Abstract

A system for correcting a spinal deformity includes an implant fixed to one side of a vertebra and a rod extending along an axis of the spine on a second side of the vertebra. An adjustment member, which may include a reel, is coupled to the rod. A force directing member, such as a cable, extends between the rod and the adjustment member. The force directing member is retractable toward and extendible from the adjustment member. A method of correcting spinal deformity includes providing an implant, a rod, an adjustment member coupled to the rod, and a force directing member extending between the rod and the adjustment member. The adjustment member can be retractable toward and extendible from the adjustment member.

Claims (63)

1. A system for correcting a spinal deformity, the system comprising:

a plurality of implants, each implant being configured to be fixed to a first side of a different vertebra;

a rod adapted to extend generally along an axis parallel to an axis of the spine and on a second side of the vertebrae;

a plurality of adjustment members coupled to the rod, the adjustment members being spaced along the rod, each adjustment member comprising a reel; and

a plurality of flexible connectors, each flexible connector being coupled to a corresponding adjustment member and adapted to extend between the corresponding adjustment member and a corresponding implant, each flexible connector being retractable toward and extendible from the corresponding adjustment member.

2. The system of claim 1 , wherein at least one of the flexible connectors is a cable.

3. The system of claim 1 , wherein at least one of the adjustment members further comprises a housing at least partially surrounding the corresponding reel.

4. The system of claim 1 , wherein at least one of the reels is rotatable on an axis normal to the axis of the rod.

5. The system of claim 1 , wherein at least one of the reels is rotatable on an axis generally in line with the axis of the rod.

6. The system of claim 1 , further comprising at least one gear configured to turn at least one of the reels.

7. The system of claim 1 , further comprising an implantable motor configured to drive at least one of the adjustment members.

8. The system of claim 7 , wherein the motor is a stepper motor.

9. The system of claim 7 , further comprising an implantable power source configured to supply power to the motor.

10. The system of claim 1 , wherein at least one of the implants comprises a first portion configured for fixation to a pedicle on the first side of a vertebra and a second portion configured to extend to the second side of the vertebra when the first portion is fixed to the pedicle.

11. The system of claim 10 , wherein the first portion is a pedicle screw.

12. The system of claim 10 , wherein the second portion is configured to pass through a spinous process of the vertebra.

13. The system of claim 12 , further comprising a load-spreading member configured to spread load applied by the flexible connector to the spinous process.

14. A system for correcting a spinal deformity, the system comprising:

means for establishing a desired orientation of vertebrae;

means for applying force to an individual vertebra;

means for directing force to the force applying means; the force directing means extending between the force applying means and the orientation establishing means; and

means for retracting the force directing means toward the orientation establishing means, wherein the force directing means is directly coupled to the retracting means.

15. The system of claim 14 , further comprising means for extending the force directing means away from the orientation establishing means.

16. The system of claim 14 , wherein the means for retracting is further adapted to extend the force directing means away from the orientation establishing means.

17. The system of claim 14 , wherein the means for directing force comprises a cable.

18. The system of claim 14 , wherein the means for retracting comprises a reel.

19. The system of claim 18 , wherein the means for retracting further comprises a gear configured to turn the reel.

20. The system of claim 19 , wherein the means for retracting further comprises an implantable motor.

21. A system for correcting a spinal deformity, comprising:

an elongate rod;

a plurality of adjustment members coupled to the rod and spaced apart along the rod;

a plurality of flexible force-directing members, each of the force-directing members being directly attached to one of the adjustment members and adapted to be drawn toward the rod by the adjustment member to which it is attached; and

a plurality of implants, each configured to connect to a vertebra of a spine and to a force directing member, allowing a plurality of vertebrae to each be drawn by the force directing members and the adjustment members toward the rod.

22. The system of claim 21 , wherein at least one of the flexible force-directing members comprises a cable.

23. The system of claim 21 , wherein at least one of the adjustment members comprises a reel.

24. The system of claim 23 , further comprising at least one gear configured to turn the reel.

25. The system of claim 21 , further comprising an implantable motor configured to drive at least one of the adjustment members.

26. A method of correcting a spinal deformity, the method comprising:

affixing an implant to a first side of a vertebra;

positioning a rod on a second side of the vertebra so that it extends generally parallel to an axis of the spine;

providing at least one adjustment member positioned along the rod, the adjustment member comprising a winding mechanism;

positioning at least one flexible force directing member so that it extends between the adjustment member and the implant, the flexible force directing member being directly coupled to the adjustment member; and

applying a force to the at least one force directing member with the adjustment member, thereby moving the vertebra toward the rod.

27. The method of claim 26 , wherein the force is applied percutaneously.

28. The method of claim 26 , wherein the force is applied non-invasively.

29. The method of claim 28 , wherein the force is applied using HF energy.

30. The method of claim 28 , wherein the force is applied using an implanted power source.

31. The method of claim 30 , wherein the force is applied by an implanted motor.

32. The method of claim 26 , wherein the method comprises affixing a plurality of the implants to the first side of a plurality of vertebrae, providing a plurality of the adjustment members positioned along the rod, and positioning a plurality of the force directing members between adjustment members and implants, and then applying a force to each of the plurality of force directing members.

33. The method of claim 32 , wherein a different force is applied to each force directing member.

34. The method of claim 26 , wherein the force directing member is a wire.

35. The method of claim 26 , wherein the force directing member is a cable.

36. The method of claim 26 , wherein the adjustment member comprises a reel.

37. The method of claim 36 , further comprising providing a housing at least partially surrounding the reel.

38. The method of claim 36 , wherein the reel is rotatable on an axis normal to the axis of the rod.

39. The method of claim 36 , wherein the reel is rotatable on an axis generally in line with the axis of the rod.

40. The method of claim 36 , further comprising at least one gear configured to turn the reel.

41. The method of claim 26 , wherein the implant comprises a first portion configured for fixation to a pedicle on the first side of a vertebra and a second portion configured to extend to the second side of the vertebra when the first portion is fixed to the pedicle.

42. The method of claim 41 , wherein the first portion is a pedicle screw.

43. The method of claim 41 , wherein the second portion is configured to pass through a spinous process of the vertebra.

44. The method of claim 43 , further comprising a load-spreading member configured to spread load applied by the force directing member to the spinous process.

45. The method of claim 26 , wherein the implant is affixed to multiple locations on the vertebra such that applying the force to the force directing member with the adjustment member both moves the vertebra toward the rod and derotates the vertebra.

46. The method of claim 26 , further comprising fusing at least two vertebrae after moving the vertebra toward the rod.
