Abstract

The methods and apparatus for lead placement on a surface of the heart are employed using an elongated body having proximal and distal end portions. The body defines a lead receiving passageway extending between a proximal inlet and a distal outlet for receiving a lead therethrough for contact with the heart surface. The elongated body is adapted for insertion between a pericardium and an epicardial surface. At least a portion of the body may have a non-circular cross-sectional shape adapted to retain the body orientation between the pericardium and the epicardial surface.

Claims (29)

1. An apparatus for placing a lead on a surface of a heart, the apparatus comprising:

an elongated body including a proximal end portion and a distal end portion, the elongated body defining a longitudinal axis and having a length configured to permit minimally invasive access to the heart, the distal end portion of the elongated body having a contact surface generally parallel to the longitudinal axis, the distal end portion of the body further having a non-circular shape defined by an upper surface and a lower surface, the lower surface including the contact surface;

a lead receiving passageway extending through the elongated body to a distal outlet on the contact surface; and

a steering member extending through the elongated body from the distal end portion to a more proximal portion of the elongated body, the steering member configured to move the distal end portion of the body in at least one plane when force is applied to the steering member at the more proximal portion of the elongated body.

2. The apparatus of claim 1 wherein the steering member moves the distal end portion of the body when tensile force is applied to the steering member.

3. The apparatus of claim 1 wherein the steering member moves the distal end portion of the body when compressive force is applied to the steering member.

4. The apparatus of claim 1 wherein the steering member moves the distal end portion of the body when torsional force is applied to the steering member.

5. The apparatus of claim 1 wherein the steering member comprises a plurality of steering members.

The apparatus of claim 5 wherein the plurality of steering members comprises a first and a second steering member and the first and second steering members are positioned within the elongated body such that applying a tensile force to the first steering member causes the distal end of the elongated body to move in a first direction and applying a tensile force to the second steering member causes the distal end of the elongated body to move in a second direction.

7. The apparatus of claim 1 wherein the steering member extends from the distal end portion to the proximal end portion.

8. The apparatus of claim 1 wherein the steering member extends from the distal end portion to a location between the distal end portion and the proximal end portion.

9. The apparatus of claim 1 wherein the second longitudinal axis is curved relative to the first longitudinal axis and the steering member is configured to move the distal end portion of the body from a curved position to a less curved position.

10. The apparatus of claim 9 wherein the second longitudinal axis is curved relative to the first longitudinal axis in at least one plane in the range of approximately 10 degrees to 80 degrees.

11. The apparatus of claim 1 wherein the steering member is configured to move the distal end portion of the elongated body so as to increase the relative curvature of the second longitudinal axis relative to the first longitudinal axis.

12. The apparatus of claim 1 wherein the steering member comprises a wire, a fiber, a filament, or a surgical tape.

13. The apparatus of claim 1 further including a steering collar axially spaced from the distal end portion of the body, wherein the collar is in operative engagement with the steering member so that movement of the steering collar causes movement of the distal end portion.

14. The apparatus of claim 1 wherein the steering member is configured to move the distal end portion of the body in at least two planes.

15. An apparatus for placing a lead on a surface of a heart, the apparatus comprising:

an elongated body including a proximal end portion and a distal end portion, the elongated body defining a longitudinal axis and having a length configured to permit minimally invasive access to the heart, the distal end portion of the elongated body having a contact surface generally parallel to the longitudinal axis, the distal end portion of the body further having a non-circular shape defined by an upper surface and a lower surface, the lower surface including the contact surface;

a lead receiving passageway extending through the elongated body to a distal outlet on the contact surface; and

means for steering the distal end portion of the elongated body extending through the elongated body from the distal end portion to a more proximal portion of the elongated body.

16. The apparatus of claim 15 wherein the means for steering comprises a wire, a fiber, a filament, or a surgical tape.

The apparatus of claim 15 wherein the means for steering further comprises a steering collar axially spaced from the distal end portion of the body, wherein the collar is in operative engagement with the means for steering so that movement of the steering collar causes movement of the distal end portion.

18. An apparatus for placing a lead on a surface of a heart, the apparatus comprising:

an elongated body including a proximal end portion and a distal end portion, the elongated body defining a longitudinal axis and having a length configured to permit minimally invasive access to the heart, the distal end portion of the elongated body having a contact surface generally parallel to the second longitudinal axis, the distal end portion of the body further having a non-circular shape defined by an upper surface and a lower surface, the lower surface including the contact surface;

a lead receiving passageway extending through the elongated body to a distal outlet on the contact surface; and

an elongated sleeve extending from the distal end portion of the elongated body to a more proximal portion of the elongated body, wherein the elongated body is slidably received in the elongated sleeve.

The apparatus of claim 18 wherein the second longitudinal axis is curved relative to the first longitudinal axis and the sleeve is configured to retain the distal end portion in a less curved position relative to the first longitudinal axis when positioned at the distal end portion of the elongated body.

20. The apparatus of claim 18 wherein the sleeve includes a curve configured to retain the distal end portion in a curved position relative to the first longitudinal axis when positioned at the distal end portion of the elongated body.
