Abstract

Devices, methods and kits are described for reducing intraocular pressure. The devices include a support that is implantable within Schlemm's canal and maintains the patency of the canal without substantially interfering with transmural fluid flow across the canal. The devices utilize the natural drainage process of the eye and can be implanted with minimal trauma to the eye. Kits include a support and an introducer for implanting the support within Schlemm's canal. Methods include implanting a support within Schlemm's canal, wherein the support is capable of maintaining the patency of the canal without substantial interference with transmural fluid flow across the canal.

Claims (28)

1. A method for reducing intraocular pressure comprising inserting a support circumferentially within Schlemm's canal such that the support is contained entirely within Schlemm's canal and the support contacts first and second interior wall portions of Schlemm's canal, the first interior wall portion being coincident with an outer peripheral boundary of the trabecular meshwork, and the second interior wall portion having collector channels extending therefrom, wherein the support maintains the patency of at least a portion of Schlemm's canal, completely traverses a central core of Schlemm's canal, and does not substantially interfere with transmural flow across Schlemm's canal.

2. The method of claim 1 further comprising dilating Schlemm's canal prior to insertion of the support.

3. The method of claim 1 further comprising anchoring the support to tissue.

4. The method of claim 1 comprising inserting at least two supports.

5. The method of claim 4 comprising positioning the at least two supports circumferentially adjacent to each other within Schlemm's canal.

6. The method of claim 4 comprising positioning the at least two supports circumferentially opposed to each other within Schlemm's canal.

7. The method of claim 4 wherein the at least two supports are connected together.

8. The method of claim 1 further comprising illuminating the support with a light source to visually enhance the position of the support.

9. The method of claim 1 further comprising altering the support using electromagnetic radiation.

10. The method of claim 9 wherein the alteration of the support is conducted after the support is implanted.

11. The method of claim 9 wherein the electromagnetic radiation comprises a laser having a wavelength absorbable by at least one localized portion of the support.

12. The method of claim 9 wherein the alteration comprises creation or enlargement of at least one aperture in the support.

13. The method of claim 1 wherein the support comprises elements that are connected together.

14. The method of claim 13 wherein the support is fabricated out of shapeable metal wires.

15. The method of claim 1 wherein the support is fabricated out of shapeable metal wires.

16. The method of claim 1 wherein the support contacts the interior wall of Schlemm's canal at least three points.

17. The method of claim 1 wherein an inside of Schlemm's canal is defined as a slightly arcuate cylinder C having a length L, extending circumferentially from a first end X 1 of the support to a second end X 2 of the support, and an inside radius R i , and the support contacts less than 100% of the surface area of the cylinder C.

18. The method of claim 17 wherein the support contacts less than 30% of the surface area of the cylinder C.

19. The method of claim 18 wherein the support contacts less than 10% of the surface area of the cylinder C.

20. The method of claim 19 wherein the support contacts less than 1% of the surface area of the cylinder C.

21. The method of claim 20 wherein the support contacts less than 0.1% of the surface area of the cylinder C.

22. The method of claim 1 , wherein the support does not substantially interfere with longitudinal flow along Schlemm's canal.

23. The method of claim 1 , wherein the support allows fluid to traverse Schlemm's canal.

24. The method of claim 1 , wherein the support comprises at least two adjacent beads and a connector linking the at least two adjacent beads.

25. The method of claim 24 , wherein the connector is flexible.

26. The method of claim 1 , wherein the support comprises at least three adjacent beads, wherein each bead is linked to its adjacent bead by a connector.

27. The method of claim 1 , wherein the support has an open network structure.

28. The method of claim 27 , wherein the support is fabricated out of shapeable metal wires.
