Abstract

A total ankle replacement system, novel surgical method for total ankle replacement, and novel surgical tools for performing the surgical method are described. The total ankle replacement system includes the calcaneus in fixation of a lower prosthesis body, thereby significantly increasing the amount of bone available for fixation of the lower prosthesis body and allowing the lower prosthesis body to be anchored with screws. The total ankle replacement system further includes a long tibial stem which can also be anchored into the tibia with, for example, screws, nails, anchors, or some other means of attachment. The novel surgical arthroscopic method allows introduction of ankle prostheses into the ankle joint through an exposure in the tibial tubercle. Various novel surgical instruments, such as a telescoping articulating reamer and a talo-calcaneal jig, which facilitate the novel surgical method, are also described.

Claims (23)

1. A method of installing a prosthesis, comprising:

forming a flat on a talus;

reaming a hole through a calcaneus and the talus;

inserting a stem of a prosthesis into the hole through the talus and the calcaneus such that the stem extends from a position above the flat formed on the talus, across a joint between the talus and the calcaneus, and into the calcaneus without contacting a tibia; and

attaching a first bearing to the stem.

2. The method of claim 1 , wherein the stem includes a Morse taper at the end that extends above the flat formed on the talus.

3. The method of claim 1 , further comprising creating an exposure at a tibial tubercle with an awl.

4. The method of claim 1 , further comprising

forming a reamed hole in the tibia;

inserting a second stem into the tibia; and

attaching a second bearing to the second stem, the second bearing being complementary to the first bearing.

5. The method of claim 4 , wherein the first bearing includes a convex dome and the second bearing includes a concave dome.

6. A method of installing a prosthesis, comprising:

forming a flat on a talus;

reaming a hole through a calcaneus and the talus;

inserting a first stem of a prosthesis into the hole through the talus and the calcaneus such that the first stem extends from a position above the flat formed on the talus, across a joint between the talus and the calcaneus, and into the calcaneus without contacting a tibia;

attaching a first bearing to the first stem;

forming a reamed hole in the tibia;

inserting a second stem into the tibia; and

attaching a second bearing to the second stem, the second bearing being complementary to the first bearing.

7. The method of claim 6 , wherein the first stem includes a Morse taper at the end that extends above the flat formed on the talus.

8. The method of claim 6 , further comprising creating an exposure at a tibial tubercle with an awl.

9. The method of claim 6 , wherein the first bearing includes a convex dome and the second bearing includes a concave dome.
