Abstract

An implantable neurostimulator system for treating movement disorders includes a sensor, a detection subsystem capable of identifying episodes of a movement disorder by analyzing a signal received from the sensor, and a therapy subsystem capable of applying therapeutic electrical stimulation to treat the movement disorder. The system treats movement disorders by detecting physiological conditions characteristic of an episode of symptoms of the movement disorder and selectively initiating therapy when such conditions are detected.

Claims (32)

1. A method for treating a movement disorder in a human patient with an implantable neurostimulator comprising:

receiving and continuously monitoring a signal from a sensor configured to sense electrical activity of the brain;

processing the signal to digitize it;

analyzing the signal to determine whether a tremor characteristic of an episode of the movement disorder is occurring, wherein analyzing the signal comprises:

measuring an amplitude of the signal;

calculating an average amplitude of the signal over a predetermined period of time;

comparing the average amplitude to a dynamic threshold that is a fixed offset of a long-term moving average amplitude wherein the long-term moving average amplitude is calculated from data in which no tremors occur; and

detecting a tremor event if the calculated average amplitude exceeds the dynamic threshold; and

delivering a treatment to the patient from the neurostimulator for so long as the tremor event is detected and ceasing delivery of the treatment when the tremor event is no longer detected thereby delivering a therapy from the implantable neurostimulator in response to the tremor.

2. The method of claim 1 wherein the tremor comprises an electrical oscillation correlated with at least one symptom of Parkinson's disease.

3. The method of claim 2 wherein the symptoms of Parkinson's disease are rigidity, bradykinesia, or combinations thereof.

4. The method of claim 1 wherein the treatment comprises an application of electrical stimulation.

5. The method of claim 1 , further comprising applying electrical stimulation pursuant to instructions programmed into the neurostimulator regardless of whether a tremor event is detected.

6. The method of claim 1 wherein processing the signal further comprises filtering the signal to isolate desired components and analyzing the signal further comprises using a half-wave detector to determine whether the amplitude of the isolated components exceeds the dynamic threshold.

7. The method of claim 1 , further comprising analyzing the signal to determine whether the patient is asleep and, if the patient is asleep, preventing delivery of any treatment to the patient from the neurostimulator for so long as the patient is asleep.

8. The method of claim 7 wherein analyzing the signal to determine whether the patient is asleep further comprises:

measuring an amplitude of the signal;

measuring a duration of the signal;

comparing the measured amplitude and measured duration to predetermined signal amplitude and duration ranges corresponding to specific sleep EEG patterns; and

if the comparison indicates a match between the measured amplitude and measured duration to one of the predetermined signal amplitude and duration ranges, detecting the condition that the patient is sleeping.

9. The method of claim 1 , further comprising delivering a treatment to the patient in the form of a drug when the tremor event is detected.

10. The method of claim 1 , further comprising receiving and processing a signal from an orientation sensor to determine whether the patient is prone.

11. The method of claim 1 , further comprising receiving and continuously monitoring a plurality of signals from a plurality of sensors, each sensor configured to sense electrical activity of the brain.

12. A method for treating a movement disorder in a human patient with an implantable neurostimulator comprising:

receiving and continuously monitoring a signal from a sensor configured to sense electrical activity of the brain;

processing the signal to digitize it;

analyzing the signal to determine whether a tremor characteristic of an episode of the movement disorder is occurring, wherein analyzing the signal comprises:

measuring an amplitude of the signal;

calculating an average amplitude of the signal over a predetermined period of time;

comparing the average amplitude to a dynamic threshold that is a predetermined percentage greater than a long-term moving average amplitude wherein the long-term moving average amplitude is calculated from data in which no tremors occur; and

detecting a tremor event if the calculated average amplitude exceeds the dynamic threshold; and

delivering a treatment to the patient from the neurostimulator for so long as the tremor event is detected and ceasing delivery of the treatment when the tremor event is no longer detected thereby delivering a therapy from the implantable neurostimulator in response to the tremor.
