Abstract

A prosthetic replacement for a posterior element of a vertebra comprising portions that replace the natural lamina and the four natural facets. The prosthetic replacement may also include portions that replace one or more of the natural spinous process and the two natural transverse processes. If desired, the prosthesis replacement may also replace the natural pedicles. A method for replacing a posterior element of a vertebra is also provided.

Claims (74)

1. A prosthesis for replacing a posterior element of a first natural vertebra of a spine, the first natural vertebra comprising a natural vertebral body, right and left natural pedicles extending from the natural vertebral body, a natural posterior arch extending between the natural pedicles, right and left natural superior articular processes extending from the natural posterior arch and the natural pedicles, and right and left natural inferior articular processes extending from the natural posterior arch, the prosthesis comprising:

a right prosthetic mount that attaches the prosthesis to a right natural pedicle location of the first vertebra;

a left prosthetic mount that attaches the prosthesis to a left natural pedicle location of the first vertebra;

an interconnecting structure formed monolithically with the prosthetic mounts and extending therebetween; and

a first articular structure coupled to the interconnecting structure to constrain motion of the first vertebra relative to an adjacent second vertebra of the spine by articulating with a natural or prosthetic articular structure on the second vertebra;

wherein, after the prosthesis is attached to the first vertebra, the prosthesis contacts the first vertebra exclusively at the natural pedicle locations.

2. A prosthesis according to claim 1 , wherein at least one of the prosthetic mounts replaces a resected portion of at least one of the natural pedicles.

3. A prosthesis according to claim 1 , wherein at least one of the prosthetic mounts replaces substantially all of at least one resected natural pedicle.

4. A prosthesis according to claim 1 , wherein the prosthesis is attached to the first vertebra by pedicle screws that couple to the prosthetic mounts.

5. A prosthesis according to claim 1 , wherein the first articular structure is formed as a separate piece from the interconnecting structure and is coupled thereto.

6. A prosthesis according to claim 1 , further comprising a second articular structure coupled to the interconnecting structure and formed as a mirror image of the first articular structure about a plane of bilateral symmetry of the prosthesis.

7. A prosthesis according to claim 1 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent inferior vertebra.

8. A prosthesis according to claim 1 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent superior vertebra.

A prosthesis for replacing a posterior element of a first natural vertebra of a spine, the first natural vertebra comprising a natural vertebral body, right and left natural pedicles extending from the natural vertebral body, a natural posterior arch extending between the natural pedicles, right and left natural superior articular processes extending from the natural posterior arch and the natural pedicles, right and left natural inferior articular processes extending from the natural posterior arch, and a natural spinous process extending from the midsection of the natural posterior arch, the prosthesis comprising:

right and left prosthetic mounts that attach the prosthesis to the first vertebra at the right and left natural pedicle locations;

an interconnecting structure monolithically formed with the prosthetic mounts and extending therebetween, wherein the interconnecting structure does not encircle the natural spinous process location of the first vertebra; and

a first articular structure extending from the interconnecting structure to constrain motion of the first vertebra relative to an adjacent second vertebra of the spine by articulating with a natural or prosthetic articular structure on the second vertebra;

wherein, after the prosthesis is attached to the first vertebra, the prosthesis does not fit against and does not bear with contact against any portion of the natural posterior arch of the first vertebra.

10. A prosthesis according to claim 9 , wherein at least one of the prosthetic mounts replaces a resected portion of at least one of the natural pedicles.

11. A prosthesis according to claim 9 , wherein at least one of the prosthetic mounts replaces substantially all of at least one resected natural pedicle.

12. A prosthesis according to claim 9 , wherein the prosthesis is attached to the first vertebra by pedicle screws that couple to the prosthetic mounts.

13. A prosthesis according to claim 9 , wherein the first articular structure is formed as a separate piece from the interconnecting structure and is coupled thereto.

14. A prosthesis according to claim 9 , further comprising a second articular structure coupled to the interconnecting structure and formed as a mirror image of the first articular structure about a plane of bilateral symmetry of the prosthesis.

15. A prosthesis according to claim 9 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent inferior vertebra.

16. A prosthesis according to claim 9 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent superior vertebra.

A method for replacing a posterior element of a first natural vertebra of a spine, the first natural vertebra comprising a natural vertebral body, right and left natural pedicles extending from the natural vertebral body, a natural posterior arch extending between the natural pedicles, right and left natural superior articular processes extending from the natural posterior arch and the natural pedicles, and right and left natural inferior articular processes extending from the natural posterior arch, the method comprising the steps of:

resecting a portion of the first vertebra;

positioning a prosthesis against the first vertebra, the prosthesis comprising:

a right prosthetic mount;

a left prosthetic mount;

an interconnecting structure formed monolithically with the prosthetic mounts and extending therebetween; and

a first articular structure coupled to the interconnecting structure to constrain motion of the first vertebra relative to an adjacent second vertebra of the spine by articulating with a natural or prosthetic articular structure on the second vertebra;

wherein positioning the prosthesis comprises positioning the right prosthetic mount proximate the right natural pedicle location of the first vertebra and positioning the left prosthetic mount proximate the left natural pedicle location of the first vertebra;

attaching the right prosthetic mount to the first vertebra at the right natural pedicle location; and

attaching the left prosthetic mount to the first vertebra at the left natural pedicle location;

wherein, after the prosthesis is attached to the first vertebra, the prosthesis contacts the first vertebra exclusively at the natural pedicle locations.

