Abstract

The invention relates to a controlled release composition comprising a combination of isosorbide dinitrate and hydralazine, such as hydralazine hydrochloride, that in operation delivers the drug in a pulsed or multi-modal manner for the treatment of angina, ischaemic heart disease, arterial hypertension and related disease conditions. Preferably, the isosorbide dinitrate and hydralazine hydrochloride can be released from the dosage form in an erodable, diffusion and/or osmotic-controlled release profile.

Claims (14)

1 . A controlled release pharmaceutical composition comprising a population of isosorbide dinitrate and hydralazine, or a salt or derivative thereof, -containing particles, wherein at least a portion of the particles incorporate a modified-release coating or, alternatively or additionally, a modified-release matrix material, such that the composition following oral delivery to a subject delivers the isosorbide dinitrate and hydralazine, or salt or derivative thereof, in a pulsatile or zero order manner and wherein at least a portion of said particles comprise also an additional active agent or a salt or derivative thereof.

2 . The composition of claim 1 wherein said additional active agent is a cardiovascular agent.

The composition of claim 2 wherein said cardiovascular agent is selected from the group consisting of: an ACE inhibitor, an aldosterone inhibitor, an ARB blocker, an antiarrhythmic agent, a diuretic, an inotropic agent, a vasodilator, an alpha-blocker, a beta-blocker, and an antiadrenergic agent.

4 . The composition of claim 3 wherein said cardiovascular agent is an ACE inhibitor selected from the group consisting of: benazepril, captopril, enelapril, fosinopril, lisinopril, moexipril, quinapril, ramipril, and trandolapril.

5 . The composition of claim 3 wherein said cardiovascular agent is an aldoesterone inhibitor which is spironolactone or eplerenone.

6 . The composition of claim 3 wherein said cardiovascular agent is an ARB blocker selected from the group consisting of: irbesartan, candesartan, losartan, valsartan, telmisartan, and eprosartan.

The composition of claim 3 wherein said cardiovascular agent is an antiarrhythmic agent selected from the group consisting of: quinidine, procainamide, disopyramide, lidocaine, tocainide, mexiletine, encainide, flecainide, moricizine, ibutilide, procainamide, disopyramide, lidocaine, tocainide, flecainide, mexiletine, propafenone, bretylium, amiodarone, adenosine, dofetilide, propranolol, sotalol, verapamil, diltiazem, nifedipine, nisoldipine, nimodipine, isradipine, bepridil, felodipine, amlodipine, and mebefradil.

8 . The composition of claim 3 wherein said cardiovascular agent is a diuretic selected from the group consisting of: loop diuretics, thiazides, and potassium sparing diuretics.

9 . The composition of claim 3 wherein said cardiovascular agent is an inotropic agent which is digoxin.

10 . The composition of claim 3 wherein said cardiovascular agent is a vasodilator which is a nitrate or a peripheral vasodilator.

11 . The composition of claim 3 wherein said cardiovascular agent is an alpha-blocker which is labetalol or carvedilol.

12 . The composition of claim 3 wherein said cardiovascular agent is an antiadrenergic agent which is a centrally active or peripherally active.

13 . The composition of claim 3 wherein said cardiovascular agent is a beta-blocker selected from the group consisting of: acebutolol, atenolol, betaxolol, bisoprolol, esmolol, metoprolol, carteolol, nadolol, penbutolol, pindolol, propanolol, sotalol, timolol, and labetalol.

14 . A method for the treatment of angina, ischaemic heart disease, arterial hypertension or related disease conditions comprising administering a therapeutically effective amount of a composition according to claim 1.
