Abstract

An implantable graft, which may be inserted into a fistula tract to occlude the primary opening of the fistula, is provided. To prevent unintentional displacement of the graft or extrusion of the graft from the fistula of a patient, the graft may be provided with a cap that extends laterally from at least one end of the body of the graft, where the cap may be integral with the body of the graft, attachable to at least one end of the body of the graft, and/or moveable along the body of the graft. The graft may also have a tail that extends from one end of the body of the graft to assist in placement of the graft in a fistula tract. The graft may be an integral unit made of a single material, such as a heterograft material, or may include distinct components made of the same or different materials. Methods for closing a fistula tract are also provided.

Claims (33)

1. A fistula graft for treating a fistula having an opening in a first tissue wall, an opening in a second tissue wall, and a fistula tract extending therebetween, the fistula graft comprising:

a first cap configured to contact tissue wall portions adjacent to the first tissue wall opening;

an elongate graft body configured to fill at least a segment of the fistula tract;

a retaining element coupled to the first cap and passing through the elongate graft body and coupling the first cap to the elongate graft body; and

a second cap configured to contact tissue wall portions adjacent to the second tissue wall opening,

wherein the elongate graft body comprises a rolled or folded intact, acellular sheet of remodelable material that promotes cellular invasion and ingrowth within the acellular sheet and reconstruction of host tissue within the acellular sheet,

wherein the remodelable material is selected from the group consisting of a cadaveric material from a human donor and a heterograft material from a non-human donor.

2. The graft of claim 1 where the elongate graft body is generally conical.

3. The graft of claim 1 where the elongate graft body is generally cylindrical.

4. The graft of claim 1 where the first cap is generally disc-shaped.

5. The graft of claim 1 where the first cap further comprises a plurality of holes.

6. The graft of claim 1 where the first cap is expandable.

7. The graft of claim 1 where the remodelable material is extracellular matrix material.

8. The graft of claim 7 where the extracellular matrix material retains growth factors native to a source tissue from which it was obtained.

9. The graft of claim 8 where the extracellular matrix material is small intestinal submucosa.

10. The graft of claim 7 where the extracellular matrix material incorporates a bioactive agent.

11. The graft of claim 1 where the graft is configured for placement in an anorectal fistula.

12. The graft of claim 1 , where the second cap is configured to be received on the elongate graft body and slideable along the elongate graft body.

13. The graft of claim 1 where the elongate graft body is configured to receive a guidewire therethrough.

14. The graft of claim 1 where the elongate graft body further comprises a plurality of passages.

15. The graft of claim 1 , where the elongate graft body further comprises a bioactive agent.

16. The graft of claim 1 where the retaining element comprises a suture material.

17. The graft of claim 1 further comprising a gasket coupled to the second cap.

18. The graft of claim 17 where the gasket comprises a tissue ingrowth material.

19. The graft of claim 1 , wherein the first cap comprises an absorbable material.

20. The graft of claim 1 , wherein the first cap comprises a synthetic polymeric material.

21. The graft of claim 20 , wherein the synthetic polymeric material is a biostable polymer.

22. A fistula graft for treating a fistula having an opening in a first tissue wall, an opening in a second tissue wall, and a fistula tract extending therebetween, the fistula graft comprising:

a cap configured to contact tissue wall portions adjacent to the first tissue wall opening;

an elongate graft body having a first end and a second end;

a retaining element coupling the cap to the first end of the elongate graft body, the retaining element having a length sufficient to extend beyond the second end of the elongate graft body,

where the elongate graft body comprises a rolled or folded intact, acellular sheet of remodelable material that promotes cellular invasion and ingrowth within the acellular sheet and reconstruction of host tissue within the acellular sheet, wherein the remodelable material is selected from the group consisting of a cadaveric material from a human donor and a heterograft material from a non-human donor.

23. The graft of claim 22 where the retaining element comprises a suture material.
