Abstract

The invention relates to spinal fusion implants or grafts and to apparatus for the installation thereof. More specifically, the invention is a serrated cutting or abrading tool designed to be pushed into an intervertebral space and thereby remove and partially penetrate the cortical bone layer that defines the endplates of the respective mutually adjacent vertebral bodies.

Claims (34)

1. A cortical endplate bone cutting instrument for insertion into a vacated intervertebral space between adjacent vertebral bodies, the instrument comprising:

a body having a proximal end and a distal end sized to fit between the adjacent vertebral bodies;

a plurality of top sharp edged serrations extending from a top surface of the body configured to cut and/or abrade cortical endplate bone of a first adjacent vertebral body;

a plurality of bottom sharp edged serrations extending from a bottom surface of the body configured to cut and/or abrade cortical endplate bone of a second adjacent vertebral body; and

a blunt front end region at the distal end of the body extending distally from the top surface and the bottom surface forward of the top and bottom serrations;

wherein the top surface and the bottom surface are parallel.

2. The instrument of claim 1 , wherein the blunt front end region is shaped to guide the instrument into the vacated intervertebral space.

3. The instrument of claim 1 , wherein the blunt front end region is shaped to distract or separate the adjacent vertebral bodies during insertion into the vacated intervertebral space.

4. The instrument of claim 1 , wherein the blunt front end region includes parabolic cross-section.

5. The instrument of claim 1 , wherein the blunt front end region includes a blunt tip that is circular, parabolic, or ogive in shape, joined with flat inclined faces which are arranged symmetrically with one another, joined with parallel flat surfaces.

6. The instrument of claim 1 , wherein the blunt front end region has a length at least twice a thickness between distal ends of top and bottom surfaces.

7. The instrument of claim 1 , wherein the body has a parallelepiped shape.

8. The instrument of claim 1 , in which the top and bottom serrations comprise the end edges of protrusions in the top surface and the bottom surface.

9. The instrument of claim 1 , further comprising a handle having a first end coupled to the proximal end of the body.

10. The instrument of claim 9 , wherein the handle has a second end comprising an anvil suitable for use with a driver.

11. The instrument of claim 9 , wherein the handle has a second end coupled to a mechanical energy transmission means.

12. The instrument of claim 11 , wherein the mechanical energy transmission means comprises a pneumatic, hydraulic or other suitable driver.

13. The instrument of claim 11 , wherein the mechanical energy transmission moves the instrument in a linear motion.

14. The instrument of claim 9 , further comprising a depth stop positioned between the body and the handle.

15. The instrument of claim 14 , wherein the depth stop is part of the handle.

16. A method of preparing an implant site using a cortical endplate bone cutting instrument comprising:

removing a portion of an intervertebral disk creating an intervertebral space between first and second adjacent vertebral bodies;

inserting a cortical endplate bone cutting instrument into the vacated intervertebral space between the adjacent vertebral bodies, the instrument comprising:

a body having a proximal end and a distal end sized to fit between the first and second adjacent vertebral bodies;

a plurality of top sharp edged serrations extending from a top surface of the body configured to cut and/or abrade cortical endplate bone of the first adjacent vertebral body;

a plurality of bottom sharp edged serrations extending from a bottom surface of the body configured to cut and/or abrade cortical endplate bone of a second adjacent vertebral body;

a blunt front end region at the distal end of the body extends distally from the top surface and the bottom surface forward of the top and bottom serrations;

wherein the top surface and the bottom surface are parallel;

moving the instrument in a linear motion between the adjacent vertebral bodies; and

cutting and/or abrading a sufficient amount of cortical endplate bone of first and second adjacent vertebral bodies with the top and bottom serrations to allow blood to flow into the intervertebral space.

17. The method of claim 16 , further comprising distracting or separating the first and second adjacent vertebral bodies with the blunt front end region during the step of inserting the cortical endplate bone cutting instrument.

18. The method of claim 17 , further comprising guiding the top and bottom serrations into position for an optimal cutting and/or abrading interaction with the cortical endplate bone of the first and second adjacent vertebral bodies.

19. The method of claim 16 , wherein after creating an intervertebral space, the method further comprising selecting an instrument from a set of instruments of differing thicknesses the one having the optimal thickness for the intervertebral space.

20. The method of claim 16 , wherein the instrument further comprises a mechanical energy transmission means and moving the instrument in a linear motion includes moving the instrument linearly with the mechanical energy transmission means.
