Abstract

The present invention relates to new and useful apparatus, systems and methods for providing an effective tool for intraluminally directed vascular anastomosis of a graft vessel to a receiving blood vessel that is performed according to a minimally invasive procedure. The intraluminally directed vascular anastomosis does not require the interruption of blood flow in the receiving blood vessel and it is versatile enough to suitably combine a variety of cutting, welding, soldering, sealing, and joining techniques. The intraluminally directed anvil apparatus comprises an anvil and a wire used for signaling the optimal anastomosis site; this signaling can be performed when the initial exploration is performed. An anastomosis device is used in conjunction with the intraluminally directed anvil apparatus for opening the anastomosis fenestra and joining the anastomosed structures.

Claims (55)

1. A method for forming an anastomosis opening in a side of a blood vessel to facilitate fluid communication with a tubular structure, the method comprising:

positioning an anvil in a lumen of a blood vessel at an anastomosis site, wherein the anastomosis site is located in the side of the blood vessel,

wherein the anvil comprises a receiving surface at one end and a side surface;

moving the anvil at least partially into an opening of a ring so that a portion of a wall of the blood vessel at the anastomosis site is held between the side surface of the anvil and the ring and such that the receiving surface of the anvil causes a region of the wall to extend beyond the portion of the wall held between the side surface of the anvil and the ring in conformance with the shape of the anvil,

wherein a component of the anvil extends through a piercing located in the extended region of the wall of the blood vessel; and

forming an anastomosis opening in the wall of the blood vessel at the anastomosis site while extending the region of the wall of the blood vessel beyond the portion that is held between the side surface of the anvil and the ring such that at least part of the region of the wall of the blood vessel that was extended rests on the side surface of the anvil after formation of the anastomosis opening,

wherein the anastomosis opening is larger than the piercing,

wherein the anastomosis opening is formed without requiring interruption of blood flow through the blood vessel at the anastomosis site,

leaving the ring at the anastomosis site after forming the anastomosis opening and after removing the anvil.

A method as recited in claim 1 , wherein moving the anvil at least partially into an opening of a ring so that a portion of the wall of the blood vessel at the anastomosis site is held between the side surface of the anvil and the ring and a region of the wall extends beyond the portion of the wall held between the anvil and the ring in conformance with the shape of the anvil causes the extended region to stretch.

3. A method as recited in claim 1 , wherein the receiving surface is flat.

4. A method as recited in claim 1 , wherein the receiving surface is convex.

5. A method as recited in claim 1 , wherein the anastomosis opening is formed through engagement of the receiving surface of the anvil with a cutter.

6. A method as recited in claim 5 , wherein the cutter comprises a cutting knife with a cutting edge, and wherein the anastomosis opening is formed as the cutting edge engages the receiving surface of the anvil at a location with a perimeter which is larger than the perimeter of the cutting edge.

7. A method as recited in claim 5 , wherein the cutter comprises a cutting knife with a cutting edge, and wherein the perimeter of the cutting edge is circular such that the perimeter of the anastomosis opening is circular.

8. A method as recited in claim 5 , wherein the component extending from the anvil is coaxially positioned in the cutter.

9. A method as recited in claim 1 , wherein at least part of the region extending beyond the portion of the wall held between the anvil and the ring also extends into a lumen of a tubular structure before formation of the anastomosis opening.

A method as recited in claim 1 , wherein the anvil has a first configuration when being positioned, wherein the anvil has a second configuration when the anastomosis opening is formed, and wherein the anvil has a greater diameter when in the second configuration relative to the first configuration.

11. A method for forming an anastomosis opening in a side of a blood vessel to facilitate fluid communication with a tubular structure, the method comprising:

(a) positioning an anvil in a lumen of a blood vessel at an anastomosis site, wherein the anastomosis site is located in the side of the blood vessel,

wherein the anvil comprises a receiving surface at one end and a side surface;

(b) positioning a ring and a cutter at the anastomosis site;

(c) moving the anvil at least partially into an opening of the ring so that a portion of a wall of the blood vessel at the anastomosis site is held between the side surface of the anvil and the ring and such that the receiving surface of the anvil causes a region of the wall to extend beyond the portion of the wall held between the side surface of the anvil and the ring in conformance with the shape of the anvil,

wherein a component of the anvil extends through a piercing located in the extended region of the wall of the blood vessel; and

(d) forming an anastomosis opening in the wall of the blood vessel at the anastomosis site, while extending the region of the wall beyond the portion of the wall held between the side surface of the anvil and the ring, through engagement of the receiving surface of the anvil with the cutter,

whereby the portion of the blood vessel held between the side surface of the anvil and the ring during formation of the anastomosis opening continues to be held between the side surface of the anvil and the ring after the anastomosis opening has been formed such that at least part of the region of the wall of the blood vessel that was extended beyond the portion held between the side surface of the anvil and the ring rests on the side surface of the anvil after formation of the anastomosis opening.

