Abstract

Provided are procedures and instruments for preparing and transplanting osteochondral allograft plugs to a host bone to repair an articular cartilage defect. An allograft bone plug having a cartilage plate and cancellous bone tissue attached thereto is removed from a donor bone. The allograft plug is further shaped by removing or cutting away cancellous bone tissue to form a cancellous stalk extending from the cartilage plate. The formed cancellous stalk can have any suitable shape including cylindrical, conical, and rectilinear. At the recipient site of the host bone, a cutout is formed corresponding in shape to the allograft plug. The allograft plug is inserted into the cutout such that the cancellous stalk is retained in the host bone and the cartilage plate aligns with the condyle surface of the host bone. Aspects of the invention may also be applicable to preparing and transplanting osteochondral autograft plugs.

Claims (20)

1. A set of medical instruments for forming a transplantable osteochondral allograft, the set of instruments comprising:

a first tubular crown saw having a proximal end and an open distal end;

a clamp assembly, the clamp assembly including a base and a vertically extending clamp adapted to receive and constrain bone tissue, the clamp assembly movably coupled relative to the first tubular crown saw to align the first tubular crown saw with constrained bone tissue, the first tubular crown saw movable with respect to the vertically extending clamp;

a guide plate including a plate base, a first guide cylinder extending from the plate base with a first bore disposed therethrough, and a second guide cylinder insertable into the first guide cylinder wherein the first and second guide cylinders are concentric, the second guide cylinder with a second bore disposed therethrough;

a second tubular crown saw having a proximal end and an open distal end, wherein the first bore receives the first tubular crown saw and the second bore receives the second tubular crown saw;

a sleeve including a tubular sleeve body and a base plate having a guide aperture disposed therein, the tubular sleeve body having an inner diameter corresponding to an inner diameter of the second crown saw; and

a guide pin adapted to be received in the guide aperture.

2. The set of instruments of claim 1 , further comprising a cannulated burring shell having a conical inner surface.

The set of instruments of claim 1 , further comprising a pair of forceps having a first lever arm and second lever arm, the first and second lever arms pivotally joined at a pivot point, the forceps further having a first curved clamp and a second curved clamp formed at the distal end of the respective first and second lever arms, the first and second clamps coated with an elastomer.

4. The set of instruments of claim 1 , further comprising a vertical alignment plate coupled between the base and the first tubular crown saw, the first tubular crown saw extending through a passage in the vertical alignment plate to engage the constrained bone tissue.

5. A set of medical instruments for forming a transplantable osteochondral allograft, the set of instruments comprising:

a first tubular crown saw having a proximal end and an open distal end;

a clamp assembly including a vertically extending clamp adapted to receive and constrain bone tissue;

a guide plate including a plate base, a first guide cylinder extending from the plate base with a first bore disposed therethrough, and a second guide cylinder that is removably inserted into an assembled position with the first guide cylinder, the second guide cylinder with a second bore disposed therethrough, the first and second guide cylinders being concentric in the assembled position;

a second tubular crown saw wherein the first bore concentrically receives the first tubular crown saw and the second bore concentrically receives the second tubular crown saw;

a sleeve including a tubular sleeve body and a base plate having a guide aperture disposed therein, the tubular sleeve body having an inner diameter corresponding to an inner diameter of the second crown saw; and

a guide pin adapted to be received in the guide aperture.

6. The set of instruments of claim 5 , further comprising a cannulated burring shell having a conical inner surface.

The set of instruments of claim 5 , further comprising a pair of forceps having a first lever arm and second lever arm, the first and second lever arms pivotally joined at a pivot point, the forceps further having a first curved clamp and a second curved clamp formed at the distal end of the respective first and second lever arms, the first and second clamps coated with an elastomer.

8. The set of instruments of claim 5 , further comprising a vertical alignment plate coupled between the base and the first tubular crown saw, the first tubular crown saw extending through a passage in the vertical alignment plate to engage the constrained bone tissue.
