Abstract

A system for performing intrastromal ophthalmic laser surgery requires Laser Induced Optical Breakdown (LIOB) of stromal tissue without compromising Bowman's capsule (membrane). In detail, the system is computer-controlled to create symmetrical cuts in the stroma relative to a defined axis of the eye. Importantly, these cuts are all distanced from the axis. The actual location and number of cuts in the surgery will depend on the degree of visual aberration being corrected. Further, the system may create different types of cuts in the stroma. For example, the symmetrical cuts (by type) may include cylindrical, radial or annular layer cuts. The different type cuts may be intersecting or non-intersecting depending on the visual aberration being treated.

Claims (30)

1. A system for performing intrastromal laser refractive surgery on an eye defining an axis, the system comprising:

a laser unit for generating a pulsed laser beam to provide for the Laser Induced Optical Breakdown (LIOB) of tissue at successive focal spots in the stroma to weaken stromal tissue and redistribute biomechanical forces in the stroma of a cornea having a corneal thickness “T”;

a device for stabilizing the eye, to hold the axis of the eye substantially stationary during the surgery;

a computer program, wherein the computer program defines a plurality of unconnected cuts, wherein each cut is made on portions of a respective cylindrical surface, wherein the respective cylindrical surfaces are concentric and are centered on the axis of the eye, and further wherein each cylindrical surface has an anterior end and a posterior end, with the posterior end of the cut being located in the stroma less than approximately 0.9 T from an anterior surface of the eye, and the anterior end of the cylindrical cut is located in the stroma more than at least eight microns in a posterior direction from Bowman's capsule in the cornea, with the weakened stromal tissue being responsive to an intraocular pressure for surgically reshaping the cornea; and

a computer electronically connected with the laser unit for using the computer program to move the focal spot of the laser beam through the stroma to create the cuts in the stroma.

2. A system as recited in claim 1 wherein the pulses of the laser beam have a less than one picosecond duration with an energy level for each pulse being selected in a range between 0.1 μJ and 1.5 μJ.

3. A system as recited in claim 1 wherein the device for stabilizing the eye is an eyetracker.

4. A system as recited in claim 1 wherein the device for stabilizing the eye is a contact lens.

5. A system as recited in claim 1 wherein the computer program further defines at least one cut is a radial cut.

6. A system as recited in claim 1 wherein the computer program further defines at least one annular layer cut.

7. A system as recited in claim 1 wherein the axis is selected from a group including a visual axis, an optical axis, a line-of-sight axis, a pupillary axis and a compromise axis.

8. A system as recited in claim 1 wherein the computer program controls operation of the laser unit.

9. A system for performing intrastromal laser refractive surgery on an eye defining an axis, the system comprising:

a laser unit for generating a pulsed laser beam to provide for the Laser Induced Optical Breakdown (LIOB) of tissue at successive focal spots in the stroma to weaken stromal tissue and redistribute biomechanical forces in the stroma of a cornea having a corneal thickness “T”;

a device for stabilizing the eye, to hold the axis of the eye substantially stationary during the surgery;

a computer program, wherein the computer program defines a plurality of unconnected radial cuts, wherein each cut is defined by an azimuthal angle, “θ”, wherein each radial cut has an anterior end and a posterior end extending from an inner radius “r i ” to an outer radius “r o ”, with both “r i ” and “r o ” being measured from the axis, with the posterior end of the cut being located in the stroma within less than approximately 0.9 T from an anterior surface of the eye, and the anterior end of the radial cut is located in the stroma more than at least eight microns in a posterior direction from Bowman's capsule, and further wherein the radial cut has a thickness of approximately two microns, with the weakened stromal tissue being responsive to an intraocular pressure for surgically reshaping the cornea; and

a computer electronically connected with the laser unit for using the computer program to move the focal spot of the laser beam through the stroma to create the cuts in the stroma.

10. A system as recited in claim 9 wherein the pulses of the laser beam have a less than one picosecond duration with an energy level for each pulse being selected in a range between 0.1 μJ and 1.5 μJ.

11. A system as recited in claim 9 wherein the computer program further defines at least one cut is a cylindrical cut.

12. A system as recited in claim 9 wherein the computer program further defines at least one annular layer cut.

13. A system as recited in claim 9 wherein the axis is selected from a group including a visual axis, an optical axis, a line-of-sight axis, a pupillary axis and a compromise axis.

14. A system for performing intrastromal laser refractive surgery on an eye defining an axis, the system comprising:

a laser unit for generating a pulsed laser beam to provide for the Laser Induced Optical Breakdown (LIOB) of tissue at successive focal spots in the stroma to weaken stromal tissue and redistribute biomechanical forces in the stroma of a cornea having a corneal thickness “T”;

a device for stabilizing the eye, to hold the axis of the eye substantially stationary during the surgery;

a computer program, wherein the computer program defines a plurality of unconnected annular layer cuts, wherein each cut is substantially perpendicular to the axis of the eye, defined by an azimuthal angle, “θ”, wherein each annular layer has an inner diameter “d i ” and an outer diameter “d o ”, with both “d i ” and “d o ” being measured from the axis, with each cut being located in the stroma within less than approximately 0.9 T from an anterior surface of the eye and more than at least eight microns in a posterior direction from Bowman's capsule, with the weakened stromal tissue being responsive to an intraocular pressure for surgically reshaping the cornea; and

a computer electronically connected with the laser unit for using the computer program to move the focal spot of the laser beam through the stroma to create the cuts in the stroma.

15. A system as recited in claim 14 wherein the pulses of the laser beam have a less than one picosecond duration with an energy level for each pulse being selected in a range between 0.1 μJ and 1.5 μJ.

16. A system as recited in claim 14 wherein the computer program further defines at least one radial cut.

17. A system as recited in claim 14 wherein the computer program further defines at least one cylindrical cut.

18. A system as recited in claim 14 wherein the axis is selected from a group including a visual axis, an optical axis, a line-of-sight axis, a pupillary axis and a compromise axis.
