Abstract

A surgical access device, such as a trocar, includes a pendent valve having an elongate structure extending from a proximal end to a septum valve disposed at a distal end. In operation, the elongate structure follows the angle of the instrument to pre-position the septum valve into the path of the instrument where it is not significantly challenged during instrument insertion or manipulation. The pendant valve can be made to float at both the proximal end and the distal end of the elongate structure, to further reduce the vulnerability of the septum valve. Since the valve is less vulnerable to instrument insertion, it can be formed to minimize friction and maximize the functional range of the access device.

Claims (37)

1. A trocar adapted to receive an instrument along a working channel, the trocar comprising:

a cannula having an axis extending between a proximal end and a distal end;

a valve housing having a proximal end having an opening therethrough, and a distal end coupled to the proximal end of the cannula;

a valve support having a distal end, a proximal end coupled to the valve housing, and a lumen extending between the proximal end and the distal end;

a valve suspended by the valve support along the working channel in proximity to the distal end of the valve support, and in proximity to the distal end of the valve housing;

a guidance structure at least partially within the lumen of the valve support for facilitating alignment of the valve with the instrument; and

a zero seal disposed in the valve housing along the working channel, wherein

the valve support has a floating relationship with the valve housing so that the valve is free to swing in an angular relationship to the axis.

2. The trocar recited in claim 1 , further comprising:

a coupling disposed between the valve support and the valve housing, the coupling having properties facilitating the floating relationship between the valve and the valve housing.

3. The trocar recited in claim 2 , wherein the coupling comprises a bellows.

4. The trocar recited in claim 1 , wherein:

the guidance structure is carried by the valve and support; and

the guidance structure has properties for detecting the angle of the instrument relative to the axis and for moving the valve support to carry the valve toward alignment with the instrument.

5. The trocar recited in claim 4 , wherein the guidance structure is disposed proximally of the valve.

6. The trocar recited in claim 5 , wherein the guidance structure includes a plurality of projections each extending inwardly of the valve support to contact the instrument when the instrument is inserted into the working channel.

7. The trocar recited in claim 6 , wherein the projections include at least one of annuli, folds, fins, fingers, and leaves.

8. The trocar recited in claim 5 , wherein the valve is a septum valve, and the zero seal is coupled to the valve housing distally of the septum valve.

9. The trocar recited in claim 1 , wherein the zero seal is disposed in the valve housing in proximity to the valve.

10. A trocar comprising:

a proximal end, a distal end, and a longitudinal axis extending from the proximal end to the distal end;

a valve housing disposed at the proximal end of the trocar;

a tubular cannula extending from a distal end of the valve housing;

a flexible coupling disposed at or near a proximal end of the valve housing;

a pendulous valve tube elongate along the longitudinal axis such that a ratio of a length of the pendulous valve tube along the longitudinal axis to a diameter of the pendulous valve tube is greater than 1:1 and extending distally from the flexible coupling and coupled thereto;

a septum valve positioned in the pendulous valve tube distal of the flexible coupling and

an instrument access channel extending through the tubular cannula and valve tube.

11. The trocar of claim 10 , wherein the septum valve is positioned proximate to a distal end of the valve tube.

12. The trocar of claim 11 , wherein the valve tube further comprises a duckbill valve distal of the septum valve.

13. The trocar recited in claim 11 , further comprising a duckbill valve positioned in the valve housing in proximity to the septum valve.

14. The trocar of claim 10 , further comprising an end cap comprising an opening therethrough, the end cap closing the proximal end of the valve housing.

15. The trocar of claim 14 , wherein the flexible coupling engages the opening in the end cap.

16. The trocar of claim 14 , wherein the end cap secures the flexible coupling within the valve housing.

17. The trocar of claim 10 , wherein the flexible coupling comprises a bellows.

18. The trocar of claim 10 , wherein the ratio of the length of the pendulous valve tube along the longitudinal axis to the diameter of the pendulous valve tube is approximately 2:1.

19. The trocar of claim 10 , wherein the ratio of the length of the pendulous valve tube along the longitudinal axis to the diameter of the pendulous valve tube is in a range between 1:1 and 3:1.

20. The trocar of claim 10 , wherein the pendulous valve tube has a generally cylindrical configuration.
