Abstract

A surgical access device includes a base having two or more ports or apertures that provide for the insertion of surgical instruments. The multi-port insert may be used with a laparoscopic access device. The multi-port insert may also include one or more instrument supports that are attached to the base to fix the position of one or more surgical instruments inserted through the multi-port insert.

Claims (28)

1. A surgical access device for providing surgical access through an incision in the body wall to the inside of a body cavity of a patient, the surgical access device having a center axis and comprising:

an assembly having a proximal end and a distal end and having an opening around the center axis, the assembly comprising a ring, and a sleeve coupled to and extending generally axially from the ring and positionable in the incision, the sleeve defining a passageway through the body wall between the opening in the assembly and the body cavity;

a subassembly releasably positionable at the proximal end of the assembly and defining at least two apertures sized for accepting surgical instruments; and

a member disposed radially outwardly and distally with respect to the at least two apertures, the member positioned with respect to the sleeve to protect the sleeve from instruments inserted through the apertures.

2. The surgical access device of claim 1 , wherein the apertures are sized from between about five and twelve millimeters.

3. The surgical access device of claim 1 , wherein at least one aperture comprises a seal assembly.

4. The surgical access device of claim 3 , wherein the seal assembly is an iris seal.

5. The surgical access device of claim 3 , wherein the seal assembly is a duck bill valve.

6. The surgical access device of claim 1 , wherein the assembly comprises at least one instrument support.

7. The access device of claim 1 comprising a latch mechanism for releasably attaching the subassembly to the proximal end of the assembly.

8. A surgical access device for providing surgical access through an incision in the body wall to the inside of a body cavity of a patient, the surgical access device having a center axis and comprising:

an assembly having a proximal end and a distal end and having an opening around the center axis, the assembly comprising an upper ring, a lower ring, and a sleeve extending generally axially from the upper ring to the lower ring, the sleeve and the lower ring positionable in the incision, the sleeve defining a passageway through the body wall between the opening in the assembly and the body cavity;

a subassembly releasably positionable on the assembly and defining at least three apertures sized for accepting surgical instruments; and

a member extending at least partially into the passageway defined by the sleeve, the member disposed radially outwardly and distally with respect to the at least three apertures and positioned with respect to the sleeve to protect the sleeve from instruments inserted through the apertures.

9. The device of claim 8 wherein the member comprises a distal end, and wherein the distal end of the member is disposed distally of the upper ring.

10. The device of claim 8 wherein the member positioned with respect to the sleeve is generally circumferentially continuous.

11. The device of claim 8 wherein the member comprises a collar.

12. The device of claim 8 further comprising a seal assembly associated with at least two of the apertures.

13. The device of claim 12 wherein the member positioned with respect to the sleeve extends distally below the seal assembly of at least one aperture.

14. The device of claim 8 comprising a plurality of seal assemblies operatively associated with the subassembly, and wherein at least a portion of the seal assemblies are positioned within the passageway defined by the sleeve.

15. A surgical access device comprising:

a flexible sleeve adapted for insertion in an incision, the sleeve extending from a sleeve proximal end to a sleeve distal end, the sleeve providing a passageway intermediate the proximal and distal sleeve ends, the passageway having a minimum width at a position spaced from the proximal and distal sleeve ends;

a plurality of instrument openings supported with respect to the proximal end of the sleeve;

at least one valve associated with at least one instrument opening;

a generally circumferentially continuous member disposed radially outwardly and distally relative to of the plurality of instrument openings: and

wherein at least one valve is disposed distal of the proximal end of the sleeve.

16. The device of claim 15 further comprising a member disposed distally of at least one of the instrument openings, the member positioned with respect to the sleeve to protect the sleeve from instruments inserted through at least one instrument opening.

17. The device of claim 16 wherein the member comprises a generally circumferentially extending collar.
