Abstract

Spinal tissue distraction devices that include at least one guide member for guiding a distraction device to a location between layers of spinal tissue. The guide member has a proximal end portion and a distal end portion. The distal end portion is adapted for insertion between the tissue layers and defines a shape of a desired support structure. The distraction device is slidably mounted on the guide member for advancing the distraction device distally along the guide member for insertion of the distraction device between the tissue layers. The distraction device substantially conforms to the shape defined by the distal end portion of the guide member to form a support structure that separates, supports or both separates and supports the tissue layers.

Claims (33)

1. An intravertebral implant system for treating a vertebral body having opposed endplates, the system comprising:

an elongated cannula, an elongated guide member and an elongated implant;

the cannula including a distal end portion for insertion into a vertebral body, an aperture in the distal end portion and a lumen extending proximally from the aperture;

the elongated guide member including a distal end portion and being advanceable through the lumen of the cannula between a first position wherein the distal end portion of the guide member is disposed within the lumen and a second position wherein the distal end portion of the guide member extends through the aperture of the cannula into the vertebral body and a more proximal portion of the guide member resides within the cannula;

the distal end portion of the guide member, when in the first position being constrained by the cannula to an insertion configuration allowing advancement through the lumen and the aperture of the cannula and, when in the second position, being unconstrained by the cannula and forming an inserted configuration that includes a plurality of generally horizontally-disposed windings within the vertebral body; and

the elongated implant being independently movable longitudinally relative to the guide member for advancement along the guide member through the lumen of the cannula and the aperture when the guide member is in the second position, the implant also being advanceable along the inserted configuration of the distal end portion of the guide member to define an intravertebral structure including a plurality of generally horizontal-disposed windings within the vertebral body.

2. The implant system of claim 1 in which at least some of the distal end portion of the guide member is generally linear in the first position.

3. The implant system of claim 1 in which the distal end portion of the guide member is configured to self-form into the inserted configuration when the distal end portion of the guide member is unconstrained by the cannula.

4. The implant system of claim 1 in which at least the distal end portion of the guide member is comprised of a shape memory material.

5. The implant system of claim 1 in which the windings of the structure extend in a generally circular direction.

6. The implant system of claim 1 in which at least one of the windings of the structure has at least one adjacent winding thereabove or therebelow and the at least one winding being in contact with the adjacent winding.

7. The implant system of claim 1 in which the windings of the support structure define a resident volume at least partially enclosed by the support structure.

8. The implant system of claim 1 in which the elongated implant has a proximal end, a distal end and a longitudinal passageway therethrough, wherein the passageway is configured to receive the guide member for advancement of the elongated implant along the guide member.

9. The implant system of claim 8 in which the passageway is configured to allow the elongated implant to be slidably advanced with respect to the guide member.

10. The implant system of claim 1 in which the guide member is removable from the intravertebral structure.

11. An intravertebral implant system for treating a vertebral body having opposed endplates, the system comprising:

an elongated cannula, an elongated guide member and an elongated implant;

the cannula including a distal end portion for insertion into a vertebral body, an aperture in the distal end portion and a lumen extending proximally from the aperture;

the elongated guide member including a distal end portion and being advanceable through the lumen of the cannula between a first position wherein the distal end portion of the guide member is disposed within the lumen and a second position wherein the distal end portion of the guide member extends through the aperture of the cannula into the vertebral body;

the distal end portion of the guide member, when in the first position being constrained by the cannula to a configuration allowing advancement through the lumen and, when in the second position, being unconstrained by the cannula and forming an arcuate configuration; and

the elongated implant being independently movable longitudinally relative to the guide member for advancement along the guide member through the lumen of the cannula and the aperture when the guide member is in the second position, the implant also being advanceable along and conforming to the arcuate configuration of the distal end portion of the guide member to define a one piece arcuate intravertebral structure extending greater than 360 degrees and that extends between and separates and/or supports the endplates of the vertebral body.

12. The implant system of claim 11 in which at least some of the distal end portion of the guide member is generally linear in the first position.

13. The implant system of claim 11 in which the distal end portion of the guide member self-forms into the arcuate configuration when the distal end portion of the guide member is unconstrained by the cannula.

14. The implant system of claim 11 in which the elongated implant has a proximal end, a distal end and a passageway therethrough, wherein the passageway is configured to receive the guide member for advancement of the elongated implant along the guide member.

15. The implant system of claim 14 in which the elongated implant is slidably advanceable with respect to the guide member.

16. The implant system of claim 11 in which the guide member is removable from intravertebral structure.

17. An intravertebral implant system for treating a vertebral body having opposed endplates, the system comprising:

an elongated guide wire and an elongated implant;

the elongated guide wire including a distal end portion sufficiently flexible for changing between a substantially linear configuration for insertion into a vertebral body and an arcuate configuration within the vertebral body in which the distal end portion of the guide wire extends at least 360 degrees about a vertical axis; and

the elongated implant being independently movable longitudinally relative to the guide wire for advancement along the guide wire while the guide wire is stationary, the implant being constrained by the guide wire to conform to the configuration of the distal end portion of the guide wire, whereby the implant has a substantially linear configuration for insertion into a vertebral body and an arcuate configuration within the vertebral body in which the implant extends greater than 360 degrees about the vertical axis to provide a support structure that separates and/or supports the endplates of the vertebral body.

18. The implant system of claim 17 in which the distal end portion of the guide wire is configured to change from the linear configuration to the arcuate configuration by self forming into the arcuate configuration.

19. The implant system of claim 17 in which the elongated implant has a proximal end, a distal end and a passageway therethrough, wherein the passageway is configured to receive the guide wire for advancement of the elongated implant along the guide wire.
