Abstract

A method of treating a vein comprises accessing a vein at an access point spaced from a sapheno-femoral junction. A bioabsorbable fibrous body is implanted into the vein through the access point. The body is moved in the vein toward the sapheno-femoral junction.

Claims (46)

1. A method of treating a blood vessel, said method comprising:

delivering into said blood vessel an implant through a percutaneous access site, said implant comprising a plurality of loose tortuous fibers, said fibers being formed from one or more bioabsorbable materials, said fibers extending longitudinally along said implant and forming an outermost, vessel-wall-engaging surface of said implant, said implant further comprising a bioabsorbable tether coupled to said fibers at a junction point in a distal portion of said implant, said tether extending longitudinally along said implant, said tether having a larger cross-section than any one of said individual fibers;

positioning said implant in said vessel with a pushrod so that said fibers and said tether extend along said blood vessel from the junction point of said fibers and said tether in said distal portion of said implant, to said access site, and said tether further extends through a wall of said blood vessel at said access site and into tissue external to said blood vessel at said access site, thereby securing said implant in position in said blood vessel;

wherein said pushrod during positioning engages the distal portion of said implant and extends past the proximal portion and out of the access site.

2. The method of claim 1 , wherein the step of delivering comprises delivering the implant from a sheath into said blood vessel.

3. The method of claim 2 , further comprising applying external compression to the blood vessel while withdrawing the sheath from the blood vessel.

4. The method of claim 1 , further comprising the step of pushing a pushrod to deliver the implant from a sheath into said blood vessel.

5. The method of claim 4 , further comprising the step of applying external compression to the blood vessel while removing the pushrod from the blood vessel.

6. The method of claim 1 , wherein the fibers are individually self-expanding.

7. The method of claim 1 , wherein individual fibers of the implant are self-expandable independently of the other fibers of the implant.

8. The method of claim 7 , further comprising allowing said implant to expand inside said blood vessel by allowing the individual fibers to expand independently of the other fibers of the implant.

9. The method of claim 1 , wherein the positioning comprises pushing said pushrod from outside the patient.

10. A method of treating a blood vessel, said method comprising:

accessing said blood vessel at an access point;

delivering into said blood vessel an implant comprising:

a plurality of loose tortuous fibers, said fibers being formed from one or more bioabsorbable materials, said fibers extending longitudinally along said implant and forming an outermost, tissue-engaging surface of said implant; and

a bioabsorbable tether coupled to said fibers at a junction point in a distal portion of said implant, said tether extending longitudinally along said implant, said tether having a larger cross-section than any one of said individual fibers;

advancing said implant from a delivery site through said blood vessel toward a sapheno-femoral junction using a pushrod that extends from outside the patient and along the length of said implant to engage the distal portion of said implant; and

positioning said implant in said blood vessel with said pushrod so that:

said fibers and said tether extend along said blood vessel from the junction point of said fibers and said tether in said distal portion of said implant, to said access point; and

said tether extends through a wall of said blood vessel at said access point, and into tissue external to said blood vessel at said access point, thereby securing said implant in position in said blood vessel.

11. The method of claim 10 , additionally comprising securing said implant in said blood vessel to limit migration of said implant within said blood vessel.

12. The method of claim 10 , wherein the step of delivering comprises delivering the implant from a sheath into said blood vessel.

13. The method of claim 12 , further comprising the step of applying external compression to the blood vessel while withdrawing the sheath from the blood vessel.

14. The method of claim 12 , further comprising the step of pushing the pushrod to deliver the implant from the sheath into said blood vessel.

15. The method of claim 14 , further comprising the step of applying external compression to the blood vessel while removing the pushrod from the blood vessel.

16. The method of claim 10 , further comprising scraping an inside wall of the blood vessel.

17. The method of claim 16 wherein scraping an inside wall comprises scraping with the implant.

18. The method of claim 10 , further comprising the step of expanding the implant, wherein the individual fibers are self-expanding.

19. A method of treating a vein, said method comprising:

accessing said vein at an access point spaced from a sapheno-femoral junction;

introducing a bioabsorbable fibrous implant into said vein through said access point, said implant comprising:

a body made up of loose, textured bioabsorbable fibers extending longitudinally along the body; and

a bioabsorbable tether coupled to the fibers at a junction point in a distal portion of the body, the tether not being coupled to a proximal portion of the body such that the tether and the proximal portion are accordingly adapted for relative motion, the tether extending longitudinally from the distal portion of the body and along the body, the tether having a larger cross-section than any one of the individual fibers;

moving said body in said vein using a pushrod that engages the distal portion of the body, wherein said pushrod extends the entire length of said implant toward said sapheno-femoral junction and urges the distal portion of said body toward the sapheno-femoral junction so that the distal portion of said the body is between the proximal portion of said body and said sapheno-femoral junction;

pushing said pushrod from outside the patient while the pushrod is engaged with the distal portion of the body within the patient

to position said body in said vein so that:

said fibers and said tether extend along said vein from the junction point of said fibers and said tether in said distal portion of said body to said access point; and

said tether extends through said vein wall at said access point, and into tissue external to said vein at said access point, thereby securing said implant in position in said vein.

20. The method of claim 19 , additionally comprising securing said body in said vein to limit migration of said body within said vein.

21. The method of claim 19 , wherein introducing comprises inserting a sheath through said access point and pushing said body with the pushrod through said sheath into said vein.

22. The method of claim 19 , additionally comprising performing a heat treatment on said vein, said heat treatment comprising one or more of delivering radio frequency energy, delivering heat energy from a resistive element, and delivering energy from a laser.

23. The method of claim 1 , further comprising the step of injecting the vein with fibrinogen, thrombin, and/or growth factor solution.

24. The method of claim 1 , wherein the fibers are individually self-expanding.

25. The method of claim 1 , wherein individual fibers of the body are self-expandable independently of the other fibers of the body.

26. The method of claim 25 , further comprising allowing said body to expand inside said vein by allowing the individual fibers to expand independently of the other fibers of the body.
