Abstract

The methods disclosed herein are directed to altering gaseous flow within a lung to improve the expiration cycle of, for instance, an individual having Chronic Obstructive Pulmonary Disease. More particularly, these methods produce and maintain collateral openings or channels through the airway wall so that expired air is able to pass directly out of the lung tissue to facilitate both the exchange of oxygen ultimately into the blood and/or to decompress hyper-inflated lungs. Devices and methods apply cryo-energy to maintain the patency of the surgically created openings.

Claims (25)

1 . A minimally invasive medical method comprising:

identifying a target site along an airway, said airway having a wall;

creating an opening through the wall of the airway at said target site; and

applying cryo-energy to said opening to maintain the patency of the opening.

2 . The method of claim 1 wherein said step of applying cryo-energy comprises inserting a dilating member into said opening.

3 . The method of claim 2 wherein said dilating member is an inflatable balloon member.

4 . The method of claim 3 wherein said balloon member is expanded with a cold fluid.

5 . The method of claim 4 wherein said cryo-energy has an associated temperature in the range of −10 to −50 degrees Celsius.

6 . The method claim 4 wherein said fluid is one fluid selected from the group of consisting of liquid nitrogen and nitrous oxide.

7 . A method for treating chronic obstructive pulmonary disease comprising modifying the wound healing response of a surgically created opening through the wall of an airway within the lung with a cold therapy.

8 . The method of claim 6 wherein said cold therapy comprises the step of placing an expandable member within said opening, and said expanding member comprising a cold fluid.

9 . The method of claim 8 wherein said expanding member is a component of a catheter having a flexible shaft.

10 . The method of claim 9 wherein said catheter is adapted to operate through a working channel of a bronchoscope.

11 . A minimally invasive method for maintaining the patency of a collateral channel in an animal, said method comprising:

manipulating an instrument through an oral opening and into an airway of a lung;

applying cryo-energy with said instrument to a surgically created opening extending through the airway wall to the parenchyma such that the patency of the opening thereby maintained.

12 . The method of claim 11 wherein said step of applying cryo-energy comprises inserting a dilating member into said opening.

13 . The method of claim 12 wherein said dilating member is an inflatable balloon member.

14 . The method of claim 13 wherein said balloon member is expanded with a cold fluid.

15 . The method of claim 14 wherein said cryo-energy has an associated temperature in the range of −10 to −50 degrees Celsius.

16 . The method claim 14 wherein said fluid is one fluid selected from the group of consisting of liquid nitrogen and nitrous oxide.

17 . The method of claim 11 further comprising creating said opening through the airway wall prior to said step of applying cryo-energy.

18 . The method of claim 17 wherein said creating step is carried out with a channel creation means for creating an opening, said means different from said instrument for applying cryo-energy.

19 . The method of claim 1 further comprising applying a second cold therapy to said opening after a time period from said first cold therapy, said time period greater than one month.

20 . The method of claim 1 further comprising inserting an instrument through said opening to carry out one step of obtaining a tissue sample and ablating the tissue.