12. A method as recited in claim 11 , wherein the anastomosis opening is formed without requiring interruption of blood flow through the blood vessel at the anastomosis site.

A method as recited in claim 11 , wherein moving the anvil at least partially into an opening of the ring so that a portion of the wall of the blood vessel at the anastomosis site is held between the side surface of the anvil and the ring and a region of the wall extends beyond the portion of the wall held between the anvil and the ring in conformance with the shape of the anvil causes the extended region to stretch.

14. A method as recited in claim 11 , wherein the receiving surface is flat.

15. A method as recited in claim 11 , wherein the receiving surface is convex.

A method as recited in claim 11 , wherein the cutter comprises a cutting knife with a cutting edge, and wherein the anastomosis opening is formed as the cutting edge engages the receiving surface of the anvil at a location with a perimeter which is larger than the perimeter of the cutting edge.

17. A method as recited in claim 11 , wherein the cutter comprises a cutting knife with a cutting edge, and wherein the perimeter of the cutting edge is circular such that the perimeter of the anastomosis opening is circular.

18. A method as recited in claim 11 , wherein the component extending from the anvil is coaxially positioned in the cutter.

19. A method as recited in claim 11 , wherein at least part of the region extending beyond the portion of the wall held between the anvil and the ring also extends into a lumen of a tubular structure before formation of the anastomosis opening.

20. A method as recited in claim 11 , wherein the anvil has a first configuration when being positioned, wherein the anvil has a second configuration when the anastomosis opening is formed, and wherein the anvil has a greater diameter when in the second configuration relative to the first configuration.

21. A method for forming an anastomosis opening in a side of a blood vessel to facilitate fluid communication with a tubular structure, the method comprising:

positioning an anvil in a lumen of a blood vessel at an anastomosis site, wherein the anastomosis site is located in the side of the blood vessel,

wherein the anvil comprises a receiving surface at one end and a side surface;

positioning a ring at the anastomosis site;

positioning a cutter at the anastomosis site;

moving the anvil at least partially into an opening of the ring so that a portion of a wall of the blood vessel at the anastomosis site is held between the side surface of the anvil and the ring and such that the receiving surface of the anvil causes a region of the wall to extend beyond the portion of the wall held between the side surface of the anvil and the ring in conformance with the shape of the anvil,

wherein a component of the anvil extends through a piercing located in the extended region of the wall of the blood vessel; and

forming an anastomosis opening in the wall of the blood vessel at the anastomosis site, while extending the region of the wall beyond the portion of the wall held between the side surface of the anvil and the ring, through engagement of the receiving surface of the anvil with the cutter,

whereby the portion of the blood vessel held between the side surface of the anvil and the ring during formation of the anastomosis opening continues to be held between the side surface of the anvil and the ring after the anastomosis opening has been formed such that at least part of the region of the wall of the blood vessel that was extended beyond the portion held between the side surface of the anvil and the ring rests on the side surface of the anvil after formation of the anastomosis opening;

removing the cutter from the anastomosis site after forming the anastomosis opening; and

leaving the ring at the anastomosis site after forming the anastomosis opening and after removing the anvil and the cutter.

22. A method as recited in claim 21 , wherein the anastomosis opening is formed without requiring interruption of blood flow through the blood vessel at the anastomosis site.

A method as recited in claim 21 , wherein moving the anvil at least partially into an opening of the ring so that a portion of the wall of the blood vessel at the anastomosis site is held between the side surface of the anvil and the ring and a region of the wall extends beyond the portion of the wall held between the anvil and the ring in conformance with the shape of the anvil causes the extended region to stretch.

24. A method as recited in claim 21 , wherein the receiving surface is flat.

25. A method as recited in claim 21 , wherein the receiving surface is convex.

A method as recited in claim 21 , wherein the cutter comprises a cutting knife with a cutting edge, and wherein the anastomosis opening is formed as the cutting edge engages the receiving surface of the anvil at a location with a perimeter which is larger than the perimeter of the cutting edge.

27. A method as recited in claim 21 , wherein the cutter comprises a cutting knife with a cutting edge, and wherein the perimeter of the cutting edge is circular such that the perimeter of the anastomosis opening is circular.

28. A method as recited in claim 21 , wherein the component extending from the anvil is coaxially positioned in the cutter.

29. A method as recited in claim 21 , wherein at least part of the region extending beyond the portion of the wall held between the anvil and the ring also extends into a lumen of a tubular structure before formation of the anastomosis opening.

30. A method as recited in claim 21 , wherein the anvil has a first configuration when being positioned, wherein the anvil has a second configuration when the anastomosis opening is formed, and wherein the anvil has a greater diameter when in the second configuration relative to the first configuration.
