Abstract

The invention provides methods for treating an obstructed biological conduit that include administering to the conduit an agent that can degrade extracellular matrix of obstructing tissue. Particular methods include delivery of an enzyme or a mixture of several enzymes to the area or region of obstruction wherein the enzyme(s) have the capability to degrade extracellular matrix components within the obstruction thereby restoring the normal flow of transported fluid through the conduit. The invention also includes prophylactically dilating a section of conduit to minimize the risk of obstruction formation. The invention further includes methods for prolonging patency of hemodialysis access.

Claims (18)

1. A method for prolonging patency of hemodialysis access in a human patient with renal failure, said method comprising:

a. identifying a segment of an artery or vein suitable for providing hemodialysis access in said patient; and

b. locally administering to said segment in said patient a composition comprising an elastase whereby patency of said hemodialysis access is prolonged.

2. The method of claim 1 , wherein said hemodialysis access is provided by an arteriovenous graft connected to said artery or vein.

3. The method of claim 1 , wherein said elastase is a pancreatic elastase.

4. The method of claim 1 , wherein said composition does not comprise a collagenase.

5. The method of claim 1 , wherein said composition is a fluid comprising said elastase and a pharmaceutically acceptable carrier.

6. The method of claim 1 , wherein said composition is administered into the wall of the artery or vein.

7. The method of claim 1 , wherein said composition is administered to the exterior of said artery or vein.

8. The method of claim 1 , wherein said segment is a segment of an artery.

9. The method of claim 1 , wherein said segment is a segment of a vein.

10. The method of claim 1 , wherein vascular obstruction affecting said hemodialysis access is delayed.

11. The method of claim 1 , wherein stenosis formation affecting said hemodialysis access is delayed.

12. The method of claim 1 , wherein intimal hyperplasia affecting said hemodialysis access is delayed.

13. The method of claim 6 , wherein said administering is performed via a catheter.

14. The method of claim 8 , wherein elastin in the wall of said artery is partially degraded.

15. The method of claim 9 , wherein elastin in the wall of said vein is partially degraded.

16. The method of claim 13 , wherein a portion of said catheter is inserted into the wall of said artery or vein.
