Abstract

Apparatus and methods are disclosed for vascular stenosis and vulnerable plaque therapies, including enhancing permeability of the vessel wall using ultrasound energy and delivering a therapeutic agent into the arterial wall. Ultrasound energy may also be delivered at frequencies less than 10 MHz and power less than 20 watts using transcutaneous techniques and endovascular methods. Therapeutic agents may be delivered to the treatment area via intravenous and endovascular methods. In some embodiments, ultrasound energy and therapeutic agent application may be combined with angioplasty techniques and with blood flow protection devices.

Claims (31)

1 . A method for treating stenosis or inhibiting restenosis at a treatment location in an artery, comprising:

enhancing permeability of the vessel wall via the delivery of ultrasound energy to the treatment location, and

delivering a therapeutic agent intravenously into the human body.

2 . The method of claim 1 , wherein ultrasound energy is delivered to the treatment area using transcutaneous methods.

3 . The method of claim 1 , wherein ultrasound energy is delivered to the treatment area using endovascular catheter methods.

4 . The method of claim 1 , wherein the therapeutic agent is delivered to the human body according to one method selected from the group consisting of: before ultrasound energy delivery, during ultrasound energy delivery, or after ultrasound energy delivery.

5 . The method of claim 1 , wherein the frequency of the ultrasonic energy is no more than about 100 kHz and the power delivered from the distal end of the ultrasound catheter is no more than about 20 watts.

6 . The method of claim 1 , wherein ultrasonic energy is delivered in one method selected from group consisting of: continuous, pulse, modulated.

7 . The method of claim 1 , wherein the therapeutic agent is an agent selected from the group consisting of: liquid, powder, particle, microbubbles, microspheres, nanospheres, liposomes and combinations thereof.

8 . The method of claim 1 , further comprising positioning a blood flow protection device within the treatment area of the artery.

The method of claim 1 , further comprising performing an angioplasty procedure before delivery of ultrasound energy and the therapeutic agent, wherein the angioplasty procedure is selected from the group consisting of: balloon angioplasty, stent placement, atherectomy, laser angioplasty, and cryoplasty.

The method of claim 1 , further comprising performing an angioplasty procedure during delivery of ultrasound energy and the therapeutic agent, wherein the angioplasty procedure is selected from the group consisting of: balloon angioplasty, stent placement, atherectomy, laser angioplasty, and cryoplasty.

The method of claim 1 , further comprising performing an angioplasty procedure after delivery of ultrasound energy and the therapeutic agent, wherein the angioplasty procedure is selected from the group consisting of: balloon angioplasty, stent placement, atherectomy, laser angioplasty, and cryoplasty.

12 . The method of claim 1 , wherein treating stenosis and inhibiting restenosis includes treating vulnerable plaque in an artery.

13 . A method for treating stenosis or inhibiting restenosis at a treatment location in an artery, comprising of:

enhancing permeability of the vessel wall via the delivery of ultrasound energy to the treatment location, and

delivering a therapeutic agent together with a contrast agent into the artery to the treatment location.

14 . The method of claim 13 wherein ultrasound energy is delivered to the treatment area using transcutaneous methods.

15 . The method of claim 13 , wherein ultrasound energy is delivered to the treatment area using endovascular catheter methods.

16 . The method of claim 13 , wherein the therapeutic agent is delivered to the vessel wall according to one method selected from the group consisting of: before ultrasound energy delivery, during ultrasound energy delivery, after ultrasound energy delivery.

17 . The method of claim 13 , wherein treating stenosis and inhibiting restenosis includes treating vulnerable plaque in an artery.

18 . The method of claim 13 , wherein treating stenosis or inhibit re-stenosis in the artery includes ablating or removing plaque or thrombotic material.

19 . The method of claim 13 , wherein treating stenosis or inhibiting restenosis includes treating a totally occluded artery.

20 . The method of claim 13 , further comprising positioning a blood flow limitation device within the artery to prevent the therapeutic agent from a quick washout by the blood.

21 . The method of claim 13 , wherein treating stenosis and inhibiting restenosis includes treating vulnerable plaque in an artery.

22 . The method of claim 13 , wherein a therapeutic agent and a contrast agent are further diluted with saline.

23 . A method for treating stenosis or inhibiting restenosis at a treatment location in an artery, comprising of:

enhancing permeability of the vessel wall via the delivery of ultrasound energy to the treatment location, and

delivering a therapeutic agent to the treatment location using a balloon device.

24 . The method of claim 23 , wherein the therapeutic agent is delivered using drug eluting balloon devices.

25 . The method of claim 23 , wherein the therapeutic agent is delivered using drug perfusion balloon devices.
