Abstract

Methods for formulating immediate and sustained release anti-infectives and delivery of such for treatment of respiratory tract infections and other medical conditions, and devices and formulations used in connection with such are described.

Claims (36)

1. A method for treating a patient, comprising:

aerosolizing a formulation comprising free ciprofloxacin and ciprofloxacin encapsulated in unilamellar liposomes comprised of cholesterol and hydrogenated soy phosphatidyl-choline (HSPC), wherein aerosolized particles of the formulation have a particle diameter in a range of 0.5 μm to 12 μm; and

inhaling the aerosolized particles into the patient's lungs wherein the free ciprofloxacin comprises between 10% and 70% of a total of both free ciprofloxacin and encapsulated ciprofloxacin in the formulation;

wherein the free and encapsulated formulations are mixed, and the mixing and aerosolizing do not degrade liposome integrity.

2. The method of claim 1 , further comprising:

diagnosing the patient as having a respiratory infection selected from the group consisting of severe acute respiratory syndrome (SARS), influenza, mumps, croup, sinusitis, bronchitis, angina, laryngitis, tracheitis, rhinitis, rhinopharyugitis, bronchiolitis, bronchitis, bronchopneumonia, pneumonia, staphylococcal pneumonia, whooping cough, the common cold, type A influenza, type B influenza, type C influenza, tuberculosis (TB), legionellosis, echinococcosis, pulmonary pleuropneumonia, 3 tonsillitis, asthma, allergies, and combinations thereof.

3. The method of claim 1 , further comprising:

diagnosing the patient as being infected with a pathogen selected from the group consisting of respiratory syncytial virus (RSV), rhinovirus, para-influenza virus, coronavirus, adenovirus, coxsackievirus, myxovirus, Pneumococcus, Staphylococcus, Streptococcus, Klebsiella, Haemophi-lus, aspergillus, blastomyces dermatitis, candidiasis, coccidioidomycosis, cryptococcosis, histoplasmosis, contact allergens, and combinations thereof.

4. The method of claim 1 , further comprising:

diagnosing the patient with a disease state selected from the group consisting of cystic fibrosis, tuberculosis, bronchiectasis, pneumonia; ventilator associated pneumonia, community acquired pneumonia, bronchial pneumonia, lobar pneumonia; infections by Streptococcus pneumoniae, Chlamydia, Mycoplasma pneumonia , staphylococci, prophylactive treatment of conditions in which infection might arise, chosen from intubated and ventilated patients, infections in lung transplant patient, bronchitis, pertussis (whooping cough), inner ear infections, streptococal throat infections, inhalation anthrax, tularemia, or sinusitis.

5. The method of claim 1 , further comprising:

diagnosing the patient as having a disease state selected from the group consisting of a pseudomonas (e.g., P. aeruginosa , P. paucimobilis, P. putida , P. fluorescens, and P. acidovorans ), staphylococcal, Methicillin-resistant Staphylococcus aureus (MRSA), streptococcal (including by Streptococcus pneumoniae ), Escherichia coli, Klebsiella, Enterobacter, Serratia, Haemophilus, Yersinia pestis, Burkholderia pseudomallei, B. cepacia , B. gladioli, B. multivorans, B. vietnamiensis, Mycobacterium tuberculosis , M. avium complex (MAC)(M. avium and M. intracellulare), M. kansasii, M. xenopi, M. marinum, M. ulcerans, or M. fortuitum complex (M. fortuitum and M. chelonei) infection.

6. The method of claim 1 , wherein the liposomes have an average diameter within a range of from about 75 to 120 nm.

7. The method of claim 1 , wherein the formulation is comprised of the pharmaceutically active ciprofloxacin in an amount of between 0.01% to 40% by weight.

8. The method of claim 1 , wherein the encapsulated ciprofloxacin is released at a rate of between about 0.1% to 100% per hour.

9. The method of claim 1 , wherein the encapsulated ciprofloxacin is released at a rate of between about 0.5% to 20% per hour with substantially complete release of ciprofloxacin occurring after about 1 to 12 hours.

10. The method of claim 1 , wherein the encapsulated ciprofloxacin is released at a rate of between about 2% to 10% per hour with substantially complete release of ciprofloxacin occurring after about 1 to 24 hours.

