Abstract

A tamponade trocar includes an elongate balloon having a closed distal end and is adapted to expand from a small diameter to a large diameter. A cannula is positioned at a proximal portion within the balloon lumen. A rigid stylet is removably positioned within the balloon and cannula lumens. A distal end of the stylet supported balloon is inserted into a perforation in a body wall and advanced into a body cavity while the balloon is unexpanded. The balloon is expanded and the cannula is advanced to a distal portion of the balloon lumen and across the body wall. At this stage, the perforation is dilated and in compressive tamponade. The proximal portion of the balloon may be removed and a seal housing may be coupled to the proximal end of the cannula. The distal end of the balloon may be punctured and opened, making the trocar ready for use.

Claims (47)

1. A tamponade trocar, comprising:

an elongate balloon, the balloon having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the distal end of the balloon being closed and gastight, the balloon being adapted to expand from a first, low-profile, small diameter condition to a second, fixed, large diameter condition;

a cannula having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the cannula being positioned within the lumen of the balloon;

wherein, with the balloon in the first, low profile, small diameter condition, the balloon is configured to contain the cannula and to have a reduced portion distal to the distal end of the cannula, and the cannula being is prevented from transitioning positions within the lumen of the balloon, and

wherein with the balloon in the second, fixed, large diameter condition, the internal diameter of the balloon being larger than the outside diameter of the cannula, and the cannula is free to transition positions within the lumen of the balloon.

2. The tamponade trocar of claim 1 , wherein an application of inflation pressure at a proximal portion of the balloon causes the expansion from the first, low-profile, small diameter condition to the second, fixed, large diameter condition.

3. The tamponade trocar of claim 1 , further comprising:

a valve positioned at a proximal portion of the balloon, the valve being adapted to communicate between the balloon lumen and the atmosphere external the balloon,

wherein an application of inflation pressure through the valve causes the expansion of the balloon from the first, low-profile, small diameter condition to the second, fixed, large diameter condition.

4. The tamponade trocar of claim 1 , the balloon being constructed of a non-distensible material.

5. The tamponade trocar of claim 1 , the balloon having a length that is at least twice the length of the cannula.

6. The tamponade trocar of claim 1 , the balloon having a length that is less than twice the length of the cannula.

7. The tamponade trocar of claim 1 , the balloon having an elongated toroid shape forming a tubular channel throughout the length of the balloon, the tubular channel of the balloon being coupled to an outer body portion of the balloon at a distal end of the outer body portion of the balloon and at a proximal portion of the outer body portion of the balloon.

8. A tamponade trocar, comprising:

an elongate balloon, the balloon having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the distal end of the balloon being closed and gastight, the balloon being adapted to expand from a first, low-profile, small diameter condition to a second, fixed, large diameter condition;

a cannula having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the cannula being positioned within the lumen of the balloon at a first, proximal portion of the balloon;

a rigid tensioning stylet having a proximal end and a distal end, the stylet being removably positioned within the lumen of the balloon and the lumen of the cannula, the distal end of the stylet being positioned at the distal end of the balloon, the proximal end of the stylet being positioned proximal to the proximal end of the balloon, the stylet being configured to provide columnar strength to the balloon for insertion of the distal end of the balloon; and

a trocar seal housing adapted to be coupled to the proximal end of the cannula,

wherein, with the balloon in the first, low profile, small diameter condition, the cannula is prevented from transitioning positions within the lumen of the balloon, and

with the balloon in the second, fixed, large diameter condition, the internal diameter of the balloon being larger than the outside diameter of the cannula, and the cannula is free to transition from the first, proximal portion of the balloon to a second, distal portion of the balloon.

9. The tamponade trocar of claim 8 , wherein an application of inflation pressure at the proximal portion of the balloon causes the expansion from the first, low-profile, small diameter condition to the second, fixed, large diameter condition.

10. The tamponade trocar of claim 8 , further comprising:

a valve positioned proximate the proximal end of the balloon, the valve being adapted to communicate between the balloon lumen and the atmosphere external the balloon,

wherein the application of inflation pressure through the valve causing the expansion of the balloon from the first, low-profile, small diameter condition to the second, fixed, large diameter condition.

11. The tamponade trocar of claim 10 , the distal end of the stylet being positioned at the distal end of the balloon and the proximal end of the stylet being positioned proximal to the valve.

12. The tamponade trocar of claim 8 , the balloon being constructed of a non-distensible material.

13. The tamponade trocar of claim 8 , the balloon being constructed from at least one of polyethylene, polyurethane, and nylon.

14. The tamponade trocar of claim 8 , the balloon having a length that is at least twice the length of the cannula.

15. The tamponade trocar of claim 8 , the balloon having a length that is less than twice the length of the cannula.

16. The tamponade trocar of claim 8 , the distal end of the balloon being rounded.

17. The tamponade trocar of claim 8 , the distal end of the balloon being tapered.

18. The tamponade trocar of claim 8 , the balloon having an elongated toroid shape forming a tubular channel throughout the length of the balloon, the tubular channel of the balloon being coupled to an outer body portion of the balloon at a distal end of the outer body portion of the balloon and at a proximal portion of the outer body portion of the balloon.

19. A tamponade trocar, comprising:

an elongate balloon, the balloon having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the distal end of the balloon being closed and gastight, the balloon being adapted to expand from a first, low-profile, small diameter condition to a second, fixed, large diameter condition, the balloon being constructed of a non-distensible material;

a valve positioned proximate the proximal end of the balloon, the valve being adapted to communicate between the balloon lumen and the atmosphere external the balloon;

a cannula having a proximal end, a distal end, and a lumen extending between the proximal end and the distal end, the cannula being positioned within the lumen of the balloon at a first, proximal portion of the balloon;

a trocar seal housing adapted to be coupled to the proximal end of the cannula; and

a rigid tensioning stylet having a proximal end and a distal end, the stylet being removably positioned within the lumen of the balloon and the lumen of the cannula, the distal end of the stylet being positioned at the distal end of the balloon and the proximal end of the stylet being positioned proximal to the valve, the stylet being configured to provide columnar strength to the balloon for insertion of the distal end of the balloon,

wherein, with the balloon in the first, low profile, small diameter condition, the balloon is configured to contain the cannula and to have a reduced portion distal to the distal end of the cannula, and the cannula is prevented from transitioning positions within the lumen of the balloon,

with the balloon in the second, fixed, large diameter condition, the internal diameter of the balloon being larger than the outside diameter of the cannula, and the cannula is free to transition from the first, proximal portion of the balloon to a second, distal portion of the balloon, and

wherein an application of inflation pressure through the valve causes the expansion of the balloon from the first, low-profile, small diameter condition to the second, fixed, large diameter condition.

20. The tamponade trocar of claim 19 , the balloon being constructed from at least one of polyethylene, polyurethane, and nylon.

21. The tamponade trocar of claim 19 , the balloon having a length that is at least twice the length of the cannula.

22. The tamponade trocar of claim 19 , the balloon having a length that is less than twice the length of the cannula.

23. The tamponade trocar of claim 19 , the distal end of the balloon being rounded.

24. The tamponade trocar of claim 19 , the distal end of the balloon being tapered.

25. The tamponade trocar of claim 19 , the balloon having an elongated toroid shape forming a tubular channel throughout the length of the balloon, the tubular channel of the balloon being coupled to an outer body portion of the balloon at a distal end of the outer body portion of the balloon and at a proximal portion of the outer body portion of the balloon.
