherein the spiral shape is flexible.

44 . The apparatus of claim 43 , wherein the spiral shape is composed of a nickel-titanium alloy.

45 . The apparatus of claim 43 , wherein the spiral shape is retractable into the lumen of the second elongate tube.

46 . The apparatus of claim 42 , wherein the spiral shape provides the ablation element.

47 . The apparatus of claim 42 , wherein the ablation element comprises one or more electrodes mounted to the spiral shape.

48 . A system comprising the apparatus of any one of claims 1 to 10 and an energy source.

49 . The system of claim 48 wherein the energy source provides energy selected from the group consisting of: acoustic energy; electromagnetic energy; thermal energy; chemical energy; light energy; mechanical energy; and radiation.

50 . The system of claim 49 wherein the energy source provides at least two forms of energy.

51 . The apparatus of claim 14 wherein the third elongate element comprises a tube having a lumen from its proximal end to its distal end.

52 . The apparatus of claim 51 wherein the lumen includes a lining over at least a portion of its sections.

53 . The apparatus of claim 52 wherein the lining is a lubricious lining.

54 . The apparatus of claim 51 wherein the third elongate tube can be completely removed from the lumen of the second elongate tube.
