Abstract

Devices, systems, and methods are employed to restore patency to arterial lesions, e.g., by debulking restenosis in a blood vessel or within a stent or coil.

Claims (34)

1. A method for debulking restenosis of a blood vessel, the method comprising:

identifying a region of restenosis of a blood vessel,

deploying into the region a vascular device, wherein the vascular device comprising:

a catheter body having:

a center axis along which axial movement occurs,

a distal end,

a cutter assembly disposed at the distal end and comprising a cylindrical housing having an opening at the distal end defining a forward cutting surface and a cutter having at least one helical cutting surface configured to rotate about the central axis relative to the housing to cut and convey tissue from the restenosis proximally into the housing, and

a first portion and a second portion,

wherein the first portion is attached to the cutter assembly, distal to the second portion, and comprises multiple bodies adapted to interact through interlocking structures and configured to allow a predefined expansion between the interlocking structures, thereby providing a controlled flexibility of the first portion and increased bending relative to the second portion, and

wherein the second portion is configured for increased stiffness relative to the first portion,

a drive mechanism at a proximal end of the catheter body,

a deflecting mechanism, slidable within the catheter body in response to a force applied at the proximal end of the catheter body, for deflecting the distal end of the catheter body relative to the center axis of the catheter body to point the cutter assembly toward a side of the blood vessel by axial advancement of the deflecting mechanism within the catheter body,

engaging and removing tissue from the restenosis using the forward cutting surface,

operating the drive mechanism to rotate the helical cutting surface to debulk the restenosis by cutting and conveying tissue from the restenosis proximally into the housing,

axially advancing the deflecting mechanism in the distal direction at the proximal end of the catheter body to deflect the distal end of the catheter body relative to the center axis of the catheter body to point the cutter assembly toward a side of the blood vessel, and

rotating the deflecting mechanism while the distal end of the catheter body is deflected, whereupon rotating the deflecting mechanism, the cutter assembly sweeps in an arc about the center axis to cut tissue from the restenosis in a region larger than an outside diameter of the cutter assembly.

2. The method of claim 1 wherein at least a portion of the region of restenosis is within a stent structure.

3. The method of claim 2 and further including performing an atherectormy to remove tissue from within the stent structure.

4. The method of claim 2 and further including performing an atherectormy to debulk restenosis of the stent structure.

5. The method of claim 1 further including introducing into the region a bioactive material.

6. The method of claim 1 and further including introducing into the region a bioactive material comprising at least one of a restenosis-inhibiting agent, a thrombus-inhibiting agent, and an anti-inflammatory agent.

7. The method of claim 1 and further including introducing into the region a balloon coated with a bioactive material, and expanding the balloon in contact with the region to deliver the bioactive material.

8. The method of claim 1 and further including visualizing the region before, during and after debulking the restenosis.

9. The method of claim 1 wherein the blood vessel is a peripheral blood vessel.

10. The method of claim 1 wherein the blood vessel is a peripheral blood vessel in a leg.

11. The method of claim 1 wherein the blood vessel is a peripheral blood vessel in a leg below a knee.

12. The method of claim 1 wherein the region of restenosis in entirely within a stent structure.

13. The method of claim 12 and further including introducing into the region a balloon coated with a bioactive material, and expanding the balloon in contact with the region to deliver the bioactive material.

14. The method of claim 12 wherein the blood vessel is a peripheral blood vessel.

15. The method of claim 12 wherein the blood vessel is a peripheral blood vessel in a leg.

16. The method of claim 12 wherein the blood vessel is a peripheral blood vessel in a leg below a knee.

17. The method of claim 1 wherein the rotating of the deflecting mechanism comprises manually rotating the deflecting mechanism to sweep the cutter assembly.

18. The method of claim 1 wherein the rotating of the deflecting mechanism comprises operating a sweep control motor sweep the cutter assembly.

19. The method of claim 1 wherein rotating the deflecting mechanism includes rotating the deflecting mechanism relative to a handle of the catheter body.
