Abstract

Disclosed herein are methods and compositions for producing articular repair materials and for repairing an articular surface. In particular, methods for providing articular repair systems. Also provided are articular surface repair systems designed to replace a selected area cartilage, for example, and surgical tools for repairing articular surfaces.

Claims (75)

1. A surgical instrument for the repair of a diseased articular joint surface of a joint, comprising:

an inner surface having a curvature or shape based on information from image data of the diseased articular joint surface; and

a slit defining a cutting path through at least a portion of the joint when the inner surface is applied to the diseased articular joint surface.

2. The surgical instrument of claim 1 , wherein the inner surface matches a shape of the diseased articular joint surface.

3. The surgical instrument of claim 1 , wherein the inner surface matches a size of the diseased articular joint surface.

4. The surgical instrument of claim 1 , wherein the inner surface matches a size of diseased cartilage of the diseased articular joint surface.

5. The surgical instrument of claim 1 , wherein the inner surface matches a curvature of diseased cartilage of the diseased articular joint surface.

6. The surgical instrument of claim 1 , wherein the inner surface matches a shape of diseased cartilage of the diseased articular joint surface.

7. The surgical instrument of claim 1 , wherein the inner surface matches a curvature of subchondral bone of the diseased articular joint surface.

8. The surgical instrument of claim 1 , wherein the inner surface matches a shape of subchondral bone of the diseased articular joint surface.

9. The surgical instrument of claim 1 , wherein the inner surface matches a curvature of subchondral bone underlying diseased cartilage of the diseased articular joint surface.

10. The surgical instrument of claim 1 , wherein the inner surface includes information of cartilage of the diseased articular joint surface.

11. The surgical instrument of claim 1 , wherein the inner surface includes information of subchondral bone of the diseased articular joint surface.

12. A surgical instrument for the repair of a diseased or damaged cartilage surface of a joint, comprising:

an inner surface having a curvature or shape based on information from image data of the diseased or damaged cartilage surface; and

a slit defining a cutting path through at least a portion of the joint when the inner surface is applied to the diseased or damaged cartilage surface.

13. The surgical instrument of claim 12 , wherein the inner surface matches a shape of the diseased or damaged cartilage surface.

14. The surgical instrument of claim 12 , wherein the inner surface matches a size of the diseased or damaged cartilage surface.

15. The surgical instrument of claim 12 , wherein the inner surface matches a curvature of the diseased or damaged cartilage surface.

16. The surgical instrument of claim 12 , wherein the inner surface matches a shape of the diseased or damaged cartilage surface.

17. The surgical instrument of claim 12 , wherein the inner surface matches a shape of subchondral bone underlying the diseased or damaged cartilage surface.

18. The surgical instrument of claim 12 , wherein the inner surface matches a curvature of subchondral bone underlying diseased cartilage of the diseased articular joint surface.

19. The surgical instrument of claim 12 , wherein the inner surface includes information of cartilage of the diseased or damaged cartilage surface.

20. The surgical instrument of claim 12 , wherein the inner surface includes information of subchondral bone underlying the diseased or damaged cartilage surface.

21. A surgical instrument for the repair of a diseased articular joint surface of a joint, comprising:

an inner surface having a curvature or shape that matches a curvature or shape of the diseased articular joint surface; and

a slit defining a cutting path through at least a portion of the joint when the inner surface is applied to the diseased articular joint surface.

22. The surgical instrument of claim 21 , wherein the inner surface matches a shape of the diseased articular joint surface.

23. The surgical instrument of claim 21 , wherein the inner surface matches a size of the diseased articular joint surface.

24. The surgical instrument of claim 21 , wherein the inner surface matches a size of diseased cartilage of the diseased articular joint surface.

25. The surgical instrument of claim 21 , wherein the inner surface matches a curvature of diseased cartilage of the diseased articular joint surface.

26. The surgical instrument of claim 21 , wherein the inner surface matches a shape of diseased cartilage of the diseased articular joint surface.

27. The surgical instrument of claim 21 , wherein the inner surface matches a curvature of subchondral bone of the diseased articular joint surface.

28. The surgical instrument of claim 21 , wherein the inner surface matches a shape of subchondral bone of the diseased articular joint surface.

29. The surgical instrument of claim 21 , wherein the inner surface matches a curvature of subchondral bone underlying diseased cartilage of the diseased articular joint surface.

30. The surgical instrument of claim 21 , wherein the inner surface includes information of cartilage of the diseased articular joint surface.

