Abstract

Devices and methods for the treatment of chronic total occlusions are provided. One disclosed embodiment comprises a method of facilitating treatment via a vascular wall defining a vascular lumen containing an occlusion therein. The method includes providing an intravascular device having a distal portion with a lumen extending therein; inserting the device into the vascular lumen; positioning the distal portion in the vascular wall; and directing the distal portion within the vascular wall.

Claims (26)

1. A method of facilitating treatment of a blood vessel, the blood vessel comprising a vascular wall defining a plurality of layers and a vascular lumen containing an occlusion therein, the plurality of layers including at least an outermost layer and an innermost layer, the innermost layer having an interior surface, the interior surface being located on a side of the innermost layer nearest a center of the vessel, the method comprising:

inserting a catheter into the blood vessel, the catheter having a balloon at a distal end, an inflation lumen in communication with the balloon, and a subintimal device lumen that is configured to receive a subintimal device;

expanding the balloon to cause an opening of the subintimal device lumen to be directed toward the vascular wall, wherein a distance remains between all expanding surfaces of the balloon and the interior surface of the innermost layer of the vascular wall when:

the balloon is expanded sufficiently to cause the opening of the subintimal device lumen to be directed toward the vascular wall, and

the subintimal device extends through the opening of the subintimal device lumen and through the innermost layer;

inserting a distal portion of the subintimal device in the subintimal device lumen of the catheter; and

advancing the distal portion of the subintimal device through less than all of the plurality of layers of the vascular wall.

2. The method of claim 1 , further including directing the subintimal device toward an intimal layer of the vascular wall.

3. The method of claim 2 , further including directing the subintimal device distal of the occlusion.

4. The method of claim 1 , wherein advancing the distal portion of the subintimal device through less than all of the plurality of layers of the vascular wall comprises advancing the distal portion of the subintimal device into the vascular wall.

5. The method of claim 1 , wherein advancing the distal portion of subintimal device comprises locating the distal portion at least partially coextensive with the occlusion.

6. The method of claim 5 , wherein upon advancement of the distal portion at least partially coextensive with the occlusion, a shape of the distal portion approximates an inside diameter of the vascular lumen of the blood vessel.

7. The method of claim 5 , further comprising providing an intravascular device, and positioning the intravasuclar device in the blood vessel such that a distal portion of the intravascular device traverses the occlusion.

8. The method of claim 7 , wherein providing the intravascular device comprises providing one or more of a guidewire, a laser device, an ultrasonic energy device, a mechanical dissection device, and an atherectomy device.

9. The method of claim 1 , wherein the distal lumen extends in a helical path around an exterior surface of the balloon.

10. The method of claim 1 , wherein a shape of the balloon is configured to be substantially the same as a shape of at least a portion of the vascular wall.

11. The method of claim 1 , further comprising advancing the distal portion of the subintimal device within the vascular wall such that a distal portion of the subintimal device traverses the occlusion.

12. The method of claim 1 , further comprising advancing the distal portion of the subintimal device between a first space defined by the vascular wall and a second space defined by the vascular wall.

13. The method of claim 12 , wherein the first space is proximal of the occlusion and the second space is coextensive with the occlusion.

14. The method of claim 12 , wherein the first space is coextensive with the occlusion and the second space is distal of the occlusion.

15. The method of claim 12 , wherein the first space is proximal of the occlusion and the second space is distal of the occlusion.

16. The method of claim 1 , wherein inserting the catheter into the blood vessel includes advancing the catheter over a guidewire.

17. The method of claim 1 , wherein the subintimal device lumen is separate from the inflation lumen.

18. The method of claim 1 , wherein, after the advancing step, the subintimal device is at least partially within a subintimal space and at least partially within the vascular lumen.

19. The method claim 1 , further including advancing the distal portion of the subintimal device within the vascular wall, and independently of the catheter, from a first position proximal the occlusion to a second position coextensive with the occlusion.

20. The method claim 1 , further including advancing the distal portion of the subintimal device within the vascular wall, and independently of the catheter, from a first position proximal the occlusion to a second position distal the occlusion.
