Abstract

Systems, methods, apparatus and devices for performing improved gynecologic and urologic procedures are disclosed. The system and devices provide simplified use and reduced risk of adverse events. Patient benefit is achieved through improved outcomes, reduced pain, especially peri-procedural pain, and reduced recovery times. The various embodiments enable procedures to be performed outside the hospital setting, such as in a doctor's office or clinic. An intrauterine access end procedure system includes a mechanical distension element, to eliminate the need for liquid distension media at pressure sufficient to create a risk of intravasation.

Claims (63)

1. An intrauterine access and procedure system, comprising:

an elongate, tubular sheath having a proximal end, a distal end and at least one lumen extending therethrough, the sheath adapted for transcervical access to a uterus;

a visualization element adapted to extend through the sheath for visualizing a site within the uterus;

a tissue removal mechanism adapted to extend through the sheath for removing targeted tissue from the site, wherein the tissue removal mechanism is an acoustic generator configured to remove tissue through non-direct contact between the acoustic generator and the targeted tissue; and

a distension element adapted to achieve an equivalent distension to that achieved by introduction of a liquid distension media at a pressure of at least 40 mm Hg.

2. The intrauterine access and procedure system as in claim 1 , wherein an outside diameter of the sheath is less than about 9 mm.

3. The intrauterine access and procedure system as in claim 1 , wherein an outside diameter of the sheath is less than about 7 mm.

4. The intrauterine access and procedure system as in claim 1 , wherein an outside diameter of the sheath is less than about 6 mm.

5. The intrauterine access and procedure system as in claim 1 , wherein the visualization element comprises a lens.

6. The intrauterine access and procedure system as in claim 1 , wherein the visualization element comprises an ultrasound transducer.

7. The intrauterine access and procedure system as in claim 1 , wherein the sheath comprises at least one radiopaque marker.

8. The intrauterine access and procedure system as in claim 1 , wherein the tissue removal mechanism comprises a tissue cutter.

9. The intrauterine access and procedure system as in claim 8 , wherein the tissue cutter comprises an oscillating cutter.

10. The intrauterine access and procedure system as in claim 8 , wherein the tissue cutter comprises a rotating cutter.

11. The intrauterine access and procedure system as in claim 1 , wherein the tissue removal mechanism comprises a tissue morcellator.

12. The intrauterine access and procedure system as in claim 1 , wherein the tissue removal mechanism comprises an ablation element.

13. The intrauterine access and procedure system as in claim 1 , further comprising a pull wire for deflecting the distal end of the sheath.

14. The intrauterine access and procedure system as in claim 1 , further comprising an inflatable balloon carried by the sheath.

15. The intrauterine access and procedure system as in claim 1 , further comprising a tissue penetrating assembly.

16. The intrauterine access and procedure system as in claim 1 , further comprising a stabilization device.

17. The intrauterine access and procedure system as in claim 1 , further comprising a drug delivery assembly.

18. The intrauterine access and procedure system as in claim 1 , wherein the distension element comprises an expandable basket.

19. The intrauterine access and procedure system as in claim 1 , further comprising a volume occupying device.

20. The intrauterine access and procedure system as in claim 1 , wherein the acoustic generator comprises a sound crystal configured to generate subsonic sound waves.

21. The intrauterine access and procedure system as in claim 20 , wherein the subsonic sound waves are configured to provide an impedance mismatch to destroy the targeted tissue.

22. The intrauterine access and procedure system as in claim 21 , wherein to destroy the targeted tissue comprises denaturing the targeted tissue.

23. The intrauterine access and procedure system as in claim 21 , wherein to destroy the targeted tissue comprises dislodging the targeted tissue from the uterus.

24. An intrauterine access and procedure system, comprising:

an elongate, tubular sheath having a proximal end, a distal end and at least one lumen extending therethrough, the sheath adapted for transcervical access to a uterus;

a visualization element adapted to extend through the sheath for visualizing a site within the uterus;

a tissue removal mechanism adapted to extend through the sheath for removing tissue from the site, wherein the tissue removal mechanism is an acoustic generator configured to generate subsonic sound waves for providing an impedance mismatch to destroy a targeted tissue; and

a distension element adapted to achieve an equivalent distension to that achieved by introduction of a liquid distension media at a pressure of at least 40 mm Hg.

25. The intrauterine access and procedure system as in claim 24 , wherein an outside diameter of the sheath is less than about 9 mm.

26. The intrauterine access and procedure system as in claim 24 , wherein an outside diameter of the sheath is less than about 7 mm.

27. The intrauterine access and procedure system as in claim 24 , wherein an outside diameter of the sheath is less than about 6 mm.

28. The intrauterine access and procedure system as in claim 24 , wherein the visualization element comprises a lens.

29. The intrauterine access and procedure system as in claim 24 , wherein the visualization element comprises an ultrasound transducer.

30. The intrauterine access and procedure system as in claim 24 , wherein the sheath comprises at least one radiopaque marker.

31. The intrauterine access and procedure system as in claim 24 , wherein the tissue removal mechanism comprises a tissue cutter.

32. The intrauterine access and procedure system as in claim 31 , wherein the tissue cutter comprises an oscillating cutter.

33. The intrauterine access and procedure system as in claim 31 , wherein the tissue cutter comprises a rotating cutter.

34. The intrauterine access and procedure system as in claim 24 , wherein the acoustic generator is configured to remove tissue through non-direct contact.

35. The intrauterine access and procedure system as in claim 24 , wherein the tissue removal mechanism comprises a tissue morcellator.

36. The intrauterine access and procedure system as in claim 24 , wherein the tissue removal mechanism comprises an ablation element.

37. The intrauterine access and procedure system as in claim 24 , further comprising a pull wire for deflecting the distal end of the sheath.

38. The intrauterine access and procedure system as in claim 24 , further comprising an inflatable balloon carried by the sheath.

44. The intrauterine access and procedure system as in claim 24 , wherein the acoustic generator comprises a sound crystal configured to generate subsonic sound waves.

45. The intrauterine access and procedure system as in claim 24 , wherein to destroy the targeted tissue comprises denaturing the targeted tissue.

46. The intrauterine access and procedure system as in claim 24 , wherein to destroy the targeted tissue comprises dislodging the targeted tissue from the uterus.

47. The intrauterine access and procedure system, comprising:

an elongate, tubular sheath having a proximal end, a distal end and at least one lumen extending therethrough, the sheath adapted for transcervical access to a uterus;

a visualization element adapted to extend through the sheath for visualizing a site within the uterus;

a tissue removal mechanism adapted to extend through the sheath for removing tissue from the site, wherein the tissue removal mechanism comprises an acoustic generator configured to generate subsonic sound waves to be conducted from the acoustic generator to the tissue through a fluid directly between the acoustic generator and the tissue to thereby destroy the tissue; and

a distension element adapted to achieve an equivalent distension to that achieved by introduction of a liquid distension media at a pressure of at least 40 mm Hg.

48. The intrauterine access and procedure system as in claim 47 , wherein acoustic generator transmits subsonic sound waves to the tissue through non-direct contact between the acoustic generator and the tissue.

49. The intrauterine access and procedure system as in claim 47 , wherein to destroy the tissue comprises denaturing the tissue.

50. The intrauterine access and procedure system as in claim 47 , wherein to destroy the tissue comprises dislodging the tissue from the uterus.

51. The intrauterine access and procedure system as in claim 47 , wherein acoustic generator is configured to destroy the tissue without a vibrating member contacting the tissue.