18. A method according to claim 17 , wherein at least one of the natural pedicles is resected so as to remove a portion of the pedicle, wherein the resected pedicle portion is replaced by the prosthetic mount.

19. A method according to claim 17 , wherein at least one of the natural pedicles is resected so as to remove substantially all of the pedicle, wherein the resected pedicle portion is replaced by the prosthetic mount.

20. A method according to claim 17 , wherein the prosthesis is attached to the first vertebra by pedicle screws that couple to the prosthetic mounts.

21. A method according to claim 17 , wherein the first articular structure is formed as a separate piece from the interconnecting structure and is coupled thereto before the prosthesis is attached to the first vertebra.

22. A method according to claim 17 , wherein the prosthesis further comprises a second articular structure coupled to the interconnecting structure before the prosthesis is attached to the first vertebra and wherein the second articular structure is formed as a mirror image of the first articular structure about a plane of bilateral symmetry of the prosthesis.

23. A method according to claim 17 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent inferior vertebra.

24. A method according to claim 17 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent superior vertebra.

25. A prosthesis for replacing a posterior element of a first natural vertebra of a spine, the first natural vertebra comprising a natural vertebral body, right and left natural pedicles extending from the natural vertebral body, a natural posterior arch extending between the natural pedicles, right and left natural superior articular processes extending from the natural posterior arch and the natural pedicles, and right and left natural inferior articular processes extending from the natural posterior arch, the prosthesis comprising:

a right prosthetic mount that attaches the prosthesis to a right natural pedicle location of the first vertebra;

a left prosthetic mount that attaches the prosthesis to a left natural pedicle location of the first vertebra;

an interconnecting structure coupled to the prosthetic mounts and extending at least therebetween, wherein the interconnecting structure has a first end and a second end opposite the first end; and

a first articular structure coupled to the interconnecting structure proximate the first end of the interconnecting structure to constrain motion of the first vertebra relative to an adjacent second vertebra of the spine by articulating with a natural or prosthetic articular structure on the second vertebra;

such that, after the prosthesis is attached to the first vertebra, the prosthesis contacts the first vertebra exclusively at the natural pedicle locations.

26. A prosthesis according to claim 25 , wherein at least one of the prosthetic mounts replaces a resected portion of at least one of the natural pedicles.

27. A prosthesis according to claim 25 , wherein at least one of the prosthetic mounts replaces substantially all of at least one resected natural pedicle.

28. A prosthesis according to claim 25 , wherein the prosthesis is attached to the first vertebra by pedicle screws that couple to the prosthetic mounts.

29. A prosthesis according to claim 25 , wherein the first articular structure is formed as a separate piece from the interconnecting structure and is coupled thereto.

30. A prosthesis according to claim 25 , further comprising a second articular structure coupled to the interconnecting structure proximate the second end of the interconnecting structure.

31. A prosthesis according to claim 25 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent inferior vertebra.

32. A prosthesis according to claim 25 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent superior vertebra.

33. A method for replacing a posterior element of a first natural vertebra of a spine, the first natural vertebra comprising a natural vertebral body, right and left natural pedicles extending from the natural vertebral body, a natural posterior arch extending between the natural pedicles, right and left natural superior articular processes extending from the natural posterior arch and the natural pedicles, and right and left natural inferior articular processes extending from the natural posterior arch, the method comprising the steps of:

resecting a portion of the first vertebra;

positioning a prosthesis against the first vertebra, the prosthesis comprising:

a right prosthetic mount;

a left prosthetic mount;

an interconnecting structure coupled to the prosthetic mounts and extending at least therebetween, wherein the interconnecting structure has a first end and a second end opposite the first end; and

a first articular structure coupled to the interconnecting structure proximate the first end of the interconnecting structure to constrain motion of the first vertebra relative to an adjacent second vertebra of the spine by articulating with a natural or prosthetic articular structure on the second vertebra;

wherein positioning the prosthesis comprises positioning the right prosthetic mount proximate the right natural pedicle location of the first vertebra and positioning the left prosthetic mount proximate the left natural pedicle location of the first vertebra;

attaching the right prosthetic mount to the first vertebra at the right natural pedicle location; and

attaching the left prosthetic mount to the first vertebra at the left natural pedicle location;

wherein, after the prosthesis is attached to the first vertebra, the prosthesis contacts the first vertebra exclusively at the natural pedicle locations.

34. A method according to claim 33 , wherein at least one of the natural pedicles is resected so as to remove a portion of the pedicle, wherein the resected pedicle portion is replaced by the prosthetic mount.

35. A method according to claim 33 , wherein at least one of the natural pedicles is resected so as to remove substantially all of the pedicle, wherein the resected pedicle portion is replaced by the prosthetic mount.

36. A method according to claim 33 , wherein the prosthesis is attached to the first vertebra by pedicle screws that couple to the prosthetic mounts.

37. A method according to claim 33 , wherein the first articular structure is formed as a separate piece from the interconnecting structure and is coupled thereto before the prosthesis is attached to the first vertebra.

38. A method according to claim 33 , wherein the prosthesis further comprises a second articular structure coupled to the interconnecting structure proximate the second end of the interconnecting structure.

39. A method according to claim 33 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent inferior vertebra.

40. A method according to claim 33 , wherein, after the prosthesis is attached to the first vertebra, the first articular structure extends from the interconnecting structure toward a second adjacent superior vertebra.