31. The surgical instrument of claim 21 , wherein the inner surface includes information of subchondral bone of the diseased articular joint surface.

32. A surgical instrument for the repair of a diseased articular joint surface of a joint, comprising:

an inner surface having a curvature that matches a curvature of cartilage of the diseased articular joint surface; and

a slit defining a cutting path through at least a portion of the joint when the inner surface is applied to the diseased articular joint surface.

33. The surgical instrument of claim 32 , wherein the inner surface matches the shape of the diseased articular joint surface.

34. The surgical instrument of claim 32 , wherein the inner surface matches a size of the diseased articular joint surface.

35. The surgical instrument of claim 32 , wherein the inner surface matches a size of diseased cartilage of the diseased articular joint surface.

36. The surgical instrument of claim 32 , wherein the inner surface matches a shape of diseased cartilage of the diseased articular joint surface.

37. The surgical instrument of claim 32 , wherein the inner surface matches a curvature of subchondral bone of the diseased articular joint surface.

38. The surgical instrument of claim 32 , wherein the inner surface matches a shape of subchondral bone of the diseased articular joint surface.

39. The surgical instrument of claim 32 , wherein the inner surface matches a curvature of subchondral bone underlying diseased cartilage of the diseased articular joint surface.

40. The surgical instrument of claim 32 , wherein the inner surface includes information of the cartilage of the diseased articular joint surface.

41. The surgical instrument of claim 32 , wherein the inner surface includes information of subchondral bone of the diseased articular joint surface.

42. A surgical instrument for the repair of a diseased articular joint surface of a joint, comprising:

an inner surface having a curvature that matches a curvature of subchondral bone of the diseased articular joint surface; and

a slit defining a cutting path through at least a portion of the joint when the inner surface is applied to the diseased articular joint surface.

43. The surgical instrument of claim 42 , wherein the inner surface matches a shape of the diseased articular joint surface.

44. The surgical instrument of claim 42 , wherein the inner surface matches a size of the diseased articular joint surface.

45. The surgical instrument of claim 42 , wherein the inner surface matches a size of diseased cartilage of the diseased articular joint surface.

46. The surgical instrument of claim 42 , wherein the inner surface matches a shape of diseased cartilage of the diseased articular joint surface.

47. The surgical instrument of claim 42 , wherein the inner surface matches a curvature of diseased cartilage of the diseased articular joint surface.

48. The surgical instrument of claim 42 , wherein the subchondral bone underlies diseased cartilage of the diseased articular joint surface.

49. The surgical instrument of claim 42 , wherein the inner surface includes information of cartilage of the diseased articular joint surface.

50. A surgical device for preparing a recipient site in a diseased or damaged joint of a patient,

wherein at least a portion of the surgical device has a shape that matches an articular shape of the diseased or damaged joint of the patient, and

wherein the surgical device includes a slit configured to guide a surgical blade.

51. The surgical device of claim 50 , wherein the articular shape of the diseased or damaged joint of the patient is derived from an electronic image.

52. The surgical device of claim 51 , wherein the electronic image includes a digital image obtained with one or more x-ray based techniques selected from the group consisting of computed or digital radiography, ultrasound, computed tomography (CT), nuclear scintigraphy, SPECT, positron emission tomography and MRI.

53. The surgical device of claim 51 , wherein the electronic image includes information associated with the diseased or damaged joint of the patient.

54. The surgical device of claim 53 , wherein the information includes information about curvature of subchondral bone, curvature of cartilage, cartilage thickness, cartilage volume, dimensions and volume of soft and hard tissues associated with the diseased or damaged joint of the patient.

55. The surgical device of claim 54 , wherein the information includes thickness of the normal or diseased cartilage surrounding a cartilage defect of the diseased or damaged joint of the patient.

56. The surgical device of claim 50 , wherein a shape of the surgical device matches a shape slightly larger than the area of diseased or damaged joint of the patient.

57. The surgical device of claim 50 , wherein a shape of the area of diseased or damaged joint includes a shape of tissue surrounding or adjacent diseased cartilage.

58. The surgical device of claim 50 , wherein the device is disposable.

59. The surgical device of claim 50 , wherein the device is integrally formed.

60. The surgical device of claim 50 , wherein the device is made of a plastic.

61. A surgical device for preparing a recipient site in a diseased or damaged joint of a patient,

wherein at least a portion of the surgical device has a shape that matches an articular shape of the diseased or damaged joint of the patient, and

wherein the surgical device includes a holding mechanism configured to guide a surgical blade.
