Abstract

Disclosed are devices, systems and methods for treating an incision. The device includes a generally planar tension relief module, comprising a central structure in fluid communication with the incision, wherein at least a portion of the central structure is adapted to be aligned with a longitudinal axis of the incision; and opposing adhesive structures coupled to the central structure and a flexible sealant structure comprising a lower adhesive surface and sized to seal over the tension relief module forming a sealed flow pathway. The opposing adhesive structures are adapted to be stretched from a relaxed configuration to a first tensile configuration and to return towards the relaxed configuration from the first tensile configuration into a second tensile configuration and impart a contracting force in a direction that is towards the opposing adhesive structure.

Claims (48)

1. A device for treating an incision in a patient's skin, the device comprising:

(a) a generally planar tension relief module, comprising:

(i) a central structure defining a central conduit at least a portion of which is configured to be aligned with a longitudinal axis of the incision and provides for fluid communication with the incision through the central structure; and

(ii) a first adhesive structure coupled to a first portion of the central structure and a second adhesive structure coupled to a second, opposite portion of the central structure, wherein the first and second adhesive structures are configured to be manually stretched along an axis from a relaxed configuration to a first tensile configuration and to return towards the relaxed configuration from the first tensile configuration into a second tensile configuration and impart a contracting force in a direction that is substantially aligned with the axis and towards the opposing adhesive structure to shield the incision from endogenous or exogenous stress, wherein the first and second adhesive structures attach directly to the patient's skin;

(b) a contact layer configured to be positioned directly on the patient's skin in fluid communication with the incision and configured to transmit pressure and fluid to the central structure, wherein the contact layer is configured to be positioned on the skin in direct contact with the incision below the central structure;

(c) a flexible sealant structure comprising a lower adhesive surface and a size configured to seal entirely over the entire tension relief module and the contact layer such that the flexible sealant structure forms a sealed flow pathway;

(d) a vacuum port coupled to the flexible sealant structure and aligned with an opening in the flexible sealant structure so as to be in fluid communication with the sealed flow pathway, wherein the vacuum port is configured to be coupled to a negative pressure source; and

(e) an indicator reversibly coupled to an upper surface of the opposing adhesive structures, wherein the indicator comprises opposing pull tabs.

2. The device of claim 1 , further comprising a negative pressure source coupled to the vacuum port, the negative pressure source comprising a constant force spring, a sliding seal and a fixed external profile.

3. The device of claim 1 , wherein application of negative pressure through the vacuum port evacuates the flow pathway through the opening in the sealant structure.

4. The device of claim 1 , further comprising a one-way flow valve positioned between the negative pressure source and the flow pathway, the valve adapted and configured to prevent the backflow of fluid within the flow pathway.

5. The device of claim 1 , wherein the vacuum port further comprises a pressure indicator that informs a user when negative pressure is successfully delivered to the flow pathway, wherein the indicator informs the user via at least one of visual and tactile feedback.

6. The device of claim 1 , wherein the contact layer is adapted and configured to serve as a vehicle for delivery of therapeutic agents.

7. The device of claim 1 , wherein the contact layer is a material selected from the group consisting of foam, mesh, gauze, sponge, particulate matter, mesh matrix, and porous biocompatible material.

8. The device of claim 1 , wherein the central structure comprises a material that is selected from the group consisting of silicone elastomer, polyurethane, polyisoprene, and an elastomer.

9. The device of claim 1 , wherein the adhesive structures comprise a material that is highly elastic and has memory.

10. The device of claim 9 , wherein the material is coated on a lower surface with an adhesive.

11. The device of claim 10 , wherein the adhesive comprises a silicone adhesive, rubber adhesive, or acrylic adhesive.

12. The device of claim 1 , wherein the sealant structure comprises a polyurethane sheet and the lower adhesive surface of the sealant structure comprises a hydrocolloid adhesive, silicone adhesive, rubber adhesive, or acrylic adhesive.

13. The device of claim 1 , wherein the incision is visible through the device.

14. The device of claim 1 , wherein the device has a thickness that is less than 12 mm.

15. The device of claim 1 , wherein the device prevents trapping of fluid under the device.

16. The device of claim 1 , wherein the opposing pull tabs each have an alignment marker.

17. The device of claim 16 , wherein the alignment markers approach alignment with one another when the adhesive structures are stretched towards the first tensile configuration.

