Abstract

Architectures and techniques for treating conditions of the eye, such as presbyopia, utilize sources of treatment energy, such as electromagnetic energy emitting devices, to implement non-corneal manipulations. According to these devices and methods, the sources of treatment energy are activated to direct energy onto parts of the eye, such as the conjunctiva and sclera, to treat presbyopia. The treatments can affect at least one property of the eye and enhance an accommodation of the eye.

Claims (42)

1. An endoscopic device for facilitating an application of electromagnetic energy to a sclera beneath a conjunctiva of an eye, the endoscopic device comprising:

an electromagnetic energy outputting fiber optic tip having a distal end constructed to be inserted beneath a conjunctiva of an eye and being configured to endoscopically output electromagnetic energy, the electromagnetic energy being outputted in a form of one or more tissue treatments onto one or more treatment areas of the sclera of the eye;

a visualization fiber optic coupled to the electromagnetic energy outputting fiber optic tip, the visualization fiber optic being configured to enable visualizing through the visualization fiber optic as the electromagnetic energy outputting fiber optic tip imparts the one or more tissue treatments onto the one or more treatment areas of the sclera of the eye; and

coupling structure contacting and holding the electromagnetic energy outputting fiber optic tip in proximity to the visualization fiber optic, the coupling structure holding and orienting the visualization fiber optic to focus a line of sight of the visualization fiber optic onto the distal end of the electromagnetic energy outputting fiber optic tip, the coupling structure comprising a relatively low-profile size and shape sufficient to enable insertion of the electromagnetic energy outputting fiber optic tip, the visualization fiber optic and the coupling structure under the conjunctiva of the eye and further to enable application of the one or more tissue treatments onto the one or more treatment areas of the sclera of the eye while the electromagnetic energy outputting fiber optic tip, the visualization fiber optic and the coupling structure are beneath the conjunctiva and while the impartation of the one or more tissue treatments onto the one or more treatment areas of the sclera of the eye is being viewed in real-time by a user through the visualization fiber optic.

2. The endoscopic device as set forth in claim 1 , the visualization fiber optic comprising a lens with a water-repelling coating disposed thereon.

3. The endoscopic device as set forth in claim 1 , further comprising a visualization light source.

4. The endoscopic device as set forth in claim 1 , further comprising a fluid output.

5. The endoscopic device as set forth in claim 4 , wherein:

the visualization fiber optic comprises a lens; and

the fluid output is oriented and configured to direct one or more of a gelled water and a viscoelastic gel over the lens.

6. The endoscopic device as set forth in claim 1 , further comprising a calibrated aspiration source.

7. The endoscopic device as set forth in claim 1 , the visualization fiber optic being disposed in a cannula, which comprises the electromagnetic energy outputting fiber optic tip, a visualization light source, a fluid output and an aspiration source.

8. The endoscopic device as set forth in claim 1 , wherein:

at the distal end an optical axis of the visualization fiber optic is not parallel with an optical axis of the electromagnetic energy outputting fiber optic tip; and

the optical axis in a vicinity of a distal end of the visualization fiber optic is aligned to intersect with the distal end of the electromagnetic energy outputting fiber optic tip.

9. The endoscopic device as set forth in claim 1 , wherein, at the distal end, an optical axis of the visualization fiber optic is substantially parallel to an optical axis of the electromagnetic energy outputting fiber optic tip.

10. The endoscopic device as set forth in claim 1 , wherein the visualization fiber optic encircles the electromagnetic energy outputting fiber optic tip in a vicinity of the distal end.

11. The endoscopic device as set forth in claim 10 , wherein:

the visualization fiber optic surrounds the electromagnetic energy outputting fiber optic tip at the distal end; and

the coupling structure surrounds and encases both the visualization fiber optic and the electromagnetic energy outputting fiber optic tip at the distal end.

12. The endoscopic device as set forth in claim 1 , wherein the coupling structure has a circular cross-sectional area at the distal end.

13. The endoscopic device as set forth in claim 1 , wherein the coupling structure has an oval cross-sectional area at the distal end.

14. The endoscopic device as set forth in claim 1 , wherein the coupling structure comprises a barb at the distal end.

The endoscopic device as set forth in claim 14 , the barb facilitating insertion of the distal end of the electromagnetic energy outputting fiber optic tip through the conjunctiva with relative ease but resisting removal of the distal end of the electromagnetic energy outputting fiber optic tip from within the conjunctiva once inserted.

The endoscopic device as set forth in claim 1 , wherein the coupling structure connects the visualization fiber optic to the electromagnetic energy fiber optic tip and fixes an orientation of the visualization fiber optic relative to the distal end of the electromagnetic energy fiber optic tip.

17. A method for treating an eye in need of one or more of a physiological and a vision correction, the method comprising:

inserting an endoscopic device beneath a conjunctiva of the eye, the endoscopic device comprising a visualization fiber optic coupled to an electromagnetic energy outputting fiber optic tip;

visualizing through the endoscopic device one or more treatment areas of the eye; and

projecting treatment energy in a form of electromagnetic radiation, the treatment energy being projected under the conjunctiva to form one or more tissue treatments in the one or more treatment areas.

wherein the one or more tissue treatments comprises (a) a first grouping in first area between a first and a second rectus muscle, (b) a second grouping in a second area between the second and third rectus muscle, (c) a third grouping in a third area between the third and a forth rectus muscle, and (d) a forth grouping in a forth area between the forth and the first rectus muscle.

18. The method for treating an eye in need of one or more of a physiological and a vision correction, the method comprising:

inserting an endoscopic device beneath a conjunctive of the eye, the endoscopic device comprising a visualization fiber optic coupled to an electromagnetic energy outputting fiber optic tip;

visualizing through the endoscopic device one or more treatment areas of the eye; and

projecting treatment energy in a form of electromagnetic radiation, the treatment energy being projected under the conjunctiva to form one or more tissue treatments in the one or more treatment areas;

wherein the one or more tissue treatments comprises four groupings; and

each of the four groupings comprises at least eight tissue treatments and is centered between a pair of adjacent ones of the rectus muscles of the eye.

19. The method for treating an eye in need of one or more of a physiological and a vision correction, the method comprising:

inserting an endoscopic device beneath a conjunctive of the eye, the endoscopic device comprising a visualization fiber optic coupled to an electromagnetic energy outputting fiber optic tip;

visualizing through the endoscopic device one or more treatment areas of the eye; and

projecting treatment energy in a form of electromagnetic radiation, the treatment energy being projected under the conjunctiva to form one or more tissue treatments in one or more treatment areas;

wherein the projecting comprises ablating one or more of a conjunctiva and a sclera of the eye; and

wherein the tissue treatments are formed to depths between about 90% and 99% of a thickness of a sclera of the eye.
