Abstract

The present invention provides sustained release compositions that can be manufactured easily while providing long duration of the pharmaceutically useful protein being delivered. The compositions comprise insoluble complexes of the pharmaceutically useful protein and a precipitating agent at an appropriate ratio to achieve a desired sustained release profile. The precipitating agent can be selected from a group of various agents including divalent metal cations, Hofmeister series salts and pH adjusters. Particularly provided are compositions comprising insulin that are suitable for pulmonary delivery.

48 . A dry powder composition comprising:

an amorphous insulin-zinc complex, wherein the molar ratio of zinc to insulin in said composition is at least about 6:1,

wherein said dry powder comprises particles having a mass median aerodynamic diameter (MMAD) of less than about 10 microns.

49 . The composition of claim 48 , wherein the molar ratio of zinc to insulin in said composition is at least about 10:1.

50 . The composition of claim 48 , wherein the molar ratio of zinc to insulin in said composition is at least about 15:1.

51 . The composition of claim 48 , wherein the molar ratio of zinc to insulin in said composition is at least about 20:1.

52 . The composition of claim 48 , wherein the composition provides sustained plasma levels of insulin for at least about 6 hours when administered to the lung of a mammalian subject.

53 . The composition of claim 48 , wherein the composition provides sustained plasma levels of insulin for at least about 7 hours when administered to the lung of a mammalian subject.

54 . The composition of claim 48 , wherein the dry powder is prepared by spray drying.

55 . The composition of claim 48 , wherein the particles have a mass median diameter (MMD) from about 1 to about 5 microns.

56 . The composition of claim 48 , wherein the particles have a mass median diameter (MMD) of less than about 4 μm.

57 . The composition of claim 48 , wherein the particles have a mass median aerodynamic diameter (MMAD) of less than about 5 microns.

58 . The composition of claim 48 , wherein the particles have a mass median aerodynamic diameter (MMAD) of less than about 3.3 microns.

59 . The composition of claim 8 , with an emitted dose of at least about 40%.

60 . The composition of claim 1 , in aerosolized form.

61 . The composition of claim 1 , further comprising a pharmaceutically acceptable excipient.

62 . The composition of claim 61 , wherein the excipient is a glass-forming excipient.

63 . The composition of claim 61 , wherein the excipient is a carbohydrate.

64 . The composition of claim 63 , wherein the carbohydrate is a sugar.

65 . The composition of claim 61 , wherein the excipient is selected from the group consisting of amino acids, amino acid dimers, amino acid trimers, and polyamino acids.

66 . A method for preparing a dry powder composition, comprising:

precipitating insulin with zinc at a molar ratio of zinc to insulin of at least about 6:1 to form an amorphous insulin-zinc complex; and

drying the amorphous insulin-zinc complex to form said dry powder, wherein said dry powder comprises particles having a mass median aerodynamic diameter (MMAD) of less than about 10 microns.

67 . The method of claim 66 , wherein the molar ratio of zinc to insulin in said composition is at least about 10:1.

68 . The method of claim 66 , wherein the molar ratio of zinc to insulin in said composition is at least about 15:1.

69 . The method of claim 66 , wherein the molar ratio of zinc to insulin in said composition is at least about 20:1.

70 . The method of claim 66 , wherein the composition provides sustained plasma levels of insulin for at least about 6 hours when administered to the lung of a mammalian subject.

71 . The method of claim 66 , wherein the composition provides sustained plasma levels of insulin for at least about 7 hours when administered to the lung of a mammalian subject.

72 . The method of claim 66 , wherein the drying comprises spray drying.

73 . The method of claim 66 , wherein the particles have a mass median diameter (MMD) from about 1 to about 5 microns.

74 . The method of claim 66 , wherein the particles have a mass median diameter (MMD) of less than about 4 μm.

75 . The method of claim 66 , wherein the particles have a mass median aerodynamic diameter (MMAD) of less than about 5 microns.

76 . The method of claim 66 , wherein the particles have a mass median aerodynamic diameter (MMAD) of less than about 3.3 microns.

77 . The method of claim 66 , wherein the composition has an emitted dose of at least about 40%.

78 . The method of claim 66 , wherein the composition is in aerosolized form.

79 . The method of claim 66 , wherein the composition further comprises a pharmaceutically acceptable excipient.

80 . The method of claim 79 , wherein the excipient is a glass-forming excipient.

81 . The method of claim 79 , wherein the excipient is a carbohydrate.

82 . The method of claim 81 , wherein the carbohydrate is a sugar.

83 . The method of claim 79 , wherein the excipient is selected from the group consisting of amino acids, amino acid dimers, amino acid trimers, and polyamino acids.

84 . A method of treating or ameliorating diabetes or related conditions comprising administering by inhalation a pharmaceutically effective amount of the composition of claim 48 .

85 . The method of claim 84 , wherein the composition is administered using a dry powder inhaler.

86 . The method of claim 84 , wherein the pharmaceutically effective amount is administered in a plurality of unit dosages.
