Abstract

A method is disclosed for operating a device that treats sleep disordered breathing (SDB) during successive treatment sessions, where the device provides continuous positive airway pressure during sleep. The method includes the steps of applying a constant treatment pressure during a first session and deriving a sleep disorder index (SOI) representative of the number of SOB episodes that occurred during the first session. If the treatment pressure should be increased based upon the derived SOI, it is increased during a second, subsequent session.

Claims (41)

1. A method for treating a patient for sleep disordered breathing using a ventilator system which includes a controller for controlling delivered pressure and a ventilator having a blower, comprising the steps of:

applying a first constant treatment pressure to a patient during a first ventilation session;

detecting and recording apnea and hypopnea events based on a flow signal derived from at least one of the speed of the blower or the current to the blower during said first ventilation session;

calculating a breathing index dependent upon the recorded apnea and hypopnea events;

determining a second ventilator pressure to be applied in a second ventilation session for said patient, said second ventilation pressure dependent upon a comparison of the breathing index to a threshold; and

delivering a second constant pressure treatment to the patient during the second ventilation session,

wherein the first ventilation session and the second ventilation session occur on different nights.

2. The method of claim 1 wherein said breathing index is an apnea hypopnea index (AHI) that represents the number of apnea and hypopnea events during said first session.

3. The method of claim 2 wherein an apnea or an hypopnea event is detected when a short term moving average of ventilation drops below a selected percentage of a long term average for a minimum period of time.

4. The method of claim 2 wherein if said breathing index is less than a minimum number, then the second constant pressure treatment is lowered from the first constant pressure treatment.

5. The method of claim 1 wherein an apnea or an hypopnea event is detected when a short term moving average of ventilation drops below a selected percentage of a long term average for a minimum period of time.

6. The method of claim 5 wherein an hypopnea event is detected when said short term average drops between a first and a second percentage of said long term average for a minimum period of time.

7. The method of claim 1 wherein, if said breathing index is less than a minimum number, then the second constant pressure treatment is lowered from the first constant pressure treatment.

8. The method of claim 1 wherein if said breathing index is greater than a minimum number, then the first constant treatment pressure is increased in the second ventilation session.

9. The method of claim 1 wherein the first constant treatment pressure is increased in the second ventilation session by an amount that is a function of the magnitude of the derived breathing index.

10. The method of claim 1 wherein the first constant treatment pressure is increased only if said breathing index was greater than a minimum number and the first constant treatment pressure is less than a maximum value.

11. The method of claim 1 wherein if said breathing index is greater than a minimum number, then the second constant pressure treatment is increased from the first constant pressure treatment.

12. The method of claim 1 wherein the second constant pressure treatment is increased from said first constant pressure treatment by an amount that is a function of the magnitude of the derived breathing index.

13. The method of claim 1 wherein the second constant pressure treatment is increased from the first constant pressure treatment only if said breathing index was greater than a minimum number and the first constant pressure treatment is less than a maximum value.

14. A system for treating sleep disordered breathing during successive treatment sessions, wherein the system provides continuous positive airway pressure during sleep, the system comprising:

a blower and a blower controller;

a mask for communicating pressurized air between said blower and a patient; and

at least one sensor for sensing signals indicative of pressure and flow, wherein the at least one sensor is a motor speed sensor or a motor current sensor, and delivering said signals to said blower controller,

said blower controller:

controls a first constant treatment pressure to be applied during a first ventilation session;

detects and records apnea and hypopnea events during said first ventilation session based on the at least one sensor;

calculates a breathing index dependent upon the recorded apnea and hypopnea events; and

delivers a second constant treatment pressure in a second ventilation session for said patient, said second constant treatment pressure determined by comparing said breathing index to a threshold,

wherein the first ventilation session and the second ventilation session occur on different nights.

15. The system of claim 14 wherein the breathing index is an apnea hypopnea index (AHI) that represents the number of apnea and hypopnea events during said first session.

16. The system of claim 15 wherein an apnea or an hypopnea event is detected when a short term moving average of ventilation drops below a selected percentage of a long term average for a minimum period of time.

17. The system of claim 15 wherein if said breathing index is less than a minimum number, then the second constant pressure treatment is lowered from said first constant treatment pressure.

18. The system of claim 15 wherein if said breathing index is greater than a minimum number, then the second constant pressure treatment is increased from the first constant pressure treatment.

19. The system of claim 15 wherein the second constant pressure treatment is increased from the first constant pressure treatment by an amount that is a function of the magnitude of the derived breathing index.

20. The system of claim 15 wherein the second constant pressure treatment is increased from the first constant pressure treatment only if said breathing index was greater than a minimum number and the first constant pressure treatment is less than a maximum value.

21. The system of claim 14 wherein an apnea or an hypopnea event is detected when a short term moving average of ventilation drops below a selected percentage of a long term average for a minimum period of time.

22. The system of claim 21 wherein an hypopnea event is detected when the short term average drops between a first and a second percentage of said long term average for a minimum period of time.

23. The system of claim 14 wherein, if said breathing index is less than a minimum number, then the second constant pressure treatment is lowered from the first constant pressure treatment.

24. The system of claim 14 wherein, if the breathing index is greater than a minimum number, then the second constant pressure treatment is increased from the first constant pressure treatment.

25. The system of claim 14 wherein the second constant pressure treatment is increased from the first constant treatment pressure by an amount that is a function of the magnitude of the derived breathing index.

26. The system of claim 14 wherein the second constant pressure treatment is increased from the first constant pressure treatment only if said breathing index was greater than a minimum number and the first constant pressure treatment is less than a maximum value.
