Abstract

A patient monitor has multiple sensors adapted to attach to tissue sites of a living subject. The sensors generate sensor signals that are responsive to at least two wavelengths of optical radiation after attenuation by pulsatile blood within the tissue sites. A patient monitor uses the sensor signals to determine changes in constant oxygen consumption as an indication of changes in metabolism at the tissue cite.

Claims (40)

1. A physiological monitoring method comprising the steps of:

determining, using at least one processor, a plurality of perfusion values and a corresponding plurality of oxygen saturation values at a tissue site;

characterizing, using said at least one processor, a constant oxygen consumption relationship between said perfusion values and said oxygen saturation values;

detecting, using said at least one processor, perfusion-limited oxygen consumption at said tissue site when said constant oxygen consumption relationship is no longer valid; and

adjusting fractional inspired oxygen according to the plurality of oxygen saturation values and so as to account for changes in the plurality of oxygen saturation values that result from perfusion-limited oxygen consumption.

2. The physiological monitoring method according to claim 1 wherein said determining step comprises the substeps of:

receiving at least first and second intensity signals from a light-sensitive detector that responds to light of at least first and second wavelengths after attenuation by pulsatile blood flow within said tissue site;

deriving a plurality of arterial oxygen saturation values from said intensity signals;

deriving a plurality of venous oxygen saturation values from said intensity signals;

deriving a plurality of perfusion index values from said intensity signals; and

calculating a plurality of delta saturation values from the difference between said arterial oxygen saturation values and said venous oxygen saturation values.

3. The physiological monitoring method according to claim 2 wherein said characterizing step comprises the substep of calculating a plurality of oxygen consumption values from products of said perfusion index values and said delta saturation values.

4. The physiological monitoring method according to claim 3 wherein said characterizing step comprises the further substep of curve-fitting a hyperbola to said oxygen consumptions values versus said perfusion index values and said delta saturation values.

5. The physiological monitoring method according to claim 4 , comprising the further step of indicating a change in oxygen demand in response to a shift in said hyperbola relative to said perfusion index values and said delta saturation values.

6. A physiological monitoring method comprising the steps of:

determining, using at least one processor, delta oxygen saturation at a tissue site;

monitoring, using said at least one processor, perfusion at said tissue site so as to account for changes in delta oxygen saturation that result from changes in perfusion; and

adjusting fractional inspired oxygen according to said delta oxygen saturation measurement so as to maintain supply-independent oxygen consumption at said tissue site.

7. The physiological monitoring method according to claim 6 comprising the further step of increasing perfusion at said tissue site until delta oxygen saturation decreases so as to avoid perfusion-limited oxygen consumption at said tissue site.

8. A physiological monitoring method comprising the steps of:

providing a plurality of FiO2 adjustments for a person having a tissue site;

measuring a plurality of oxygen consumption values at said tissue site corresponding to said FiO2 adjustments;

determining, using said at least one processor, an oxygen supply independent region and at least one of an oxygen supply dependent region and a plasma dependent region for said oxygen consumption values; and

adjusting FiO2 based upon a transition between at least one of said regions,

said transition being a deflection in said oxygen consumption values as a function of FiO2.

9. The physiological monitoring method according to claim 8 wherein said measuring step comprises the substeps of:

measuring SpaO2 at said tissue site;

measuring SpvO2 at said tissue site; and

calculating a difference between SpaO2 and SpvO2.

10. The physiological monitoring method according to claim 9 , wherein said measuring step further comprises the substep of:

measuring perfusion at said tissue site to account for changes in the difference between SpaO2 and SpvO2 that result from changes in perfusion.

11. The physiological monitoring method according to claim 10 comprising the further step of increasing perfusion at said tissue site until the difference between SpaO2 and SpvO2 decreases so as to avoid perfusion-limited oxygen consumption at said tissue site.

The physiological monitoring method according to claim 8 wherein said person is a neonate and said adjusting step comprises adjusting FiO2 so that a corresponding one of said oxygen consumption values falls within said oxygen supply independent region proximate to said oxygen supply dependent region.

13. The physiological monitoring method according to claim 12 wherein:

said adjusting step comprises the substep of decreasing FiO2, and

said determining step comprises the substep of detecting a decrease in said oxygen consumption values corresponding to said decreasing FiO2 so as to indicate said oxygen supply dependent region.

14. The physiological monitoring method according to claim 8 wherein said person is an adult and said adjusting step comprises adjusting FiO2 so that a corresponding one of said oxygen consumption values falls within said oxygen supply independent region proximate to said plasma dependent region.

15. The physiological monitoring method according to claim 14 wherein:

said adjusting step comprises the substep of increasing FiO2, and

said determining step comprises the substep of detecting a decrease in said oxygen consumption values corresponding to said increasing FiO2 so as to indicate said plasma dependent region.