The method of claim 1 , wherein the formulation further comprises a therapeutic agent selected from the group consisting of DNase, a mucoregulatory agent, chemicals that up-regulate the chloride ion channel or increase flow of ions across the epithelial surface of cells, including lantibiotics such as duramycin, agents to promote hydration or mucociliary clearance including epithelial sodium channel (ENaC) inhibitors or P2Y2 agonists such as denufosol, a bronchodilator, a steroid, an elastase inhibitor such as Alpha-1 antitrypsin (AAT), N-acetylcysteine, agents that enhance the activity of an anti-infective against biofilm bacteria such as sodium salicylate, gamma interferon, alpha interferon, a fluoroquinolone, an antibiotic, or a fluoroquinolone selected from the group consisting of amifloxacin, cinoxacin, ciprofloxacin, danofloxacin, difloxacin, enoxacin, enrofloxacin, fleroxacin, irloxacin, lomefloxacin, miloxacin, norfloxacin, ofloxacin, pefloxacin, rosoxacin, rufloxacin, sarafloxacin, sparfloxacin, temafloxacin and tosufloxacin or an antibiotic selected from the group of tobramycin, colistin, azithromycin, amikacin, cefaclor (Ceclor), aztreonam, amoxicillin, ceftazidime, cephalexin (Keflex), gentamicin, vancomycin, imipenem, doripenem, piperacillin, minocycline, or erythromycin.

12. The method of claim 1 , wherein the aerosolizing creates particles having a diameter in a range from about 2 microns to about 4 microns and the free drug comprises 5% or more of a total of both free drug and liposome-encapsulated drug in the formulation.

13. The method of claim 1 , wherein the aerosolized formulation has a particles size in the range of 1 μm to 6 μm.

14. The method of claim 1 , wherein the aerosolized formulation has a particles size in the range of 2 μm to 4 μm.

15. The method as claimed in 1 , wherein the pH of the formulation is from 4.0 to 5.0.

16. The method as claimed in 1 , wherein the aerosolizing is by nebulization and liposomes in the formulation maintain integrity in response to nebulization.

17. The method of claim 16 , wherein upon deposition in lung the encapsulated ciprofloxacin is released from the liposomes at a rate of about 2% to about 10% per hour with substantially complete release of ciprofloxacin occurring between about 2 hours and 150 hours.

18. A method of down-regulating cytokine markers, comprising:

aerosolizing a formulation comprising free ciprofloxacin and ciprofloxacin encapsulated in unilamellar liposomes comprised of cholesterol and hydrogenated soy phosphatidyl-choline (HSPC), wherein aerosolized particles of the formulation have a particle diameter in a range of 0.5 μm to 12 μm; and

inhaling the aerosolized particles into the patient's lungs wherein the ciprofloxacin comprises between 10% and 70% of a total of both free ciprofloxacin and encapsulated ciprofloxacin in the formulation;

whereby the formulation acts to down-regulate cytokine markers of the patient.

19. A method of reducing influx of macrophages into lungs of a patient, comprising:

aerosolizing a formulation comprising free ciprofloxacin and ciprofloxacin encapsulated in unilamellar liposomes comprised of cholesterol and hydrogenated soy phosphatidyl-choline (HSPC), wherein aerosolized particles of the formulation have a particle diameter in a range of 0.5 μm to 12 μm; and

inhaling the aerosolized particles into the patient's lungs wherein the ciprofloxacin comprises between 10% and 70% of a total of both free ciprofloxacin and encapsulated ciprofloxacin in the formulation;

whereby the formulation acts to reduce influx of macrophages into lungs of the patient.

20. A method for attenuating release of cytokines to reduce attraction of macrophage to the lung, comprising:

aerosolizing a formulation comprising free ciprofloxacin and ciprofloxacin encapsulated in unilamellar liposomes comprised of cholesterol and hydrogenated soy phosphatidyl-choline (HSPC), wherein aerosolized particles of the formulation have a particle diameter in a range of 0.5 μm to 12 μm; and

inhaling the aerosolized particles into the patient's lungs wherein the free ciprofloxacin comprises between 10% and 70% of a total of both free ciprofloxacin and encapsulated ciprofloxacin in the formulation;

whereby the formulation acts to attenuate cytokine release and reduce attraction of macrophage to lungs of the patient.
