Method and device to prevent ventilator acquired pneumonia using nitric oxide
A respiratory assist device and method for the prevention of ventilator acquired pneumonia in a patient is described. The respiratory assist device administers nitric oxide to the oropharyngeal area in order to decontaminate or prevent the contamination of secretions that collect in the oropharyngeal area during intubation of the patient. The respiratory assist device and method may be adapted for use, for example, as an endotracheal tube or as a tracheotomy tube.
1 . A method of decontaminating secretions in an intubated mammal, comprising delivering a concentration of nitric oxide gas to the secretions in the intubated mammal sufficient to substantially decontaminate the secretions.
2 . The method of claim 1 , wherein the mammal is intubated with an endotracheal tube or a tracheotomy tube equipped with a balloon cuff, and secretions collect at the balloon cuff.
3 . The method of claim 1 , wherein the concentration of nitric oxide gas is from about 100 ppm to about 20,000 ppm.
4 . The method of claim 1 , wherein the concentration of nitric oxide gas is from about 160 ppm to about 200 ppm.
5 . The method of claim 1 , further comprising aspirating the secretions from the intubated mammal.
6 . A method of decontaminating the oropharyngeal area in an intubated mammal, the method comprising the steps of:
sealing an area of the oropharyngeal from the lungs; and
delivering an effective concentration of nitric oxide gas to the sealed area of the oropharyngeal.
7 . The method of claim 6 , wherein the mammal is intubated with an endotracheal tube or a tracheotomy tube equipped with a balloon cuff, and wherein sealing an area of the oropharyngeal comprises inflating the balloon cuff.
8 . The method of claim 6 , wherein the concentration of nitric oxide gas is from about 100 ppm to about 20,000 ppm.
9 . The method of claim 6 , wherein the concentration of nitric oxide gas is from about 160 ppm to about 200 ppm.
10 . The method of claim 6 , further comprising aspirating the sealed area of the oropharyngeal.
11 . A method of preventing ventilator acquired pneumonia caused by secretions in an intubated mammal, comprising:
sealing an area of the oropharyngeal from the lungs so that secretions collect in the sealed area; and
delivering a concentration of nitric oxide gas to the sealed area sufficient to substantially decontaminate the secretions that collect in the sealed area.
12 . The method of claim 11 , wherein the mammal is intubated with an endotracheal tube or a tracheotomy tube equipped with a balloon cuff, and wherein sealing an area of the oropharyngeal from the lungs comprises inflating the balloon cuff.
13 . The method of claim 11 , wherein the concentration of nitric oxide gas is from about 100 ppm to about 20,000 ppm.
14 . The method of claim 11 , wherein the concentration of nitric oxide gas is from about 160 ppm to about 200 ppm.
15 . The method of claim 11 , further comprising aspirating the sealed area of the oropharyngeal.
16 . A method of mechanically ventilating a mammal without causing ventilator acquired pneumonia, comprising:
intubating the mammal's trachea;
mechanically ventilating the mammal's lungs;
sealing an area of the oropharyngeal from the lungs so that secretions collect in the sealed area; and
delivering a concentration of nitric oxide gas to the sealed area sufficient to substantially decontaminate secretions that collect in the sealed area.
17 . The method of claim 16 , wherein the mammal is intubated with an endotracheal tube or a tracheotomy tube equipped with a balloon cuff, and wherein sealing an area of the oropharyngeal from the lungs comprises inflating the balloon cuff.
18 . The method of claim 16 , wherein the concentration of nitric oxide gas is from about 100 ppm to about 20,000 ppm.
19 . The method of claim 16 , wherein the concentration of nitric oxide gas is from about 160 ppm to about 200 ppm.
20 . A method of mechanically ventilating a mammal without causing ventilator acquired pneumonia, comprising:
intubating the mammal's trachea with a catheter having an inflatable balloon cuff positioned at about the distal end of the catheter;
mechanically ventilating the mammal's lungs though the catheter;
inflating the balloon cuff to seal an area of the oropharyngeal from the lungs so that secretions collect in the sealed area; and
delivering a concentration of nitric oxide gas to the sealed area sufficient to substantially decontaminate secretions that collect in the sealed area.
21 . The method of claim 20 , wherein the concentration of nitric oxide gas is from about 100 ppm to about 20,000 ppm.
22 . The method of claim 20 , wherein the concentration of nitric oxide gas is from about 160 ppm to about 200 ppm.
23 . The method of claim 20 , further comprising aspirating the secretions from the intubated mammal.