18. The device of claim 1 , wherein the sealant structure comprises a plurality of smaller segments of sealant layers.

19. A device for treating an incision in a patient's skin, the device comprising:

an elongated central conduit sized and shaped to be placed over the incision such that the elongated central conduit follows a contour of the incision;

a sealant layer configured to be sealed directly to an outer surface of the patient's skin over the central conduit such that the sealant layer encloses the central conduit;

first and second elastic structures coupled to the central conduit, the first elastic structure configured to be adhered directly to the outer surface of the patient's skin on a first side of the incision and the second elastic structure configured to be adhered directly to the outer surface of the patient's skin on a second side of the incision opposite the first side;

a first pull tab attached to the first elastic structure and a second pull tab attached to the second elastic structure, the first and second pull tabs configured to pull the first and second elastic structures away from one another to stretch the first and second elastic structures so as to place the first and second elastic structures into a state of tension such that the first and second elastic structures draw opposed edges of the incision toward one another when the first and second elastic structures are adhered to the outer surface of the skin;

a first alignment member coupled to the first elastic structure and a second alignment member coupled to the second elastic structure, wherein the first alignment member moves upon stretching of the first elastic structure and the second alignment members moves upon stretching of the second elastic structure to provide a visual indication that the first and second elastic structures have been stretched;

a source of negative pressure coupled to the elongated central conduit and configured to provide negative pressure to the central conduit.

20. The device of claim 19 , wherein the first and second elastic structures are sized and shaped to be positioned entirely beneath the sealant layer when the sealant layer is sealed to the patient's skin.

21. The device of claim 19 , wherein the first and second alignment members move toward one another as the first and second elastic structures stretch such that the first and second alignment members align with one another upon a predetermined stretch of the first and second elastic structures.

22. The device of claim 19 , further comprising a one-way flow valve positioned between the source of negative pressure and the central conduit, the valve adapted and configured to prevent the backflow of fluid within the central conduit.

23. The device of claim 19 , further comprising a pressure indicator that informs a user when negative pressure is successfully delivered to the flow pathway, wherein the pressure indicator informs the user via at least one of a visual and a tactile feedback.

24. The device of claim 19 , further comprising a contact layer configured to be positioned beneath the central conduit and over the incision in direct contact with the patient's skin.

25. The device of claim 24 , wherein the contact layer is a material selected from the group consisting of foam, mesh, gauze, sponge, particulate matter, mesh matrix, and porous biocompatible material.

26. The device of claim 1 , wherein the sealant layer comprises a polyurethane sheet having a lower adhesive surface comprised of a hydrocolloid adhesive, silicone adhesive, rubber adhesive, or acrylic adhesive.

27. The device of claim 19 , wherein the incision is visible through the device.

28. A device for treating an incision in a patient's skin, the device comprising:

(a) a generally planar tension relief module, comprising:

(i) a central structure defining a central conduit at least a portion of which is configured to be aligned with a longitudinal axis of the incision and provides for fluid communication with the incision through the central structure; and

(ii) a first adhesive structure coupled to a first portion of the central structure and a second adhesive structure coupled to a second, opposite portion of the central structure, wherein the first and second adhesive structures are configured to be manually stretched along an axis from a relaxed configuration to a first tensile configuration and to return towards the relaxed configuration from the first tensile configuration into a second tensile configuration and impart a contracting force in a direction that is substantially aligned with the axis and towards the opposing adhesive structure to shield the incision from endogenous or exogenous stress, wherein the first and second adhesive structures attach directly to the patient's skin;

(b) a contact layer configured to be positioned directly on the patient's skin in fluid communication with the incision and configured to transmit pressure and fluid to the central structure, wherein the contact layer is configured to be positioned on the skin in direct contact with the incision below the central structure;

(c) a flexible sealant structure comprising a lower adhesive surface and a size configured to seal entirely over the entire tension relief module and the contact layer such that the flexible sealant structure forms a sealed flow pathway;

(d) a vacuum port coupled to the flexible sealant structure and aligned with an opening in the flexible sealant structure so as to be in fluid communication with the sealed flow pathway, wherein the vacuum port is configured to be coupled to a negative pressure source; and

(e) an indicator reversibly coupled to an upper surface of the opposing adhesive structures, wherein the indicator comprises opposing pull tabs, and wherein the opposing pull tabs each have an alignment marker.
