Abstract

Methods and apparatus for treatment of patent foramen ovale (PFO) provide for applying energy to tissues adjacent the PFO with a catheter device to substantially close the PFO acutely. Apparatus generally includes a catheter device having at least one energy transmission member at or near its distal end configured to apply energy to PFO tissues to acutely, substantially close the PFO. Applied energy may be monoploar or bipolar radiofrequency energy or any other suitable energy, such as laser, microwave, ultrasound, resistive heating or the like. Some embodiments of a catheter device further include one or more tissue apposition members near the distal end for helping bring PFO tissues together, such as a PFO covering member, a vacuum applying member and/or the like. PFO closure via energy-based approaches of the invention may help prevent stroke, treat migraine headache, and possibly treat or prevent other medical conditions.

Claims (46)

1. A method for closing a patent foramen ovale, the method comprising:

providing an energy transmission member coupleable to an energy source and an expandable member;

advancing the expandable member to expand within the patent foramen ovale;

apposing tissues of the patent foramen ovale,

inserting the energy transmission member into an opening between a first portion of the patent foramen ovale and a second portion of the patent foramen ovale;

applying energy to the patent foramen ovale with the energy transmission member at a first position; and

repeating application of energy to the patent foramen ovale with the energy transmission member at a second position adjacent to the first position.

2. The method of claim 1 , wherein applying energy to the first and second positions substantially closes the patent foramen ovale along at least a portion of the patent foramen ovale.

3. The method of claim 1 , further comprising removing the energy transmission member from the opening.

4. A method as in claim 3 , wherein removing the energy transmission member comprises retracting the energy transmission member into an elongate flexible member.

5. A method as in claim 3 , wherein removing the energy transmission member comprises retracting the energy transmission member as the energy transmission member collapses to a reduced profile.

6. A method as in claim 1 , wherein the energy transmission member comprises an electrode.

7. A method as in claim 1 , wherein applying energy to the patent foramen ovale comprises applying monopolar energy.

8. A method as in claim 1 , wherein applying energy to the patent foramen ovale comprises applying bipolar energy.

9. A method as in claim 1 , wherein applying energy to the patent foramen ovale comprises applying one of radiofrequency energy, cryogenic energy, resistive heat energy, heat energy, ultrasound energy, microwave energy and laser energy.

10. A method as in claim 1 , wherein inserting the energy transmission member comprises applying a compressive force so as to draw the first and second portions of the patent foramen ovale toward the energy transmission member.

11. A method as in claim 10 , wherein the compressive force is a vacuum.

12. A method as in claim 1 , further comprising delivering a tissue soldering material to the patent foramen ovale.

13. A method as in claim 1 , further comprising imaging the patent foramen ovale.

14. A method as in claim 13 , wherein imaging the patent foramen ovale comprises imaging with an ultrasound device.

15. A method for closing a patent foramen ovale, the method comprising:

advancing a guide catheter through the patent foramen ovale, between tissues adjacent the patent foramen ovale, the guide catheter containing an expandable member;

retracting the guide catheter to expose the expandable member;

expanding the expandable member within the patent foramen ovale;

apposing the tissues adjacent the patent foramen ovale; and

applying energy to the tissues adjacent the patent foramen ovale to weld the tissues adjacent the patent foramen ovale together.

16. A method as claimed in claim 15 , further comprising removing the expandable member from the patent foramen ovale after apposing the tissues adjacent the patent foramen ovale and before applying energy to the tissues adjacent the patent foramen ovale.

A method as claimed in claim 15 , wherein the applying energy to the tissues adjacent the patent foramen ovale includes a first welding and a second welding, and the expandable member is left in place in the patent foramen ovale during the first welding and then removed prior to the second welding.

18. A method as claimed in claim 15 , wherein the expandable member is self-expanding.

19. A method as claimed in claim 15 , wherein the expandable member includes one or more distal tips configured to locate the patent foramen ovale.

20. A method as claimed in claim 15 , wherein the expandable member includes one or more distal tips configured to cross the patent foramen ovale.

21. A method as claimed in claim 15 , wherein the expandable member includes one or more distal tips configured appose the tissues adjacent the patent foramen ovale.

22. A method as claimed in claim 15 , wherein the expandable member includes one or more distal tips configured to anchor the guide catheter to the tissues adjacent the patent foramen ovale.

23. A method for closing a patent foramen ovale, the method comprising:

advancing a guide catheter through the patent foramen ovale, between tissues adjacent the patent foramen ovale, the guide catheter containing an expandable member;

retracting the guide catheter to expose the expandable member;

expanding the expandable member outside of the patent foramen ovale and then moving the expanded expandable member into the patent foramen ovale;

apposing the tissues adjacent the patent foramen ovale; and

applying energy to the tissues adjacent the patent foramen ovale to weld the tissues adjacent the patent foramen ovale together.

24. A method as claimed in claim 23 , further comprising removing the expandable member from the patent foramen ovale after apposing the tissues adjacent the patent foramen ovale and before applying energy to the tissues adjacent the patent foramen ovale.

A method as claimed in claim 23 , wherein the applying energy to the tissues adjacent the patent foramen ovale includes a first welding and a second welding, and the expandable member is left in place in the patent foramen ovale during the first welding and then removed prior to the second welding.

26. A method as claimed in claim 23 , wherein the expandable member is self-expanding.

27. A method as claimed in claim 23 , wherein the expandable member includes one or more distal tips configured to locate the patent foramen ovale.

28. A method as claimed in claim 23 , wherein the expandable member includes one or more distal tips configured to cross the patent foramen ovale.

29. A method as claimed in claim 23 , wherein the expandable member includes one or more distal tips configured appose the tissues adjacent the patent foramen ovale.

30. A method as claimed in claim 23 , wherein the expandable member includes one or more distal tips configured to anchor the guide catheter to the tissues adjacent the patent foramen ovale.
